Book excerpt: Resistance training and protein supplementation have been shown to increase muscle mass and strength, and thus may be an effective method to combat muscle wasting (sarcopenia) that occurs with aging. Thirty-six males (62.5 yrs, 177 cm and 95.25 kg) were randomly assigned to ingest either whey isolate protein, casein protein or carbohydrate while participating in a 12-week high intensity resistance training program. Resistance training increased both upper and lower body strength, body mass and lean muscle mass while decreasing body fat percentage. However, no effects from training were observed on serum anabolic hormones and indirect markers of satellite cell activation. A trend towards significantly higher lower body strength was observed in the whey isolate (p=0.053) and casein (p=0.086) groups, respectively, compared to carbohydrate placebo group. The present study suggests protein supplementation in conjunction with high intensity resistance training may be important in combating sarcopenia.

Book excerpt: Aging is associated with gradual loss of muscle mass, termed sarcopenia, which often leads to progressive disability and loss of independence. Though resistance exercise has shown to be an effective method at reducing the rate of age-related muscle loss and decline in force output; when combined with well known muscle building agents [such as creatine monohydrate (CrM)], these training-induced improvements are enhanced. To explore this idea further, a double-blind, randomized, controlled trial was conducted on 20 males aged between 55-75 yrs at Baylor University, Waco TX. Participants were randomly assigned to consume either CrM [20g/d CrM + 5g Carbohydrate (CHO) x 7 days, then 5g/d CrM +5g CHO x 77 days] or carbohydrate placebo (20g/d CHO x 7 days, then 5g/d CHO x 77 days) while participating in a high intensity resistance training program (3 sets x 10 repetitions at 75% of 1RM), 3 days per week for 12 weeks. Testing sessions were complete prior to, 4 weeks, 8 weeks and 12 weeks post resistance training and supplementation. Each testing session included body composition measurement as determined by Dual Energy X-Ray Absorptiometry (DEXA), muscle strength measurement as determined by 1 repetition maximum (RM) on leg press and bench press, blood collection and vastus lateralis muscle biopsy. The blood serum was analyzed for insulin-like growth factor 1 (IGF-1), free testosterone and hepatocyte growth factor (HGF) and the muscle tissue for phophorylated mesenchymal-epithelial transition factor (c-Met), myogenic regulatory factors (MyoD, myogenin, Myf-5, MRF-4), and total myofibrillar protein. Book excerpt: PURPOSE: The purpose of this study was to investigate the effects of 12 weeks of protein supplementation and Autoregulatory Progressive Resistance Exercise (APRE) training on measures of body composition, muscular performance, and free testosterone in middle-aged men. METHODS: Untrained males (n = 35; 43.3 ± 6.6 years) participated in this study. Lean mass (LM) and fat mass (FM) were measured via Dual Energy X-ray Absorptiometry (DEXA), while strength was measured utilizing one repetition maximum (1RM), endurance measured using maximum repetitions completed (REPS) at 75% 1RM for the bench and leg press, and free testosterone was measured using changes in serum values. Subjects were randomly placed into one of four groups: protein (PO), APRE, protein plus APRE (PAPRE), or control (CON). Subjects repeated testing for the DEXA, 1RM, REPS, and blood draw every four weeks for 12 weeks. Both PO and PAPRE groups ingested 25g of supplemental protein twice daily. Subjects in the exercise groups completed a resistance training program, 3 days per week, for 12 weeks. RESULTS: Analysis revealed a significant time interaction (p .01) and a trend towards significance for group time interaction (p = .08) between baseline and week 12 for LM, with the APRE and PAPRE groups showing a significant increase following the 12-weeks. Book excerpt: Physiological responses after maximal and submaximal exercise are routinely monitored in a plethora of diseases (e.g. cardiovascular diseases, cancer, diabetes, asthma, neuromuscular disorders), and normal populations (e.g. athletes, youth, elderly), while slower or irregular post-exercise recovery usually indicates poor health and/or low fitness level. Abnormal post-exercise recovery (as assessed via blunted post-exercise heart rate dynamics) helps to predict the presence and severity of coronary artery disease, while differences in recovery outcomes in athletes might discriminate between fit and unfit individuals. Disturbances in post-exercise recovery might be due to acute or persistent changes in: (1) adaptive responses mediated by the autonomic nervous system and vasodilator substances, (2) cellular bioenergetics, and/or (3) muscular plasticity. Preliminary evidence suggests possible role of time-dependent modulation of nitric oxide synthase and adenosine receptors during post-exercise recovery, yet no molecular attributes of post-exercise recovery are revealed so far. Currently several markers of post-exercise recovery are used (e.g. heart rate measures, hormone profiles, biochemical and hematological indices); however none of them meets all criteria to make its use generally accepted as the gold standard. In addition, recent studies suggest that different pharmacological agents and dietary interventions, or manipulative actions (e.g. massage, cold-water immersion, compression garments, athletic training) administered before, during or immediately after exercise could positively affect post-exercise recovery. There is a growing interest to provide more evidence-based data concerning the effectiveness and safety of traditional and novel interventions to affect post-exercise recovery. The goals of this research topic are to critically evaluate the current advances on mechanisms and clinical implications of post-exercise recovery, and to summarize recent experimental data from interventional studies. This knowledge may help to identify the hierarchy of key mechanisms, and recognize methods to monitor and improve post-exercise recovery in both health and disease.

