Book excerpt: Cette étude porte sur l'expression de la «composante évaluative » des récits produits par des enfants dysphasiques expressifs âgés de 9 ans à 10 ans Yl, et en particulier, elle s'intéresse aux effets de deux procédures d'intervention sur l'expression de cette composante. Les récits des enfants sont construits à partir d'une séquence de cinq images qui « racontent» l'histoire d'un malentendu entre deux personnages. Après avoir produit spontanément un premier récit, les enfants le racontent une deuxième fois, soit après avoir été questionnés sur les raisons des événements - procédure d'étayage « interactif », soit après avoir entendu 1 'histoire racontée par l'expérimentateur - procédure d'étayage «modèle », soit encore après avoir joué à un jeu de «Memory » avec les images de l'histoire - procédure de contrôle de l'effet de la simple répétition. Les résultats montrent qu'après les deux procédures d'étayage, les enfants racontent des récits dont la composante évaluative et la cohérence globale augmentent, et plus précisément, cette progression est d'autant plus marquée après un étayage de type «modèle ». De plus, les effets positifs obtenus après l'intervention sont maintenus, voire progressent, une semaine plus tard. Ces résultats pointent vers la nécessité de disposer de moyens d'évaluation multidimensionnels pour saisir au mieux les compétences réelles des enfants dysphasiques, et incitent à appliquer ces méthodes sur le terrain.

Objectifs: Nous voulons déterminer la part d'un déficit de « Théorie de l'Esprit» dans ces difficultés persistantes. De même, nous souhaitons évaluer les liens existants entre cette habileté socio-cognitive et le niveau de maîtrise morphosyntaxique. Méthode et résultats: En évaluant la compréhension d'une histoire en images, nous mettons en évidence chez eux une difficulté dans la compréhension des états mentaux. Chez les témoins, ces résultats ne sont jamais corrélés aux performances morpho syntaxiques. Conclusion : Ainsi les enfants dysphasiques, même après rééducation de leur langage oral, semblent présenter des difficultés en « Théorie de l'Esprit ». Cette capacité socio-cognitive appartient à la pragmatique du langage, et est indispensable à une communication efficace, un récit adapté et informatif, c'est-à-dire une utilisation fine et élaborée du langage oral. Les facteurs possibles d'un tel déficit, et ses liens avec le développement morph syntaxique sont ensuite discutés.

Les tâches de fausses croyances constituent l'outil d'évaluation le plus fréquent de cette faculté. Notre étude a pour objectif l'élaboration d'épreuves de théorie de l'esprit permettant de compléter les épreuves de fausses croyances. Nous avons ensuite comparé les aptitudes en théorie de l'esprit de 50 enfants tout-venant et de 21 enfants dysphasiques. Puis, nous avons étudié le lien entre les résultats obtenus à notre test et les capacités communicationnelles des enfants. Ces capacités ont été évaluées par un questionnaire qui a été adressé à différents intervenants: les parents, les enseignants de chaque enfant et les orthophonistes des enfants dysphasiques ont également été sollicitées. Les résultats montrent que les enfants dysphasiques obtiennent une moyenne similaire aux enfants tout-venant à notre test de théorie de l'esprit, malgré leur taux d'échec aux épreuves de fausses croyances supérieur aux enfants tout-venant. De plus, notre étude indique l'existence d'une forte corrélation entre la communication des enfants tout-venant et leur note au test, or ce lien ne se vérifie pas chez les enfants dysphasiques.

Book excerpt: Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
