In the past, surgery has been taught in a "see one, do one, teach one, paradigm, which is only effective with high surgical volumes unavailable to today's trainees. Additionally, medicolegal and economic pressures of today dictate that learning skills be conducted, at least in part, outside of the operating room. Effective training requires a surgical skills curriculum encorporating the use of models, mental imagery as well as more traditional didactic sessions. This issue of Obstetrics and Gynecology Clinics of North America will provide the scaffolding to develop and implement a surgical skills curriculum and covers the following topics: Teaching pelvic anatomy, the use of computers and virtual reality, OSATS and surgical labs, evaluating surgical competency, the use of mental imagery in surgical skills teaching, as well as teaching and evaluating ultrasound skills."

The first four chapters consider core facets of surgical education, such as simulation, while subsequent chapters take a key idea, often well known in another field, and examine its relevance to surgical education. Of course, performing invasive procedures is no longer the exclusive preserve of ‘traditional’ surgeons. Boundaries between surgery and the interventional specialties (radiology, cardiology, intensive care) are becoming increasingly blurred, especially as technology continues to expand. Changing work patterns and explosive technological development mark this out as a major growth area. New educational approaches (e.g. the use of simulation) are emerging. And all clinical practice is a team activity, where clinicians from many specialties (medicine, nursing, allied professions) come together with shared goals. Book excerpt: On 9-10 July, 2001 the Metrics for Objective Assessment of Surgical Skills Workshop convened an international assemblage of subject matter experts in objective assessment of surgical technical skills and of representatives of relevant official bodies involved in surgical education, evaluation and certification. There have been recent advances in the educational science of objective assessment and the technology of medical modeling and simulation (with surrogate tissue, abstract objects or virtual reality (VR) systems) for training of technical skills. In addition, focus on identification and prevention of medical errors, the need for objective criteria for assessment of surgical skills and the increasing demand for accountability to the public has revealed that there is no infrastructure for the objective assessment of technical skills. Several investigators are validating many different systems for training and evaluation, using different tests, criteria, validation methods and even nomenclature. This workshop is an attempt to establish a standardization of nomenclature and assessment methodologies so the surgical education, training and evaluation community can communicate with a common language and have a common basis for comparing statistics from different centers. The results of this workshop are to be considered a first order approximation from the community of subject matter experts that can provide a "straw man" for future refinement.

Book excerpt: "Emerging evidence links adverse surgical outcomes to errors in intra-operative cognitive skills, such as judgment and decision-making. However, the complex thought patterns and behaviours that characterize safe and effective surgeons are poorly understood and highly situation-dependent, and therefore challenging to evaluate and teach. Despite prior attempts to define these constructs, they remain largely theoretical and do not provide for a roadmap for optimizing performance and minimizing errors. This thesis aims to better understand the factors that govern intra-operative decision-making to inform the design of educational curricula to improve behaviours, and to develop novel metrics that evaluate these complex skills. To understand the various facets contributing to intra-operative competence, a large breadth and volume of evidence from surgical and non-surgical domains was reviewed and synthesized into conceptual frameworks that define these constructs. We then demonstrate how simulation and other technologies can be used to design educational curricula that reinforce sound judgment. This is first done through the development of a bench-top simulation curriculum to teach the safe use of electrosurgery, and a randomized-controlled trial to evaluate its effectiveness. Subsequently, we examine the use of Internet and software-based technologies (e-learning) as an alternative method to overcome some of the limitations of a bench-top model, by performing a systematic review to evaluate the effectiveness of e-learning for surgical training. Finally, we design a novel e-learning platform (Think Like A Surgeon) to teach and assess advanced cognitive skills during laparoscopic cholecystectomy by developing objective metrics and obtaining validity evidence to support their use.In conclusion, comprehensive frameworks were developed to allow us to understand the processes that guide experts' decisions, and ultimately their actions and behaviours. The taxonomies and classification systems from this analysis provide the groundwork for future work attempting to address performance and safety gaps in surgical care. This thesis further demonstrated how to effectively utilize bench-top and pervasive computer-based simulation as tools to incorporate expert knowledge and cognitive competencies into educational curricula designed to reinforce sound intra-operative judgment and effective decision-making. Finally, we developed a novel method for objectively assessing advanced cognitive skills using an e-learning platform and provided validity evidence to support its use." --
