Book excerpt: This study compared the effectiveness of subgingival bacterial plaque and calculus removal following periodontal scaling and root planing using an indirect (closed) versus direct (open) periodontal flap approach. Patients requiring extractions due to advanced periodontal disease were the source of the 60 single rooted teeth comprising the study group. The periodontal status of test and control teeth was evaluated initially by the Gingival Index (G.I.) of Loe and Silness, the Plaque Index (PII) of Silness and Loe and the Periodontal Index (PDI) of Ramfjord. The results favor the open flap procedure by a mean of 20.51% greater effectiveness. However, if one considers the root surfaces by various depths and surface areas, there is no statistically significant difference between the open and closed treatment when the periodontal pocket is greater than 3.0 mm deep. Although there was an 18% mean increase in effectiveness of plaque and calculus removal at depths greater than 3.0 mm by the open approach it did not prove to be statistically significant. This was due to the large variance within group values.

Book excerpt: A basic goal of periodontal therapy is the re-establishment of a healthy peridontium through the removal of supragingival and subgingival bacterial plaque, primary etiologic factors associated with peridontal diseases. Scaling and root planing are the two most common techniques used to remove bacterial plaque and calculus from diseased root surfaces. The purpose of this clinical investigation was to determine if visual access to the root surfaces of molar teeth would significantly enhance the ability to remove subgingival deposits of plaque and calculus on external root surfaces and in furcal areas. The results of this study showed an overall increased thoroughness of subgingival plaque and calculus removal for external root surfaces with an open flap approach versus a closed approach.

Patients with gingivitis have gingival inflammation, redness, and bleeding with probing, brushing or flossing. Periodontitis includes gingivitis and loss of supportive tissues including alveolar bone. As a result, there is a loss of attachment of teeth and a probe placed between the gingiva and tooth structures will show increased probing depth. Bone loss around the teeth is detectable on radiographs. Progression of disease may lead to increased tooth mobility and tooth loss. Periodontal disease is closely associated with oral hygiene and is often preventable. Treatment includes good dental hygiene, and scaling and root planing by a dental professional to remove subgingival plaque. Adjunctive therapies include topical or systemic antibiotics. Low dose doxycycline (Periostat 20 mg twice daily) is thought to inhibit collagenase activity and prevent breakdown of connective tissue and alveolar bone in patients with periodontitis. This report will review the evidence on the clinical effectiveness, cost-effectiveness and guidelines for use of doxycycline (Periostat) to inform funding decisions by drug programs.
