Peu de sages-femmes le pratiquent, nous avons donc voulu savoir pourquoi, quelles étaient les difficultés rencontrées, comment était organisé le suivi de grossesse et l’accouchement et quel était l’avenir de l’accompagnement global. Matériels et méthodes : Il s’agit d’une étude descriptive, avec envoi d’un questionnaire aux 80 sages-femmes libérales pratiquant l’accompagnement global en France. Résultats : Il y a beaucoup de difficultés rencontrées : méfiance de la pratique par le corps médical, assurances à coût élevé, grande responsabilité et disponibilité nécessaire. Discussion : Les difficultés rencontrées expliquent le faible nombre de sages-femmes pratiquant l’accompagnement global ainsi que le faible nombre de sages-femmes ayant accès à un plateau technique. Pourtant, les sages-femmes ont les compétences pour pratiquer l’accompagnement global, et ce qui pourrait les motiver serait une plus grande accessibilité aux plateaux techniques et la création des maisons de naissances dans un cadre réglementé. Conclusion : L’accompagnement global est minoritaire en France, mal considérée par le corps médical du fait qu’elle n’est pas réglementée. Une possibilité existe, celles des maisons de naissances attenantes à une maternité, qui sont encore à l’état de projet et qui, si des cahiers des charges avec protocoles établis sont mis en place, pourront permettre aux sages-femmes d’accompagner leurs patientes dans des conditions optimales de sécurité.

Les maisons de naissance, actuellement en expérimentation, proposent ainsi d'allier accompagnement personnalisé et sécurité, grâce au soutien continu des sages-femmes, de la grossesse aux suites de couches. Objectifs : Mettre en avant les motivations des femmes qui accouchent en maison de naissance et décrire leur expérience de l'accompagnement global à la naissance. Méthode : Il s'agit d'une étude qualitative, de type descriptive et interprétative. Six entretiens semi-directifs ont été réalisés chez quatre femmes enceintes souhaitant accoucher en maison de naissance et chez deux femmes ayant déjà accouché en maison de naissance en France. Résultats et discussion : Les femmes souhaitent un accompagnement personnalisé, respectueux de la place du père, ainsi que la présence d'un professionnel connu lors de l'accouchement, permettant le respect de leurs choix. La maison de naissance permet de respecter le caractère intime de la naissance tout en apportant une sécurité, par la proximité des urgences obstétricales. Plusieurs études montrent l'importance du respect des choix et du soutien, dans le vécu de la naissance. De plus, les maisons de naissance françaises ont un niveau de sécurité équivalent à celles des pays voisins. Conclusion : L'information concernant les lieux de naissances disponibles pour les futurs parents doit devenir une priorité afin qu'ils puissent choisir librement ce qui leur convient le mieux. La diversification des lieux de naissance, par la généralisation des maisons de naissance à l'ensemble du territoire français, est nécessaire pour répondre aux demandes des couples.

Cette étude compare les issues d'accouchement, les modalités du travail et les états néonataux des patientes ayant bénéficié de l'accompagnement global versus celles ayant bénéficié d'un suivi standard pendant la grossesse et l'accouchement. Matériel et Méthode : Il s'agit d'une étude de cohorte rétrospective à la maison de naissance et au groupe hospitalier mutualiste de Grenoble entre le 1er Janvier et le 31 Décembre 2017. Deux groupes de 46 patientes ont été constitués: celui des femmes bénéficiant de l'accompagnement global à la maison de naissance de Grenoble et celui des femmes bénéficiant d'un suivi classique accouchant à la clinique mutualiste. Les deux groupes ont été appariés en fonction de l'âge maternel, la parité, l'âge gestationnel à l'accouchement, l'indice de masse corporel et le faible niveau de risques. Résultats : Il n'y a pas de différence statistiquement significative entre les deux groupes concernant les issues obstétricales et les états néonataux. On note cependant, qu'avec un accompagnement global les femmes sont préparées au travail physiologique, elles ont un bishop plus favorable à l'entrée, demandent moins l'analgésie péridurale, en découle une moindre nécessité de travail sous oxytocine de synthèse. Elles ont plus de déchirures de 1er degré mais moins de déchirures du 2ème degré et d'épisiotomies. Conclusion : Les résultats obtenus n'étaient pas en accord avec la majorité de la littérature, cela pouvant s'expliquer par des critères d'appariements impliquant une nécessité d'effectifs plus grands. Cette étude montre qu'il n'y a aucun risque maternel et foetal avec une prise en charge globale.

Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
