Book excerpt: Excerpt from The Cleveland Medical Gazette, Vol. 13: A Monthly Journal of Medicine and Surgery The well-known action of atropin in arresting secre tion generally has been applied to the treatment of ex Ophthalmic goiter, assuming this disease to be due to an hyperactivity of the thyroid gland; and the clinical re sults Obtained indicate more than ordinary worth. Its undoubted value in opium poisoning is due rather to its general than to its specific action, as its antidotal power lies in substituting for the coma or passive cerebral con gestion of the narcotic an active or arterial congestion, as well as stimulating the various centers in the medulla which are depressed by the lethal dose of the opiate. Homatropin is an alkaloid derived from atropin and resembles it in action, but its effect, while somewhat simi lar in'ikind, differs in degree, and it is rarely used inter nally. A physiological difference exists in the fact that although atropin increases the frequency of the pulse beats, homatropin lessens it. It is useful in determining refractive errors, and while its effect on the pupil and ciliary muscle is as decided as that Of atropin, it is less lasting. Its prompt mydriatic action, as well as slighter degree of toxicity, renders it preferable to atropin for that purpose, but in general Ophthalmic therapy and for inflammatory conditions, atropin is probably preferred by oculists generally. In former years, when the university was still wholly German, this used to be the Mecca of a large number of M. D.'s from all parts of the world, but the division into a Bohemian and a German department has greatly taken from the advantages of the place. Having dwelt more fully on this topic in another letter to the gazette (june, 1886) I will refer the reader to this and simply add that outside of obstetrics, Prague also offers good facilities in surgery and in dermatology. In fact, in this latter department, Professor Pick's clinics and private course are hardly to be excelled. Life in Prague is not so agreeable as in other European cities, for the reason that it is virtually a Bohemian city, and there is a constant Silent war between the German and Bohemian elements, which crops out in an unpleasant manner every now and then. The expense of living is cheaper in Prague than in any other European city of which I know, but the restaurant and boarding accommoda tions are also poorer. The majority of the pavements are miserable, making walking any distance very tiresome. To balance all this, Prague has a beautiful location and abounds in any number of places of great historical interest. Book excerpt: Excerpt from The Cleveland Medical Gazette, Vol. 1: September, 1886 When jurors shall all become intelligent and moral; when lawyers shall have no other ends in view than the attainment of exact justice; when courts shall become incorruptible; in the good time coming, when every man shall be a law unto himself, and repressive statute laws shall be well-nigh needless, we shall see a tolerable correspondence between what we call wholesome laws and human conduct, and not till then. It is a comparatively easy matter to get laws enacted, but generally a very difficult matter to get them thoroughly enforced, even if the enforcement of them is a thing to be desired. It will be found in the end that it is worse than useless for one system, as that of medicine, to attempt to remedy its defects and its faults by the aid of another system, as that of law, which is equally defective and more faulty. We cannot make a really good article out of defective material with the help of poor tools. There is a wide-spread superstition pervading the community, from which physicians are not exempt, that defective human character can so organize itself, socially, as to get out of itself conduct that is not proportionately defective; that by some well contrived legal device society can be cured of all its follies, and saved from the consequences of them. Societies are made up of individual units; the character of the society is as the average of the characters of these units, and the extent to which laws can elevate such characters is almost 7221. If society is to be forever guarded against its own ignorance and folly by law, when will it learn to guard itself against these evils and, moreover, who shall play the part of its perpetual guardian? Shall the physician? There is, there can be, no genuine reform, but voluntary, personal reform there is no reform of organizations like the medical profession, but such as comes through the individual reform of its members.
