Les déficits osseux nécessitent une reconstitution osseuse à l'aide de greffons osseux autogènes d'origine intra-buccale el plus souvent, mais également pariétale ou iliaque lorsque ce déficit est majeur. L'utilisation des facteurs de croissance permet de stimuler la synthèse et la cicatrisation osseuse. Ainsi ; le PRF( Plasma Riche en Fibrine) combiné à des greffes osseuses (autogène ou non) permet une accélération de la vitesse de cicatrisation de la greffe. La formation osseuse, ainsi que sa maturation, sont augmentée grâce aux propriétés biologiques de la fibrine. Le PRF permet ainsi de réduire considérablement les temps de cicatrisation et donc la durée ces traitements implantaires lorsqu'il y a une chirurgie d'aménagement osseux. Les travaux de Choukroun et coll. Permettent d'envisager de nombreuses applications à venir pour le PRF, tant en chirurgie plastique qu'en chirurgie osseuse, à conditions d'être capable d'en évaluer de manière impartiale et rigoureuse les effets réels."

L'évolution des techniques de restaurations et de la demande esthétique des patients pousse les praticiens à utiliser de nouvelles techniques. L'amélioration de l'esthétique repose sur l'environnement implantaire, en grande partie celle des tissus mous. Cette revue de littérature vient répertorier les techniques chirurgicales ou non chirurgicales qui vont permettre d'agir sur l'aspect et la qualité des tissus mous péri-implantaires. Les articles sélectionnés permettent de déterminer une liste de procédures qui reste non exhaustive, mais qui sont les plus utilisées. Le niveau scientifique des publications reste globalement peu élevée. Il est de ce fait difficile de comparer l'efficacité des techniques, bien qu'il se dégage un consensus clinique pour mettre en avant l'intérêt des techniques de greffes conjonctives enfouies sous lambeau tracté coronnairement.

Aujourd'hui avec l'essor de l'implantologie, on peut considérer la prothèse sur implant comme une solution de choix pour pallier à un édentement. Cependant, dans la majorité des cas, l'avulsion d'une dent va conduire à une résorption osseuse plus ou moins importante qu'il va falloir gérer avant ou pendant la mise en place de l'implant. Dans une première partie bibliographique on va ainsi se donner les moyens de comprendre l'environnement parodontal autours de l'implant, ses différences par rapport à celui d'une dent, dans quel cadre s'inscrit la résorption et la cicatrisation osseuse mais surtout quels sont les moyens à notre disposition (greffes, biomatériaux, membranes...) pour faire face à la perte osseuse et en vue d'obtenir un environnement compatible à la mise en place du ou des implants. Dans une seconde partie, notre recherche électronique nous aura permis d'identifier tous les essais cliniques évaluant les différentes techniques et les différents bio-matériaux d'aménagement osseux en pré-et per- implantaire, publiés au cours des 5 dernières années, et de les comparer en termes d'efficacité clinique. C'est ainsi, qu'en étudiant et mettant en parallèle les résultats de toutes les études que l'on aura pu sélectionner, on essaiera de tirer des conclusions pertinentes quant à l'utilisation de ces greffes et biomatériaux.
