Book excerpt: Almost a third of US children ages 2-19 are deemed overweight or obese, and part of the problem is the habitual decision to consume high calorie, low nutrient foods. We propose that the school lunchroom provides a 'teachable moment' to engage children in making healthful choices. We conduct a field experiment with over 1,500 participants in grades K-8 and evaluate the impact of small non-monetary incentives on the selection of milk in the school lunchroom. At baseline, only 16% of children select white milk relative to 84% choosing chocolate milk. We find a significant effect of incentives, which increase white milk selection by 2.5 times, to 40%. One concern with incentives is that they may decrease intrinsic motivation to eat healthy, called 'crowd-out of intrinsic motivation.' However, we do not find evidence of 'crowd-out'; rather, we see some suggestive evidence of the positive habit forming effect of incentives.

A common complaint has been that our system is unfair, a concern that comes from providers as well as consumers and from both public and private sectors. This bookaims to develop a framework for measuring various health reform proposals and current trends in relation to underlying social values in the U.S. In so doing, it seeks to expose social values that are at stake in current and future changes. At the heart of this book is the question: If the current situation is perceived to be unfair, will the future improve or worsen our situation? Targeted primarily for policy makers in government and throughout the health sector, this book will also stimulate graduate students in the health and social sciences. A wide audience will find the book of interest in providing a different perspective as to how current trends and specific legislative and policy proposals stack up against the authors' ten benchmarks of fairness. The book makes very limited use of illustrations, although tables provide understandable summaries of the concepts and their application in scoring proposals and trends. References are ample and pertinent. This is a stimulating and provocative work that shifts our focus to the collective social values at stake in an evolving health system. The book argues that our current system is unfair both in comparison to our values and the approaches taken throughout the rest of the industrialized world. Its sobering message is that the gap between what we value and what we have will likely increase until we recognize what is at stake.

Book excerpt: Children's replacement of milk with sugar-sweetened beverages (SSB) is associated with lower intake of calcium, vitamin D, vitamin A, folate, vitamin B-12, and magnesium. Research indicates a relationship between this consumption pattern and obesity, osteoporosis, nutrient deficiency and dental caries. This study is a secondary analysis of the cross-sectional dietary data from the Healthy Youth and Parent Program (HYPP). HYPP, a family behavioral intervention produced significant increases in calcium intake among the experimental group as compared to controls (N = 124). This secondary analysis analyzed beverage consumption, calcium-rich beverages and sugar sweet beverages, to test if the experimental intervention resulted in differential change from baseline to 3-months. Beverages included milk (whole, reduced and fat free), 100% juices, SSB (juice and soda) and calcium-fortified juices (calcium-100% juices and calcium-not 100% juices). This study evaluated if the increase in calcium intake in HYPP participants was due to an increase intake of milk and explored the type of milk that caused significant differences. Results indicate that total milk consumption increased in the experimental group relative to controls (p = .004). Consumption of whole milk decreased (p = .019) with a corresponding increase in reduced (p = .002) and fat-free milk (p = .036). No effect was observed for juices or SSB. This analysis supports parent-child interventions as a means of increasing the consumption of calcium rich beverages including reduced and fat-free milk in children. Dietary modification through such interventions should be considered as a means to successfully decrease the childhood obesity epidemic and potential osteoporosis rise among children in the United States.

Being overweight as a child increases one's risk of developing obesity as an adult and is a health risk for developing a variety of different diseases. The goal of this education program was to educate parents on how low-fat milk and limited juice consumption can help children maintain a healthy weight. Parents of overweight child participants in the Women, Infants and Children (WIC) Program were educated on the benefits of consuming low-fat milk and limiting daily juice consumption. When the participants returned six months later for their next WIC appointment the type of milk and the number of ounces of juice consumed by their children was again assessed. The data indicated that nutrition education is effective in improving parental feeding practices.

Book excerpt: Decades of research have demonstrated that the parent-child dyad and the environment of the familyâ€"which includes all primary caregiversâ€"are at the foundation of children's well- being and healthy development. From birth, children are learning and rely on parents and the other caregivers in their lives to protect and care for them. The impact of parents may never be greater than during the earliest years of life, when a child's brain is rapidly developing and when nearly all of her or his experiences are created and shaped by parents and the family environment. Parents help children build and refine their knowledge and skills, charting a trajectory for their health and well-being during childhood and beyond. The experience of parenting also impacts parents themselves. For instance, parenting can enrich and give focus to parents' lives; generate stress or calm; and create any number of emotions, including feelings of happiness, sadness, fulfillment, and anger. Parenting of young children today takes place in the context of significant ongoing developments. These include: a rapidly growing body of science on early childhood, increases in funding for programs and services for families, changing demographics of the U.S. population, and greater diversity of family structure. Additionally, parenting is increasingly being shaped by technology and increased access to information about parenting.
