Book excerpt: ABSTRACT: The purpose of the study was to determine if a behavioral modification program based on the Social Cognitive Theory would have an impact on sedentary time, cardiovascular disease (CVD) risk, and self-efficacy (SE for PA) in older adults from a local YMCA. Participants were placed in either the experimental (n = 12) or the control (n = 9) group. The experimental group received group counseling, goal setting, and education on physical activity and sedentary time. The control group received nutrition information. The study lasted for 6 weeks total with weeks 1 and 6 being the pre and post testing, and weeks 2-5 being the group sessions. Six 2x2 mixed factorial ANOVAs were conducted with group being the independent group factor and time being the repeated measures factor. Book excerpt: Background: Women face significant threat from cardiovascular disease (CVD), and this threat increases with age. Physical activity (PA) has been shown to reduce the risk of CVD in women; furthermore, self-efficacy has been identified as a strong predictor of whether a woman will engage in this health behavior. Purpose: To explore the relationship between women's risk for CVD and self-efficacy beliefs in relation to the behavior of physical activity. Methods: The Health Promotion Model was used to guide the retrospective, cross-sectional study. The sample (n=120) included participants from the Happy Hearts Health Promotion Program (HHHPP), an intervention study aimed at improving cardiovascular risk for CVD. Baseline data was analyzed using descriptive, bivariate, and multivariate analyses. Results: On average, the women in this sample were 68 years old, overweight, highly educated, and highly active. Bivariate analyses revealed no significant correlation between cardiac risk and self-efficacy, but a positive correlation was identified between BMI and frailty, and a general decline in walking activity (6MWT) with increasing age. In regression analysis, frailty emerged as a significant predictor of the total and subtypes of Multidimensional Self-Efficacy for Exercise (R2= 2.9%-11%). Moderate self-reported physical activity and the 6MWT were the only activity measures to significantly predict any of the self-efficacy variables. Discussion: Nurses are ideally positioned to lead health promotion initiatives, such as evidence based exercise self-efficacy enhancing interventions and educational strategies for aging women that focus on the importance of physical activity in reducing the risk of CVD and frailty. As a holistic approach is central to nursing, it is important for nurses to incorporate the components of self-efficacy, frailty, and CVD risk into their assessments and ongoing care of aging female patients. Significance: This study provides novel research evidence regarding the influence of frailty on the exercise self-efficacy of aging women at risk for heart disease; furthermore, the study findings support emerging data related to the relationship between BMI and frailty. This research establishes foundational evidence for future nursing research aimed at strengthening the exercise self-efficacy beliefs of aging women with the goal of improving physical activity to reduce CVD and frailty risk.

Available in PDF, EPUB and Kindle. Book excerpt: The ultimate goal of this project was to gain a better understanding of the role sedentary behavior plays in determining the health of adults. Three studies were completed that were designed to answer questions relating to the association between sedentary behavior and various health outcomes in older adults, how the interaction between sedentary behavior and physical activity affects these associations, a description of time spent in various sedentary behavior domains by older adults, and how age and participation in physical activity affects the dose-reposonse relationship between sedentary behavior and health outcomes. We found that, in older adults, time spent in sedentary behavior is positively associated with biomarkers linked to increased risk of cardiovascular disease and type II diabetes even in those who are active enough to meet the physical activity recommendations for public health. Also, the manner in which sedentary behavior is accumulated is an important predictor of physical function in older adults, whereby taking fewer breaks and spending time in longer bouts of sedentary behavior is associated with worse performance on tests of function. Lastly, the presence of a linear relationship between sedentary behavior and odds of having metabolic syndrome suggests that there is no threshold of sedentary behavior that needs to be met before the risk of harm begins to rise for that particular outcome. Taken together, the findings from these three studies contribute to the growing body of evidence to suggest that the amount of time adults spend in sedentary behavior can have a significant impact on their health. What we found confirms the recommendation by the Physical Activity Guidelines for Americans to "avoid inactivity" in addition to accumulating adequate moderate to vigorous activity, because it appears that increasing amounts of total time and time spent in longer bouts of sedentary behavior are harmful, and participation in physical activity does not protect against this harm. Hopefully, these findings will foster interventions to reduce sedentary behavior and meaningfully contribute to future revisions of the Physical Activity Guidelines with more specific information concerning strategies to avoid excessive sedentary behavior in order to promote health.

Book excerpt: "Sedentary behavior (SB) is emerging as an independent risk factor for cardiovascular disease and type 2 diabetes (Katzmarzyk, 2010, Owen et al., 2010), and is a target for intervention. This study examined effects of a Social Cognitive intervention to increase self-efficacy (SE) for reducing SB and increasing PA. Female members (Mage=58.5, SD=12.5 yrs) from seven weight loss support clubs were enrolled in a 6-week intervention (n=40) or waitlisted (n=24) based on club randomization. The intervention, delivered via group sessions and email, used mastery feedback from goal-setting activities along with behavioral cues and modeling to reduce SB and increase steps. Quantitative and qualitative process evaluation data were collected throughout. PA and SB were measured by accelerometers. SE (to reduce SB, to increase light & moderate PA) were measured pre, mid, and post. Repeated-measures MANOVA found no significant change over time or Group x Time interaction for behavior. A significant effect for time was noted for SE to reduce SB (F=3.34, p
