Book excerpt: A click-evoked Auditory Brainstem Response (ABR) was recorded on 28 normal hearing adult participants to determine the effects of different degrees of conductive hearing loss on the response properties of the ABR. Participants were divided at random into two groups: the normal hearing group and the simulated conductive hearing loss group. In the normal hearing group, baseline behavioral pure tone audiometry from 250-8000 Hz and acoustic immittance testing (tympanometry and contralateral acoustic reflex thresholds from 500-2000 Hz) were performed to ensure normal hearing and middle ear function. Then, ABR testing was performed in both right and left ears to click stimuli beginning at 80 dBnHL. Stimulus intensity was decreased in 10 dB increments until a stimulus intensity of 30 dBnHL was reached. At stimulus intensities less than 30 dB nHL, stimulus intensity was decreased in 5 dB increments until the participant's ABR threshold was determined. In the simulated conductive hearing loss group, baseline behavioral pure tone audiometry from 250-8000 Hz and acoustic immittance testing (tympanometry and contralateral acoustic reflex thresholds from 500-2000 Hz) were performed to ensure normal hearing and middle ear function. Then, 5 mm of moleskin was placed in the tubing for the ER3A insert receivers and pure tone air conduction thresholds were re-measured in each ear. For the conductive group, the ABR was recorded using the ER3A insert receivers with moleskin. The ABR was recorded in both right and left ears to click stimuli beginning at 80 dBnHL. Stimulus intensity was decreased in 10 dB increments until a stimulus intensity of 60 dBnHL was reached. At stimulus intensities less than 60 dBnHL, stimulus intensity was decreased in 5 dB increments until the participant's ABR threshold was determined. For both groups, results revealed an increase in the mean absolute latencies of waves I, III, and V as stimulus intensity decreased. In contrast, results showed a decrease in the mean peak-to-peak amplitudes for waves I-I' and V-V' as a function of stimulus intensity. Mean interpeak latency values were consistent across stimulus intensities. Mean absolute latency values for all waves were greater for the simulated hearing loss group than the normal hearing group. In contrast, the mean peak-to-peak amplitude values for waves I-I' and V-V' were smaller for the simulated hearing loss group than the normal hearing group. These results were compared to previous data collected in the ABR literature on both normal hearing and conductive hearing loss participants. Results for both groups in the current study were in good agreement with the ABR literature as seen in the mean ABR values and the variability represented by standard deviation measurements. The data collected from this study will be used to develop a parametric approach to generating simulated responses for the commercially available Auditory Brainstem Response (ABR) recording simulator, ISAO by Intelligent Hearing Systems. This data will serve as basis for developing functions of response characteristics such as peak latency and amplitude as well as recording parameters like intensity, rate and stimulus characteristics. Further research should be conducted to determine the effects of other stimulus parameters on the ABR. These stimulus parameters include rate, polarity, and frequency.

Approximately 95% of those people have partial losses, with varying degrees of residual hearing. These percentages are similar in other developed countries. What changes in the function of the cochlea or inner ear cause such losses? What does the world sound like to the 19 million people with residual hearing? How should we transform sounds to correct for the hearing loss and maximize restoration of normal hearing? Answers to such questions require detailed models of the way that sounds are processed by the nervous system, both for listeners with normal hearing and for those with sensorineural hearing loss. This book contains chapters describing the work of 25 different research groups. A great deal of research in recent years has been aimed at obtaining a better physiological description of the altered processes that cause sensorineural hearing loss and a better understanding of transformations that occur in the perception of those sounds that are sufficiently intense that they can still be heard. Efforts to understand these changes in function have lead to a better understanding of normal function as well. This research has been based on rigorous mathematical models, computer simulations of mechanical and physiological processes, and signal processing simulations of the altered perceptual experience of listeners with sensorineural hearing loss. This book provides examples of all these approaches to modeling sensorineural hearing loss and a summary of the latest research in the field.

Book excerpt: We investigated the contribution of auditory inputs to balance control in healthy young adults and older adults with normal hearing by simulating hearing loss, as well as in older adult with hearing loss by testing with and without hearing aids. Twenty healthy young adults with normal hearing, twenty older adults with normal hearing, and twenty older adults with hearing aids completed single- and dual- tasks consisting of a standardized audiology test (BKB-SIN) and maintaining standing balance in response to surface translations. Participants performed an auditory task of repeating back sentences from a standardized audiological test, the Bamford-Kowal-Bench Speech-In-Noise (BKB-SIN), played through wireless noise-cancelling headphones under randomized normal hearing and simulated hearing loss conditions or through surrounding speakers under hearing aid or no hearing aid condition. Simulated hearing loss was achieved using Adobe Audition software and a FFT logarithmic curve to manipulate sound volume and frequencies of standardized sentences according to age-related moderate hearing loss documented in literature. Backward surface translation perturbations inducing a forward loss of balance were synchronized with the auditory task and presented randomly at three levels (0m/s2, 2m/s2, and 5 m/s2). Primary outcome measures included: maximum Center of Pressure - Center of Mass (COP-COM) distance in response to perturbation during the first compensatory step, reaction time for initiating the first compensatory step, number of steps after loss of balance, and performance on the BKB-SIN. Repeated measures ANOVA were conducted for each dependent variable with respect to perturbation level and auditory condition. Results show reaction time decreases, maximum COP-COM distance increases, and number of steps increases as perturbation level increases across all groups. BKB-SIN scores and reaction time were significantly worse under the simulated hearing loss condition. Hearing aids significantly improved BKB-SIN scores, but not balance scores. Hearing loss affects reactive balance control, particularly while simultaneously attending to auditory tasks. Older adults maintain the ability to initiate compensatory steps, but they require an increase number of steps to regain balance. Individuals with hearing loss may be at greater risk of falling compared to individuals with normal hearing due to age-related cognitive and neurodegenerative changes associated with hearing loss.
