Chronic obstructive pulmonary disease (COPD) is a disease state characterized by airflow limitation that is not fully reversible. The airflow limitation is usually progressive and associated with an abnormal inflammatory response of the lungs to noxious particles or gases. Clinically, it is characterized by complaints of cough, sputum production, and/or dyspnoea, and diagnosed by a significantly reduced Tiffeneau index. It was the fourth leading cause of death in the United States in 1996, exceeded only by heart attacks, cancers and stroke. Hence, COPD is a major global health problem and its burden on society is growing rapidly. For these reasons, COPD was chosen as a study population in the present project.

Book excerpt: Chronic obstructive pulmonary disease (COPD) is a progressive, debilitating and irreversible condition characterized by dyspnea or breathlessness upon exertion. Comprehensive pulmonary rehabilitation, which includes exercise training, has been shown to improve exercise tolerance, health-related quality of life and dyspnea. Long-term studies of rehabilitation outcomes show that these benefits tend to diminish over time. This study of patients in a pulmonary rehabilitation program examined (1) the physical and psychological factors related to exercise maintenance and (2) the role of exercise maintenance on long-term functional outcomes. One hundred sixty-four patients with chronic lung disease completed an 8-week rehabilitation program and were randomized to a 12-month maintenance program (n = 83) or a standard care condition (n = 81). Assessments of exercise, health-related quality of life (Quality of Well-Being Scale), dyspnea (UCSD Shortness of Breath Questionnaire and Dyspnea Scale of the Chronic Respiratory Questionnaire), exercise tolerance (6-minute walk test and Perceived Breathlessness), depression (CES-D), self-efficacy for walking, and disease severity were taken at pre- and post-rehabilitation, and at 6, 12, 24 months and 5-8 years after completing rehabilitation. Results show that the maintenance intervention did not prevent attrition from exercise as walking declined similarly across both groups by 24 months after rehabilitation. Patients in the maintenance condition, on average, walked 90 minutes per week during the 12-month intervention. Hierarchical regression analyses show that disease severity at post-rehabilitation was a statistically significant and unique predictor of overall exercise adherence during the 12 months following rehabilitation after controlling for health care utilization, age, and self-efficacy for walking. Patients with higher exercise participation had maintained better levels of self-efficacy, tolerance for dyspnea, and dyspnea during activities of daily living by 12 months after rehabilitation as compared to those who walked less. Findings support the presence of a relationship between health status and exercise maintenance among patients who complete pulmonary rehabilitation. Exercise maintenance in the form of walking may play an important role in the long-term outcomes of pulmonary rehabilitation for some patients. Maintenance interventions including regular medical follow-up need to be tailored to the patient's health status to further optimize long-term benefits of pulmonary rehabilitation.

OBJECTIVE: To assess the effect of ACTs before and after exercise training on COPD patients participating in PR METHODS: Thirty-five patients with COPD from three PR centers were randomly assigned to either: 1) usual care group, (PR - Exercise Training), n = 12, 2) usual care plus high-frequency chest wall oscillation (HFCWO, The Vest[trademark symbol]) n =11, or 3) usual care plus vibratory positive expiratory pressure (VPEP, The Acapella®, n=12. Airway clearance therapy (HFCWO or VPEP) was given for 10 min before and after the exercise for PR patients assigned to those treatment arms. Assessments of Pulmonary Function Status Scale (PFSS), Six-Minute Walk (SMW) test, Physical Activity Stage of Change Questionnaire (PASCQ), Forced Expiratory Volume in one second (FEV1) and Peak Expiratory Flow (PEF) were measured at baseline and completion of PR. Sputum quantity was measured before exercise, during exercise, and after exercise. Book excerpt: Endurance and strength training may both lead to improvements in physical function and health-related quality of life (HRQL) in chronic obstructive pulmonary disease (COPD) patients. The few studies that have compared endurance training (ET) to strength training (ST) have produced conflicting results. PURPOSE: To compare changes in physical function, HRQL, and lean mass in COPD patients completing both an ET and ST program. METHODS: Eleven mildly diseased patients completed an ET program, and subsequently, a ST program. RESULTS: Pulmonary function remained stable over the five year period. ET and ST increased six-minute walk distance. ET, but not ST, increased self-reported physical function, chair rise time, SF-36 physical component score, and Chronic Respiratory Disease Questionnaire (CRDQ) fatigue score. ST, but not ET, increased stair climb time. ET and ST failed to change SF-36 mental component score; CRDQ dyspnea, emotion, or mastery scores; and arm or leg lean mass. CONCLUSION: Both training methods will improve objective measures of physical function. However, an ET program results in significant improvements in self-reported physical function and HRQL, whereas ST does not.
