Book excerpt: Excerpt from The Dublin Journal of Medical Science, Vol. 107: January to June, 1899 I have elected to present the following case to the Medical rather than to the Pathological Section of the Royal Academy of Medicine, as the interest lies in its clinical quite as much as in its pathological aspect, and for the same reason I have designated it under the title of haemorrhagic ascites, as embodying its most prominent and, indeed for some weeks, its sole characteristic. It is the only case I have met with, or heard of, in which effusion of sero-sanguinolent fluid in the peritoneal cavity was so copious as to necessitate tapping, and which this necessity became so often imperative, and at such short intervals, if life was to be prolonged. Mr. G., aged forty-two, married, of a healthy, clear complexion, a full build, weighing over 14 stones, came under my care on Sept. 30, 1897, on the recommendation of Thomas Parr, MD, of Drogheda, complaining of gastric symptoms, a sense of fulness and hardness in the epigastrium, with a feeling of soreness. It influences severely the life-history of the poor, among whom almost every individual must be a breadwinner: it mars many a career, which otherwise might be so successful, among those with the best social surroundings. It is, then, of great importance among the things that form the activities of a surgeon's life; and in dealing with it he may worthily exercise his best qualities, for not only does it demand high powers of brain and hand, but it will test his skill and patience to the utmost. And to my younger confreres may I say that they will meet tubercular arthritis far more frequently than abdominal mysteries, with all their fascinating perplexities? So it seems to me that I am justified in trying to lay before you shortly the claims of competing methods, and to arrive at a fair appreciation of them. I start with the admission that by far the greater number of cases, if seen early, are amenable to a certain degree to treatment by mechanical methods. Some appliance, such as Thomas's splint, which gives rest to the joint, and enables the patient to take exercise in the open air, supplemented by generous constitutional treatment, will frequently stay the progress of the disease, and leave the patient with a joint which may be damaged, but useful. It is for this reason that we seldom find it necessary to interfere actively with children of the better class. The watchful care which they enjoy leads to an early recognition of symptoms, and medical aid is sought. But unhappily that is not so with the children of the poor, whom we see in such numbers in the out-patient departments and hospitals of large towns. Their condition is not recognised by parents who are too often unobservant or neglectful, or their complaints are attributed to some passing strain or to "growing pains." When we see these cases many of them have far advanced on the road which leads not only to permanent damage, but to the imperilling of the joint or limb altogether. What is to be done for them? In deciding this important point the pylorus played the chief part. Indeed, a personality was actually supposed to be concerned with the performance of this function, and the name of pylorus signifies the guard or keeper of the entrance - the janitor or porter. For instance, Galen looked upon the pylorus as a regulator or governor of the entrance to the intestine, having sovereign authority and monarchical power over all-the other structures of the stomach. This governor was endowed with the faculty Of judgment and decision, by virtue of which he opened the portal only when he himself considered best. He did so at the end of digestion, not because he was urged thereto by the chyme, but because he had a knowledge of the events happening in the stomach. On the other hand, the pylorus closed the orifice and set up vomiting when he considered it would be injurious to the health to allow the stomach contents to pass further into the system.
