Book excerpt: Excerpt from The Canadian Medical Review, Vol. 7: March, 1898 IN this short paper I shall limit myself to a discussion of the cardiac weakness and dilatation which result from senile changes or rather from those conditions and in uences which affect patients in the latter half of life. Balfour has referred to the fact that the heart is one of the organs which under normal conditions is least affected by old age. The liver and kidneys may undergo senile atrophy When the heart may present little change other than a slight increase in the thickness of the wall of the left ventricle. It must be remembered that the condition of the normal old man, if I may be permitted to use that term, is one of health and not disease. An old man of ninety-two whom I frequently see often, exclaims: I am astonished that I feel so well, never better in my life. There are, however, many circumstances in the life of the busy man which, acting upon the heart, produce greater or less dilatation. Among the most frequent of these are depressing mental emotions, which may produce their effects gradually or suddenly. Where the colon is distended the lumbar opera tion is an easy one, but where the bowel is accid and lies deeply in a fat patient it is one of the most difficult operations in surgery. In doing inguinal colotomy the soft parts are much thinner, and after opening the peritoneum the colon is much more accessible. In a stout patient the soft parts are easier to divide, and the resulting wound less deep and more readily dealt with than one in the loin; the bowel is more easily reached and with less disturbance of deep lying soft parts. There is no risk of opening small intestine or of failing through abnormality of the colon. Secondly. The peritoneum being Opened, the site and extent of disease can be ascertained. Thirdly. The shallower wound makes it much easier to draw out the intestine and make a satisfactory angle and spur, or to perform colectomy. I have had the tube of the canula constructed of platinum. It may thus be used with the positive pole and I have added a second stop-cock, which renders it independent of the reservoir. As the use of chemical solutions corrodes metal parts, I employ for injection a second bottle, with tubes of glass leading to and from it. I have also furnished it with a third tube to facilitate the introduction of the solution. Provision is also made for emptying the sac after treatment without polluting the contents of reservoir.
