Book excerpt: Whilst a substantial amount of research has demonstrated the negative impact of sleep deprivation on human performance, studies looking at recovery of performance following sleep deprivation are limited. The current study examined recovery of performance following two nights deprivation where recovery opportunity was restricted to 6 hours each night over five nights. Following sleep deprivation it was hypothesised that (a) performance would not recover to baseline levels, and (b) different performance tasks would vary in how they recovered. Eleven healthy subjects aged 19 to 24 years were recruited and remained in the laboratory for nine consecutive nights: an adaptation and baseline night with 9 hours time in bed (TIB) from 2300 to 0800; two nights of sleep deprivation (SD) culminating in 66 hours of sustained wakefulness (SW); followed by five nights recovery sleep with 6 hours TIB from 0200 to 0800. During the experimental period subjects were tested on performance tasks every two hours for approximately 30 minutes. These included the Psychomotor Vigilance Task (PVT), measuring sustained attention, and three other tasks including Stroop, measuring cognitive interference; Serial Addition, measuring mental arithmetic and Six Letter Search, measuring working memory. Across the recovery period PVT performance remained impaired, stabilising at lower than baseline levels and failing to recover. In contrast, recovery of the other tasks occurred by the second recovery day. The PVT is highly sensitive to the effects of sleep loss, and is considered to be a basic indicator of sleepiness. Therefore, results indicate that even after five restricted recovery sleep opportunities subjects were still sleepy, albeit at stabilised levels. For the remaining more complex tasks, results may indicate a functional recovery with limited sleep. However, learning effects may have influenced the results on these tasks. Overall, results suggest that more complex tasks may recover more quickly from the effects of sleep loss.

In 2003 the organization overseeing graduate medical education adopted common program requirements to restrict resident workweeks, including limits to an average of 80 hours over 4 weeks and the longest consecutive period of work to 30 hours in order to protect patients and residents from unsafe conditions resulting from excessive fatigue. Resident Duty Hours provides a timely examination of how those requirements were implemented and their impact on safety, education, and the training institutions. An in-depth review of the evidence on sleep and human performance indicated a need to increase opportunities for sleep during residency training to prevent acute and chronic sleep deprivation and minimize the risk of fatigue-related errors. Although considerable data show that sleep deprivation alters many aspects of behavior, little is known about changes in the brain substrate underlying the behavioral effects, and even less is known about the cerebral effects of recovery sleep. The overarching objective of this study is to investigate the effects of 2 full nights of sleep loss (66 hours total) and 2 full nights of recovery sleep on cognitive performance and brain function. We have studied 40 individuals for 6 nights and 6 days. Over the course of this period, subjects received 4 polysomnograms and 10 functional magnetic resonance imaging (FMRI) sessions. During the FMRI sessions, functional brain imaging data was collected while subjects performed each of 3 cognitive tasks. These data provide a rich amount of information concerning the effects of prolonged total sleep deprivation and recovery sleep on cognitive performance and the cerebral underpinnings of that performance. Early analyses of these data are revealing the course of deterioration and recovery in cognitive performance, the specific component processes of cognition affected by sleep deprivation, and the changes in brain function associated with sleep deprivation. We have also initially reported distinct patterns of recovery for different sleep parameters after sleep deprivation, and the possibility of using the FMRI measures to identify neural correlates of vulnerability and resilience to sleep deprivation.
