Participants included representatives of academic obstetrics, academic pediatrics, and public health. After an introductory discussion by Howard C. Taylor, Jr. on changes in obstetrics in the last 25 years, the 16 conference papers are presented in the following sections: (1) "Departments of Obstetrics," as viewed by a non-expert and by a pediatrician; (2) "Support of Academic Obstetrics," coming from both the public and the private sector; (3) "Education in Academic Obstetrics," which included discussions on a school of family and community medicine and on the value of "a major clinical service" in the academic field; (4) "Current Residency Requirements" at the American College of Obstetricians and Gynecologists; and (5) "The Impact of Reproductive Biology on Academic Obstetrics," as viewed by an endocrinologist and an obstetrician-biochemist. Included in this section are discussions of the impact of perinatology on academic obstetrics, the relationship of departments of obstetrics to other medical school administrative units, and the relationship of obstetrics and pediatrics in a changing world. Finally, the needs of academic obstetrics are assessed along with suggestions on how to meet them. (PJM)

Strengthening Research in Academic OB/GYN Departments determines whether such departments are capable of fulfilling this promise. The committee finds that these departments have a poor track record in their ability to compete for research funds and in the extent to which their faculty contribute to research. Adding to the problem are factors that deter OB/GYNs from a research career and a dearth of dynamic research leaders in the profession. The volume explores reasons for this situation and makes recommendations to counteract them. Also included is an agenda of needed research.

Book excerpt: Since 2005 a dozen states and more than 15 specialties have reported a physician shortage or anticipate one in the next few years. This anticipated shortage and a worsening of physician distribution are compounded by a projected increased demand for women’s healthcare services. Women’s healthcare is particularly vulnerable, because the obstetrician-gynecologist workforce is aging and is among the least satisfied medical specialists. Furthermore, fellowship training in women’s healthcare in internal medicine and in maternal child health in family and community medicine involves only a small portion of general internists and family physicians. In response to this challenge, the Association of American Medical Colleges called for an expansion of medical schools and graduate medical education enrollments. As we cope with significant and rapid changes in organizations and reimbursement, academic departments of obstetrics and gynecology, family and community medicine, and internal medicine have opportunities to create a unified women’s health curriculum for undergraduate students, share preventive health and well-woman expertise in training programs, provide improved continuity of care, instill concepts of lifelong learning to our graduates, and better develop our research programs. This volume’s chapters focus on strategic planning on behalf of academic faculty who will train the anticipated additional load of students, residents, and fellows in women’s healthcare. -changing demographics of faculty -expanding roles of clinician educators -physician investigators and their future -the hidden value of part-time faculty -faculty salaries -required skillsets of academic leaders -the meaning of tenure and faculty satisfaction and retention. Recommendations presented here from authors with distinguished leadership skills indicate a consensus, but not unanimity. In furthering these goals, we summarize in the final chapter our collective expertise and offer ways to implement recommendations to better prepare for tomorrow’s needs in academic women’s healthcare.

The discussions focused on how to increase human capacity in the field of obstetrics and gynecology. The meeting was hosted by the University of Michigan Department of Obstetrics and Gynecology Global Initiatives program and was supported through a grant from the Flora Family Foundation. Within the pages of this document, the current status of women's health and OB/GYN training programs in 10 sub-Saharan African countries are described, with a Call to Action and Way Forward to training new OBGYNs in country. These are the words of obstetricians in the field, some who work as lone faculty in fledgling OB/GYN departments. These committed people are charged with the task of not only teaching the next generation, but may be the only OBGYN per 500,000 population or more. Their tireless pursuits are recognized, and their yearning for collegial support is palpable. Every country should have a cadre of highly trained OB/GYNs to teach the next generation, contribute to policy development and advocate for progressive legislation, conduct the research needed to solve local clinical problems, and contribute to the field of women's health in general. But most of all, it must be recognized that women across the globe have the right to access a full scope and high quality obstetrical and gynecological care when and where they need it. These pages bring to light successes achieved and shared, and lessons learned that have already spurred new programs and given hope to those eager for a new way forward.
