It is biological but also behavioral and social. Mental illness is a problem of both the brain and the mind, and this ambiguity presents a challenge for those who seek to accurately classify psychiatric disorders. The leading resource we have for doing so is the American Psychiatric Association’s Diagnostic and Statistical Manual, but no edition of the manual has provided a decisive solution, and all have created controversy. In The Diagnostic System, the sociologist Jason Schnittker looks at the multiple actors involved in crafting the DSM and the many interests that the manual hopes to serve. Is the DSM the best tool for defining mental illness? Can we insure against a misleading approach? Schnittker shows that the classification of psychiatric disorders is best understood within the context of a system that involves diverse parties with differing interests. The public wants a better understanding of personal suffering. Mental-health professionals seek reliable and treatable diagnostic categories. Scientists want definitions that correspond as closely as possible to nature. And all parties seek definitive insight into what they regard as the right target. Yet even the best classification system cannot satisfy all of these interests simultaneously. Progress toward an ideal is difficult, and revisions to diagnostic criteria often serve the interests of one group at the expense of another. Schnittker urges us to become comfortable with the socially constructed nature of categorization and accept that a perfect taxonomy of mental-health disorders will remain elusive. Decision making based on evolving though fluid understandings is not a weakness but an adaptive strength of the mental-health profession, even if it is not a solid foundation for scientific discovery or a reassuring framework for patients.

There are several factors that have contributed to this empirical influx, including improved diagnostic cri teria for many psychiatric disorders, increased nosological attention to childhood psychopathology, and development and standardization of several structured diagnostic interview schedules for both adult and child populations. With the advent of DSM-III-R, and in anticipation of DSM-IV, diagnostic labels and their definitions have been in a state of change, as evinced by the many refinements and modifications currently taking place. However, the basic purpose or raison d'etre of the nosological scheme has not been altered. Psychiatric diagnosis is the means by which we classify or categorize human psychopathology. And, as is the case in the medical arena, psychiatric diagnosis serves three central functions: classification. communication. and prediction. As research accumulates, our understanding of psychiatric disorders increases, and we are in a much better position to classify reliably and with validity, as well as to com municate and predict Despite periodic changes in the diagnostic system, the basic strategies for conducting diagnostic research (e. g. , genetic-family studies, biological markers, follow-up studies, etc. ) do not vary appreciably over time. But in over one decade no scholarly book has appeared that tackles the essential research issues involved in upgrading the diagnostic endeavor.

The book begins with a review of the nature of classification procedures by highlighting some of its main problems and controversies. In chapter 2, the evolution of our current diagnostic system--the DSM--is discussed and the central argument is advanced. The authors suggest that for the DSM to continue to advance, we must begin to scientifically determine the underlying nature of these diagnostic entities through the use of procedures such as taxometrics. Chapter 3 offers a detailed analytic primer on the nature of taxometrics. The primer is written in a user-friendly manner so clinicians and others not familiar with the underlying mathematics associated with taxometrics can gain a full understanding of the importance and utility of these procedures. Chapter 4 is specifically focused on outlining a method by which taxometric procedures can be applied to diagnostic entities within the DSM. The final two chapters provide a review of the current taxometrics literature and the degree to which it has been applied to specific psychopathological entities (e.g., schizophrenia spectrum, anxiety, eating disorders). In summary, this book represents a "call to action" to revolutionize the diagnostic system. The point of this book is not that a diagnostic revolution has occurred; it is that it can and should occur and that, to a degree, it is occurring.
