This report lays out the recommendations of the UN Millennium Project Task Force 5 Working Group on Access to Essential Medicine. The Working Group recommends increasing the availability, affordability, and appropriate use of medicines in developing countries. This will require new incentives for research; better procurement, supply and distribution; strengthened primary health systems; pro-poor planning and budgeting; close collaboration with communities; and large increases in funding and the number of health workers. These bold yet practical approaches will ensure that substantially more people living in developing countries will have access to essential medicines by 2015.

Differences over the best cure reflect the political confrontation between its 191 member states. The institution has been criticized to lack legitimacy, to need accountability and to be inefficient with a bloated bureaucracy. Recently, allegations of mismanagement and corruption in the Oil-for-Food Program have led to a crisis of confidence. The public debate followed reform initiatives for enlarging the Security Council, achieving the Millennium Development Goals, and establishing new collective mechanisms to protect human rights, counter terrorism and respond to crimes against humanity. Strengthening oversight, governance and management practices aimed at introducing fundamental institutional changes. The publication describes the reform process leading to the United Nations Summit in September 2005. The achievements remain disappointing with the failure to approve a grand bargain. A number of recommendations are put forward to facilitate the reform process in the United Nations, realising, however, that this will remain cumbersome and a lengthy step-by-step effort.

However, people suffering these diseases can hardly access these drugs owing primarily to their exorbitant prices. This book, which recognises global efforts at promoting access to essential medicines via the Trade Related Aspects of Intellectual Property Rights [TRIPs] of 1995 and the 2003 amendment to its article 31, argues that the access to drugs mechanisms of western countries which possess the capacity to produce drugs are onerous to utilise in manufacturing generic drugs. In view of the foregoing, a scheme, modeled on and targeted at harnessing the benefits of the Regional Trade Agreement [RTA] system, which will enable economically disadvantaged countries to easily purchase much needed essential drugs, is designed and analysed in this book.
