Book excerpt: Excerpt from The American Journal of the Medical Sciences, 1920, Vol. 159 The mortality rate following prostatectomy is in direct ratio to three vitally important factors: (1) The recognition of absolute contra-indications to operation; (2) the recognition of relative contra-indications to operation, and (3) the knowledge of methods of treatment preliminary to operation that will remove such relative operative contra-indications and thus bring the individual within the operable class. Characteristic illustrations of these factors are most frequently found in subnormal function of the kidneys. In one instance an impending uremia may be due solely to an antecedent nephritis of the chronic interstitial type, in which event, the cause being irremovable, there is an absolute contra-indication to operation. In another instance a minor degree of interstitial nephritis may exist, but the dangerously low renal function present is dependent upon back pressure incident to lower urinary obstruction. Under these circumstances there is a relative contra-indication to operation, because after decompression of the kidneys the renal function im proves, so that eventually the obstructing prostate can be removed successfully. From experience in army base hospitals, two facts of importance have been rather universally recognized. The first fact refers to the general preva lence of the streptococcus (normally one of the hemolytic varieties) in pneumonia following measles. The second fact refers to the prevalence of the streptococcus, likewise one of the hemolytic varieties, as an organism associated in many localities with the pneumococcus in the production of epidemic pneumonia not pre ceded by measles or influenza, but as a distinct type in which the pneumococcus has appeared to play but a minor role. In certain localities, during the recent epidemic of influenzal pneumonia, the pneumococcus as the prevalent type of infection both in the acute disease and in the subsequent empyemas. In other localities the pneumococcus and streptococcus have been associated, while in others streptococcus pneumonias have been more frequently encountered than had been heretofore generally recognized. In the series of empyema here reported only those are included which required treatment by aspiration or operation. There were available for this report 35 empyema patients who recovered by repeated aspirations alone, while came to Operation. Those patients were not included who died within a few days of admission with an established diagnosis of empyema or those who died during the course of aspirations designed for their relief, but who, because of activity of the pneumonic process, acute bilateral empyema, purulent pericarditis, meningitis or other serious complications, were not considered reasonable operative risks. If the patient's symptoms improved, but the lesions did not Show improvement, the case was not considered improved. Unimproved means that the patient's lesions or symptoms or both were not improved and that the lesions steadily progressed despite treatment. Some of the cases showed such marked improvement under treat ment at first that although they subsequently died it was deemed important to call attention to this class of cases by tabulating them under the heading of primarily improved, subsequently died. Our experience has led us to expect certain cases to do very well under treatment at first and raise great hopes of recovery in the patient and the therapeutist. Such cases have frequently been reported in the literature during this stage of improvement, and not uncommonly the patients have relapsed and died by the time the reports have been published. Some cases are improved but still unde'r treatment, so that their ultimate results are not known. These are classified in the column improved, still under treat ment. I found that she had been attended, at the time of her delivery, by a German midwife, who stated that, after a brief labour, she had given birth to a male infant, weighing ten and one-half pounds. She also stated that the after-birth soon came away, accompanied by a large tumour, which descended into the vagina. This tumour she supposed to be a mole or false conception, and she stated that it was as large as a cannon-ball. The flooding, at the time, she described as terrific, producing protracted syncope. A day or two previous to my first visit, whilst making an effort to evacuate the bowels, the tumour had descended through the os externum and became suspended between the patient's thighs. All her efforts to remove the tumour proving unavailing, the midwife had sent for the gentlemen above mentioned for that purpose, and they now associated me with them in the treatment of the case. This is largely due to the fact that with our advanced knowledge of bacteriology many points in its etiology and causation have been cleared up, and at the same time our methods of treatment are more scientific and accurate. Acute arthritis of infants is a disease which may properly be described under the acute infectious processes of bone. The term was first used by Jules Teilhard La Terrisse in his thesis presented to the Faculty of Medicine of Paris for the degree of doctor of medicine, in 183. He reports three eases in children of three, six, and nine days respectively, and on autopsy in each case found pus in several of the joints. In one case there were also several abscesses in the lung. He believes the joint collections of pus were metastatic, and says that M. Dance, in an article on phlebitis, has cited similar cases. In 1874, Mr. T. Smith, surgeon to St. Bartholomew's Hospital, describes the affection and report twenty-one cases, under the name "Acute Arthritis of Infants." These were described as roundish bodies about twice the size of pus corpuscles and difficult to distinguish from the latter except by their independent motion. The motion was produced by certain organs, viz., a flagelliform appendage at the anterior end which worked with great rapidity, and a row of short cilia, at one side and near the flagellum, which kept up a sort of rotary motion. At the posterior end was a tail-like appendage of variable shape (Fig. 18). On account of the fact that the parasite resembled monads in having a flagellum, and "trichodines" (ciliata) in having cilia, Donne gave it the name of trichomonas, with the specific name vaginale on account of the habitat. Although there were errors in Donne's description, it was remarkably good considering the methods of observation and the knowledge of micro-parasites then available. Certain details in the description will be mentioned later. Notwithstanding Donne's growing reputation as a microscopist, and the fact that his observations were controlled by such men as Dujardin, Milne-Edwards, Ricord, and others, many microscopists (Vogel, Siebold, Valentin) denied the parasitic nature of the bodies, and held them to be altered epithelial cells from the uterus. Later, however, Scanzoni and Kolliker, Hennig, Haussmann, and Gasser re-examined the organisms, mostly from the side of clinical microscopy.
