Book excerpt: Excerpt from Ten Years Record of the Treatment of Cancer Without Operation Prior to giving in detail an account of the past ten years' work on this important sub jcet, it will, perhaps, be interesting to my readers to learn what the circumstances were which led up to severing my con nection with the routine practice which has for so long a period dominated the Medical Profession, and which has, so far, obscured the mental vision Of its members and this, notwithstanding the fact that a successful result has never yet, and, I venture to predict, never will be Obtained by the use of the knife in a bond-tide case of cancer. I hope I may be able to prove this to be the case later on not, however, that I do not ardently wish it were otherwise. During twenty years I was connected with the Glasgow Hospital for Women as Senior Member of the Staff, and, therefore, had numberless opportunities for operating upon cancer, as well as upon other diseases requiring surgical interference, not only in my hospital, but in private practice. I had been taught that this was the only method by which malignant disease could be suc cessfully treated, and, at the time, believed this to be true. But failure after failure following each other, without a single break, induced me to alter my opinion. Moreover, those failures compelled me to recognise and face the fact that, as sure as operative methods were adopted, the dis ease invariably recurred with renewed viru lence, suffering was intensified, and the life of the patient shortened, which latter was, perhaps, matter for thankfulness. Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.
