Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. Book excerpt: It is a notable irony that as democracy replaces other forms of governing throughout the world, citizens of the most established and prosperous democracies (the United States and Canada, Western European nations, and Japan) increasingly report dissatisfaction and frustration with their governments. Here, some of the most influential political scientists at work today examine why this is so in a volume unique in both its publication of original data and its conclusion that low public confidence in democratic leaders and institutions is a function of actual performance, changing expectations, and the role of information. The culmination of research projects directed by Robert Putnam through the Trilateral Commission and the American Academy of Arts and Sciences, these papers present new data that allow more direct comparisons across national borders and more detailed pictures of trends within countries than previously possible. They show that citizen disaffection in the Trilateral democracies is not the result of frayed social fabric, economic insecurity, the end of the Cold War, or public cynicism. Rather, the contributors conclude, the trouble lies with governments and politics themselves. The sources of the problem include governments' diminished capacity to act in an interdependent world and a decline in institutional performance, in combination with new public expectations and uses of information that have altered the criteria by which people judge their governments. Although the authors diverge in approach, ideological affinity, and interpretation, they adhere to a unified framework and confine themselves to the last quarter of the twentieth century. This focus--together with the wealth of original research results and the uniform strength of the individual chapters--sets the volume above other efforts to address the important and increasingly international question of public dissatisfaction with democratic governance. This book will have obvious appeal for a broad audience of political scientists, politicians, policy wonks, and that still sizable group of politically minded citizens on both sides of the Atlantic and Pacific.

It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendations of Improving Diagnosis in Health Care contribute to the growing momentum for change in this crucial area of health care quality and safety.

Nurses have a critical role to play in achieving the goal of health equity, but they need robust education, supportive work environments, and autonomy. Accordingly, at the request of the Robert Wood Johnson Foundation, on behalf of the National Academy of Medicine, an ad hoc committee under the auspices of the National Academies of Sciences, Engineering, and Medicine conducted a study aimed at envisioning and charting a path forward for the nursing profession to help reduce inequities in people's ability to achieve their full health potential. The ultimate goal is the achievement of health equity in the United States built on strengthened nursing capacity and expertise. By leveraging these attributes, nursing will help to create and contribute comprehensively to equitable public health and health care systems that are designed to work for everyone. Book excerpt: From Victorian anxieties about syphilis to the current hysteria over herpes and AIDS, the history of venereal disease in America requires us to examine social attitudes as well as purely medical concerns. This brilliant study is the first book to chronicle the range and direction of American reactions to the VD problem over the last hundred years. As the author makes clear, the medical promise of "magic bullets"--Drugs that would rid us of disease- is, in the case of VD, a promise unfulfilled. Despite dramatic advances, these diseases continue to exist in epidemic proportions. Focusing on this paradox of effective medicine and persistent disease, the author recounts the assorted medical, military, and public health responses to the problems that have arisen over the years; these have ranged from the widespread incarceration of prostitutes during World War I to the legal requirements for premarital blood tests. In the author's view, American concerns about venereal disease have been inextricably tied to a set of social and cultural values relating to sexuality, gender, ethnicity, and class. He shows how plans to combat sexually transmitted infections have typically emphasized the regulation of individual conduct. At the heart of such efforts, Brandt argues, is an ongoing tendency to see venereal disease as both a punishment for sexual misbehavior and an index of social decay. The tension between medical and moral approaches to VD has significantly impeded efforts to control these infections, for it has been too often assumed that merely controlling behavior is the answer.
