Book excerpt: Prevention and treatment of type 2 diabetes mellitus (T2DM), relies heavily on self-care behaviours such as dietary modification, physical activity, and medication adherence. Ability to perform these self-care behaviours depends, at least in part, on executive function (EF). Recent evidence suggests a correlation between T2DM and impaired cognitive function, including EF. Given the importance of EF for regulation of behaviours, and the importance of self-care in diabetes management, attenuated EF would represent a potential barrier to proper disease management. Thus the objective of Study 1 was to examine the association between T2DM and EF through meta-analytic techniques. Medline, PsychoInfo, and Scopus, as well as article references, were used to identify studies comparing individuals with T2DM to a control population. Effect size was calculated using cohen's d and random effects modeling, and the potential impact of moderators (age, sex, and T2DM duration) were examined. It has been reported that improving fitness levels exhibits a positive effect on cognition (Colcombe & Kramer, 2003; Colcombe et al., 2004). Older adult females 60-75 years of age who were moderately to highly fit performed better on working memory tasks after an acute bout of moderate or high-intensity aerobic exercise than before exercise. Participants included healthy females (N = 11) who were not on medications for cognitive impairment and who did not show signs of dementia or Alzheimer's disease. A submaximal walking exercise test was administered to assess fitness levels, as well as modified flanker tasks and d2-tests, to measure executive functioning in the brain. Results indicated that older adults females, 60-75 years of age showed improved scores on the modified flanker task and d2-tests after a session of moderate or high-intensity aerobic exercise. These findings suggest that an acute bout of exercise, regardless of intensity, can improve tests of executive control.

Book excerpt: The purpose of the study was to identify the effect of a single bout of moderately intense aerobic exercise on measures of executive function in young adults with ADHD. Thirty-two young adults between the ages of 18 and 25 years old were randomly assigned to either an acute exercise group or a control group. The exercise protocol was designed to address the methodological shortcomings in the exercise and executive function literature, and involved cycling on a stationary bike at moderate intensity for 30 minutes. Participants completed executive function tasks related to set shifting, working memory, and inhibition prior to and immediately following the treatment session. Results of the study reveal that acute exercise facilitated performance on the Stroop Color-Word task when compared to sedentary controls. However, unlike previous research on healthy adults, no improvements on the other measures of executive function were observed. These findings suggest that young adults with ADHD may specifically benefit from acute exercise on aspects of executive function related to the Stroop Color-Word measure. In conclusion, further research on this topic should continue to investigate this population, as well as consider the relationship between acute exercise and additional neuropsychological measures of inhibition, set shifting, and working memory in order to better determine the effects on each construct.

To date, exercise has been acknowledged as an active and efficient way to prevent and treat a variety of chronic diseases. However, disputes exist in the current evidence, including but not limited to the lack of rationales in exercise program design, improper implementation of intervention without following basic training principles, and conflicting opinions towards the optimal exercise prescriptions for different chronic conditions, to name a few. Some of these questions may stay hard to answer, but we look forward to new perspectives, advanced methods/measures, innovative findings, and exploratory solutions to understand exercise therapy in chronic disease management and health promotion.

Book excerpt: A growing body of literature suggests that higher levels of physical activity and cardiorespiratory fitness, as well as an alternative treatment such as low level laser therapy (LLLT), have beneficial effects on cognition, while sedentary lifestyle-induced obesity and vascular risk negatively influence cognition. However, the relationship between cardiorespiratory fitness and vascular risk on cognitive function and the effect of an acute bout of very vigorous aerobic exercise (acute exercise) on cognitive performance has not yet been investigated. Additionally, the effects of combined acute exercise and LLLT treatments on cognitive performance have not yet been characterized. Therefore, the goal of this dissertation was to conduct a series of three research studies ranging from a cross-sectional exploratory study about the association of cardiorespiratory fitness, obesity, and vascular risk on cognitive function, to an experimentally designed study that compared the effects of acute exercise, LLLT, and the combination of these two treatments on cognitive performance in young adults. Study1 examined the relationship among cardiorespiratory fitness (maximal oxygen consumption, VO2max), obesity indices (body mass index, BMI; waist circumstance), and vascular risk (C-reactive protein; CRP). Cognitive function included crystalized intelligence (Kaufman Brief Intelligence Test; KBIT), executive functions of inhibition (Stroop test), switching (Trail making test; TMT), attention (Psychomotor Vigilance Task; PVT), and working memory (Delayed-Match-to-Sample, DMS). Study 2 determined the effect of acute exercise on cognitive performance including executive response inhibition (Stroop test) and response switching (TMT) and brain-derived neurotrophic factor (BNDF). Study 3 compared the effectiveness of LLLT and acute exercise on cognitive performance, which included attention (PVT) and working memory (DMS). Results demonstrated there was a beneficial effect of physical exercise-induced improvements in cardiorespiratory fitness on vascular risk and cognitive functions particularly in working memory and inhibitory control (Study1). Acute exercise improved performance in inhibitory control and increased the BNDF level compared to the control condition, suggesting the acute exercise-induced the increase in BDNF level may be at least in part of mediating the cognitive performance improvement (Study 2). All three conditions (acute exercise, LLLT, or the combination) improved performance in attention and working memory, as measured by reaction time and response accuracy, when compared to a control group. Specially, the combined group showed a trend of greater improvement in attention and worming memory performance (Study 3). Taken together, the results of this research series suggest that acute exercise and LLLT can improve cognitive performance, which is also mediated by health indices including cardiorespiratory fitness, obesity, and vascular risk. It is anticipated that these findings will make substantive contributions to the empirical literature concerning the beneficial effect of exercise and LLLT on cognitive health in young adults, given the current paucity of research.
