Book excerpt: Introduction : le docetaxel (DOCE) est le traitement standard du cancer de la prostate métastatique résistant à la castration (CPMRC). De nombreuses études rétrospectives suggèrent une diminution de l'incidence et de la mortalité du cancer de la prostate (CaP) avec la metformine (MET). La MET présente également une activité anti-tumorale sur des modèles précliniques de CaP avec une augmentation de l'apoptose quand elle est associée au docetaxel. L'ajout de la MET pourrait potentialiser l'efficacité du DOCE dans le CPMRC. Méthode : TAXOMET est une étude de phase II prospective, multicentrique, randomisée contre placebo. Des patients (pts) non diabétiques avec un CPMRC ont été randomisés selon un ratio 1:1 pour recevoir du DOCE à 75mg/m2 tous les 21 jours plus de la prednisone (5mg x 2/j) avec soit de la MET à 850mg x 2/j (groupe D+M) soit du placebo (groupe D+P), pour un total de 10 cycles de chimiothérapie. Le critère de jugement principal était le taux de réponse de l'antigène prostatique spécifique (PSA) (une réduction de ≥ 50 % à partir de l'inclusion). Les critères de jugement secondaires sont le taux de réponses objectives (TRO) (selon les critères d'évaluation de la réponse dans les tumeurs solides (RECIST) v.1.1), la survie sans progression (SSP) clinique, biologique et/ou radiologique, la survie globale (SG), la tolérance et la qualité de vie (QdV). Le test du Chi2 pour les données qualitatives et le test du Log-rank pour les données de survie ont été utilisés pour comparer les bras D+M et D+P. Résultats : de janvier 2013 à décembre 2015, 99 pts ont été randomisés (50 dans le bras D+M et 49 dans le bras D+P) dans 10 centres français et 95 patients étaient évaluables. Aucune différence n'a été observée entre les bras D+M et D+P sur le taux de réponse PSA (66 % vs 63 %), le TRO (28 % vs 24 %), la SSPm (7,8 vs 6,0 mois, p=0,70) et la SGm (24,6 et 19,7 mois, p=0,70). La tolérance était similaire dans les deux bras excepté plus de diarrhée avec la MET (70 % vs 50 %, p=0,72). Aucune dégradation de la QdV n'a été constatée. Conclusion : il s'agit de la première étude prospective, randomisée contre placebo qui évalue la combinaison MET avec DOCE dans le CPMRC. L'ajout de la MET n'a montré aucun bénéfice clinique dans cette population.

Available research data in ASD suggests that it is a neural connectivity disorder and that the social communication and related neurobehavioural symptoms result from reduced synchronization between key "social brain" regions. These interconnected neural systems can be understood through the relationship between functionally relevant anatomic areas and neurochemical pathways, the programming of which are genetically modulated during neurodevelopment and mediated through a range of epigenetic and environmental modulators. Elucidating the underlying molecular mechanisms can provide an invaluable window for understanding the neural wiring that regulates higher brain functions and consequent clinical phenotypes. In keeping with the multi modal and diverse origins of ASD, this Research Topic explores the genetic underpinnings and environmental modulation in the aetiology; neural substrates, biomarkers and endophenotypes that underlie clinical characteristics; as well as neurochemical pathways and pathophysiological mechanisms that pave the way for therapeutic interventions. Furthermore, since genetically mediated deficits and consequent functional impairments involve activity-dependent synapse development that depends on postnatal learning and experience, the trajectory towards the final clinical expression could be modulated by early interventions that exploit the neuronal maturation and brain plasticity. However, identifying these diverse pathogenetic processes and tailoring interventions would require subtyping ASD into homogeneous subgroups. In this regard, this topic covers the current state of evidence in the literature through topic reviews as well as ongoing original work that provides tangible hypotheses and directions for future research.
