Book excerpt: Radiology, the youngest of the major medical sciences, has undergone an extraordinary technical evolution since the discovery of X-rays. It began with the development of the different types of tomography and the adoption of many contrast agents, then proceeded rapidly to serioscopy, subtraction of images, direct enlargement, echography, thermography, and xerography. Today, even before all these innovations have come into common use, another branch of radiologic technology has evolved: computerized (axial) tomography. More than just an innovation, its true dimensions are unfore seeable. Radiology has become in less than a century an indispensable adjunct to the practice of medicine. The development of radiology as a speciality followed its technical advances, which varied greatly from country to country. This rapid development led quickly to subspecialization, even the very early development of radiotherapy and radiodiagnostics as separate entities. However, the entry of radiology into the university has preserved it a single branch of medicine, avoiding the frequent tendency toward auto nomy of the branches of a speciality. Today the fourth generation of radiologists is faced with another deci sion: whether to become technologists subjugated to their machinery, to become sub specialists with a single skill, or to remain doctors. The vast majority of this fourth generation has rejected becoming an accessory to a master technique and rather has specialized according to the hippocratic concept of medicine.

Its purpose is for the exchange and development of ideas and philosophy regarding the diagnosis and treatment of cervical spine injury and disease. The com position of the membership should reflect the varying specialities and disciplines dealing with the cervical spine; biochemical, engineering, neurology, neurosurgery, radiology, orthopedic surgery and others", wrote J. William Fielding, New York City, one of the Society'S founding members. In 1984 the membership list contained 118 names. The overwhelming majority of members came from the United States, four from Canada and 9 Europeans, amongst them Mario Boni. He was the organizer, in con junction with the members of the Cervical Spine Research Society, of a splendid "International Meeting for the updating on pathology and surgery Preface VI of the cervical spine" held in Pavia (Italy) in May 1982. It was from this meeting that the idea of a European branch of the Society first originated, with the purpose of establishing multi-disciplinary meetings in Europe. The first meeting of the European branch was organized by A. Weidner in September 1984 in Osnabriick (Federal Republic of Germany). Mario Boni was elected president. He was not only the initiator but also the drivinglorce who tirelessly promoted the development of the European branch. Mario Boni's death in 1986 at the age of 64 was a great loss to us.

Nevertheless, the pathology of the skull may involve crossing these boundaries. A good example is the cooperation needed in the treat ment of skull base trauma. The Skull Base Study Group was founded some years ago in view of this situa tion. Its purpose is to organize international conferences on topics related to skull base pathology. The first was held in June 1982 and focused on the area of trauma tology. The present volume contains articles written by the highly qualified con tributors to the June 1982 conference, dealing with the anatomical, biomechan ical, and neuroradiological aspects of skull base fractures and their consequences in neurologic and vascular dysfunction and deficits. Many authors from Europe and abroad have contributed to the high standard of this book. I would like to congratulate the Editors on the publication of this volume. It contains many important and interesting aspects of particular value to all medical and surgical specialists working in or on one of either sides of the skull base.

It is a harmless and almost painless technique that does not require the introduction of contrast medium into the subarachnoid spaces. Its interpretation is based on the modifications of the epidural veins which do not themselves have any proven pathology. The longitudinal epidural veins are situated in the anterolateral angles of the spinal canal between the nerve root and the intervertebral disc. They conse quently represent a major anatomic landmark in the investigation of disc pathol ogy because they are compressed by a disc herniation before the corresponding nerve root is reached. The improvement of the techniques of lumbar phlebography by catheterization of the lateral sacral veins and of cervical phlebography by catheterization of the vertebral veins now permits opacification of the epidural venous system along the total length of the lumbar and cervical spine. Spinal phlebography can consequently be used routinely, primarily in disc pathology but now also in other fields of vertebromedullary pathology such as myelopathy, tumors, or stenosis of the spinal canal, where it provides new information as compared to the other techniques of investigation. The techniq ue and results of spinal phlebography in these various conditions are presented in this monograph which is an attempt, with the collaboration of several authors, to summarize current knowledge in this field. Only lumbar and cervical phlebography will be discussed in this monograph.
