M. Hodges, dur ing a period of ten years, is reported for the purpose of showing the ultimate and excellent results of excision of the elbow when recovery takes place, and the advantages of a single straight incision in its performance. This method, largely avoiding the cross-cutting of any tissues, allows the connection of the triceps-extensor tendon, with the investing aponeurosis of the arm and fore-arm, to be preserved almost intact. An attachment for the muscle is thus retained which diminishes, to a certain extent, the loss of power following its unavoid able separation from the olecranon. 1 Transverse incision of the integ uments, even though the above mentioned connection is maintained, is in itself prejudicial to the subsequent motions of the limb, if the ound does not unite by first intention, but cicatrizes by granulation, as it almost invariably does. But there is a class of cases, which begin in real or supposed injuries, involving litigation, and where medical opinion is to be decisive; and these cases, though founded on attested facts, may result in conditions against the rule - and proved facts, as exceptions, may be classed as improbabilities. Theory, raised by men of high social position and deserved reputation, against the detailed certainties collected under the observation of a younger practitioner, may prevail, when discussed by a jury whose minds have not been enlightened upon medical subjects. A practitioner of very limited experience may occasionally be called to a case of surgery, unique in its class. It is not the less so, because older professional gentlemen in a long and extensive practice have never happened to see a similar case, or to have read or heard of one. The statistics of surgery are for the most part based upon military, naval or civil hospital reports. These fields of practice are extensive. But surgery is not fairly represented in its results, by the facts furnished from these sources alone. The resources and accommodations of swell governed hospital are undoubtedly superior to those at the command of the majority of persons exposed to accident or suffering from disease and yet, for a reason which others may find, the surgery of private practice, so far as I am acquainted with it, is more successful, if in skilful hands, than that of the hospital. In this consideration, the more formidable operations may be excluded - such cases naturally finding their termination in the hospital, for reasons appreciated by all who are acquainted with the matter. Book excerpt: Excerpt from The Boston Medical and Surgical Journal, Vol. 99: July-December, 1878 The case is reported of a house in Hadley, built by a clergyman, where a well existed in the cellar into which foul air from the sink drain had access. Vegetables were kept in the cellar; the windows were never taken out; there was no escape for the foul, damp air; and water stood in drops upon the ceiling and walls. After a few months' residence in the house the minister's wife died, of fever, so far as I can learn. He soon married again, and within one year of the death of the first wife the second died, from, as I understand, the same disease. The children were also sick. He lived in the house about two years. The next occupant was a man named B His wife was desperately sick. A physician then took the house. He mar ried, and his wife died of the fever. Another physician was the next tenant, and be, within a few months, came near dying of erysipelas. All this while matters had remained as before described with reference to ventilation. A school-teacher then rented the house, and tore up the closed box, but did not cover the well. This was about eight years after building the house. The sickness and fatality were so marked that the property became unsalable. When last sold every sort of pre diction was made with reference to the risks of occupancy, but, by a thorough attention to sanitary conditions, no such risks have been en countered.
