Aboriginal communities and lower-income people in Canada are at particularly high risk for developing a range of chronic health conditions including diabetes. Depending on the age group, First Nations adults are two to eight times more likely to have diabetes than Canadians generally, and they also have substantially higher rates of serious complications from diabetes, including kidney failure, foot amputations, heart disease and infectious disease. In this report, the Health Council profiles an initiative in northern Manitoba - the Island Lake Regional Renal Health Program - developed in response to the high rates of kidney failure in a remote First Nations region. And we take a look at the Kahnawake Schools Diabetes Prevention Program which, after 12 years of sustained effort, has put the brakes to the persistent increase in new cases of diabetes in that Mohawk First Nation community near Montreal. The connection between health and wealth is also important in understanding how to influence health outcomes. The proportion of people who are overweight/obese is increasing, and the amount of physical activity is decreasing in most populations, including urban populations in many developing countries. The increasing prevalence of overweight/obesity is presumably due to the increasing availability of highly palatable, high-energy foods, and an increasing sedentary lifestyle due to mechanisation of both workplace and leisure activities. Overweight/obesity and reduced physical activity increases the risk of cancers in various organs. Maintaining a healthy body weight and regular physical activity is the second most important way to prevent cancer, after tobacco control. The suggestions of possible public health actions to tackle these risk factors include the promotion of balanced diets, which are not excessive in energy, and broad education and planning to enable and encourage physical activity during work and leisure. Recommendations and a full discussion of these topics are included in the sixth volume in this series of Handbooks.

Available in PDF, EPUB and Kindle. Book excerpt: Chronic pain is a common problem, affecting 1 in 5 Canadians. Despite this, the burden of chronic pain has not been fully investigated in Canada. This research sought to characterize the burden of chronic pain in Ontario, Canada in three studies. Individuals with and without chronic pain who were identified through the Canadian Community Health Survey (CCHS) were matched using propensity score methods on demographic and comorbidity variables using linked healthcare administrative data. The first study estimated the annual incremental cost to manage chronic pain at $1,742 per person, 51% more than the control group. This translated into $2.8 billion (reference year 2014), or 5% of the Ontario publicly funded health budget. The second study estimated health utilities from the CCHS as a measure of health-related quality of life in people with chronic pain. Utilities are anchored by 0 (dead) and 1 (perfect health), with a difference of 0.03 being clinically meaningful. The mean utility for people with chronic pain was estimated at 0.59 compared to 0.90 for those without. These utilities were lower than those seen with most other chronic diseases including heart disease, diabetes and chronic obstructive pulmonary disease. The third study described mortality by following people until December 2013. There were numerically more deaths in the chronic pain group (2,063 versus 1,722) and a higher death rate for chronic pain as measured by the Kaplan Meier product limit estimator (p.

Book excerpt: Nearly 90% of older Canadians have at least one chronic disease; 65% have two or more. The aims of my thesis were to apply business analytics techniques to predict the presence of an exemplar chronic disease, heart disease, among older Canadians, and to calculate the corresponding expected healthcare costs. I used neural networks to develop logistic regression models of heart disease using demographic, lifestyle, and health information for 15,599 older adults from the Canadian Longitudinal Study on Aging. The Economic Burden of Illness in Canada provided healthcare cost data. The best model identified 65.8% of heart disease cases from 40% of participants with the highest predicted probabilities of heart disease, accounting for $2.7 million more expected annual healthcare costs than a randomly sampled 40%. Among all older Canadians, this difference would be $1.1 billion. These methods could assist healthcare decision makers to optimize the delivery of chronic disease prevention interventions.