Book excerpt: Individuals at risk for developing cardiovascular disease (CVD) can reduce risk factors through diet and exercise before resorting to drug treatment. Regular exercise can reduce many risk factors associated with CVD, primarily lowering cholesterol, triglycerides and body fat mass, while raising high density lipoproteins. Ingesting vegetable versus animal protein has also been shown to have beneficial effects on various risk factors. The effect of a combination of resistance exercise training with vegetable-based (soy) versus animal-based (whey) protein supplementation on CVD risk reduction has received little study. Purpose. To study the effects of 12 weeks of resistance exercise training with soy versus whey protein supplementation on changes in strength, body composition, serum lipid levels, C-reactive protein (inflammatory biomarker), and human serum paraoxonase (HDL-associated antioxidant enzyme) in overweight, hypercholesterolemic men. Methods. Twenty-eight overweight male subjects (BMI 25-30) from the community with serum cholesterol >200 mg/dl were randomly divided into 3 groups: resistance training + carbohydrate supplement as placebo (n=9); resistance training + soy supplementation (n=9); and resistance training + whey supplementation (n=10). Results. After twelve weeks of resistance exercise training all 3 groups had significant gains in strength averaging 47% in all major muscle groups, with no difference among groups. Serum cholesterol, waist-to-hip ratio and percent body fat decreased significantly in all 3 groups an average of 6%, 2%, and 8% respectively, again with no difference between groups. Twelve weeks of resistance exercise training brought about no significant change in hs-C-reactive protein and no effect between groups. Human serum paraoxonase, as measured by arylesterase activity, did not differ significantly before versus after twelve weeks of resistance exercise training or between groups. Conclusions. Participation in a 12 week resistance exercise training program significantly increased strength and decreased CVD risk factors in overweight, hypercholesterolemic men with no added benefit of protein supplementation.

Book excerpt: Nutrient timing is a popular nutritional strategy that involves the consumption of combinations of nutrients-primarily protein and carbohydrate-in and around an exercise session. Some have claimed that this approach can produce dramatic improvements in body composition. It has even been postulated that the timing of nutritional consumption may be more important than the absolute daily intake of nutrients. The post-exercise period is widely considered the most critical part of nutrient timing. Theoretically, consuming the proper ratio of nutrients during this time not only initiates the rebuilding of damaged muscle tissue and restoration of energy reserves, but it does so in a supercompensated fashion that enhances both body composition and exercise performance. Several researchers have made reference to an anabolic "window of opportunity" whereby a limited time exists after training to optimize training-related muscular adaptations. However, the importance - and even the existence - of a post-exercise 'window' can vary according to a number of factors. Not only is nutrient timing research open to question in terms of applicability, but recent evidence has directly challenged the classical view of the relevance of post-exercise nutritional intake with respect to anabolism. Therefore, the purpose of this paper will be twofold: 1) to review the existing literature on the effects of nutrient timing with respect to post-exercise muscular adaptations, and; 2) to draw relevant conclusions that allow practical, evidence-based nutritional recommendations to be made for maximizing the anabolic response to exercise. Book excerpt: Resistance exercise (RE) is purported to induce muscle protein accretion primarily by stimulating muscle protein synthesis (MPS), with its effect potentiated by providing a protein or amino acid (AA) supplement post exercise. Glutamine, a conditionally essential AA, is increasingly recommended to improve exercise performance, but it is poorly soluble and unstable in sports drinks. This limitation can be overcome by combining L-glutamine with L-alanine to form a dipeptide (AlaGln). The first study demonstrated that AlaGln supplementation post resistance exercise significantly reduced the phosphorylation of AMPK and NF-kB p65 without activating intracellular signals for MPS. This study also showed that whey protein (WP) activated the mTOR signaling pathways without affecting signaling proteins that controls muscle protein breakdown (MPB). The results provide indirect evidence that AlaGln blocks MPB via suppressing the activation of AMPK-FOXO3A and NF-kB p65, while WP promotes MPS. Recent research suggests that adding carbohydrate (CHO) to a protein supplement post exercise can produce greater protein accretion and subsequently increase the magnitude of resistance training adaptation relative to protein supplementation alone. Early studies from our laboratory found that co-ingestion of CHO and protein, as compared with CHO or protein intake individually, had a greater effect on the activation of anabolic signaling proteins. However, the effect of CHO plus protein on MPS and muscle adaptation is controversial. The second study demonstrated that adding CHO to a protein supplement (CP) accelerated MPS via activating the mTOR-signaling pathway in comparison with placebo (PLA) and WP during early exercise recovery, but CP did not affect signaling proteins that regulate MPB. The third study found that CP enhanced muscle strength relative to PLA and WP. The greater strength development in CP appeared due to increased myofibrillar protein content. Increased muscle hypertrophy occurred without an increase in myonuclei suggesting satellite cell activation was not required for muscle fiber development. Taken together, the results of this series of studies suggest that 1) AlaGln inhibits MPB following acute RE; 2) Adding CHO to a protein supplement accelerates muscle recovery by stimulating MPS, and increases the magnitude of muscle strength by accumulating more myofibrillar protein in comparison with PLA and WP.

The use of caffeinated beverages and supplements is common among collegiate and recreational athletes with 85% of people aged 18-24 consuming caffeine. The use of caffeine may influence the adaptations to resistance training by altering protein metabolism. PURPOSE: To determine if caffeine intake will alter markers of protein metabolism, strength gains, or body composition in response to resistance training in resistance trained young adult males during an 8-wk training protocol. METHODS: Resistance trained males (n=20, 22.1±2.4 y; 178.1±7.5 cm; 82.8±15.7 kg) were randomly assigned into a caffeine (CAF; n=10) or placebo supplementation group (PLA; n=10).
