A systematic search and analysis of recommendations and policy guidelines from intergovernmental organizations and international bodies identified practical policy options for action on social determinants within these four themes. Policy options focused on early childhood education and care; child poverty; investment strategies for an inclusive economy; active labour market programmes; working conditions; social cash transfers; affordable housing; and planning and regulatory mechanisms to improve air quality and mitigate climate change. Applying combinations of these policy options alongside effective governance for health equity should enable WHO European Region Member States to reduce health inequities and synergize efforts to achieve the United Nations Sustainable Development Goals.

Proxy means testing, community-based selection of individuals and demographic targeting to children show good results on average, but with considerable variation. - Demographic targeting to the elderly, community bidding, and self-selection based on consumption show limited potential for good targeting. - There is no single preferred method for all types of programs or all country contexts. Successful targeting depends critically on how a method is implemented. Adaptive Social Protection: Building Resilience to Shocks outlines an organizing framework for the design and implementation of ASP, providing insights into the ways in which social protection systems can be made more capable of building household resilience. By way of its four building blocks—programs, information, finance, and institutional arrangements and partnerships—the framework highlights both the elements of existing social protection systems that are the cornerstones for building household resilience, as well as the additional investments that are central to enhancing their ability to generate these outcomes. In this report, the ASP framework and its building blocks have been elaborated primarily in relation to natural disasters and associated climate change. Nevertheless, many of the priorities identified within each building block are also pertinent to the design and implementation of ASP across other types of shocks, providing a foundation for a structured approach to the advancement of this rapidly evolving and complex agenda.

Even so, successive New Zealand governments had mixed results from using traditional public management tools to lift the performance of the public service and address persistent problems that required multi-agency action. In 2012 the government decided to try something different. As part of a reform package called Better Public Services, the government challenged the public service to organize itself around achieving just ten results that had proven resistant to previous interventions. The plan was deceptively simple: set ambitious targets and publicly report on progress every six months; hold small groups of public managers collectively responsible; use lead indicators; and learn from both success and failure. This book explores how and why the New Zealand government made progress and how the program was able to create and sustain the commitment of public servants and unleash the creativity of public entrepreneurs. The authors combine case studies based on the experience of people involved in the change, together with public management research. They explain how ambitious targets and public accountability were used as levers to overcome the bureaucratic barriers that impeded public service delivery, and how data, evidence, and innovation were used to change practice. New Zealand experimented, failed, succeeded, and learned from the experience over five years. This New Zealand experience demonstrates that interagency performance targets are a potentially powerful tool for fostering better public services and thus improving social outcomes.

Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

Book excerpt: July 1998 A social program that relies partly on geographic decentralization for placement provides indicators helpful for identifying the program's impact on welfare. An assessment of the welfare gains from a targeted social program can be seriously biased unless it takes proper account of the endogeneity of program participation. Bias comes from two sources of placement endogeneity: the purposive targeting of the geographic areas to receive the program, and the targeting of individual recipients within selected areas. Decentralization of program placement decisions is common, because of the administrative cost of centralized placement decisions and the fact that local groups and governments are likely to be better informed about who most needs help. But full decentralization is uncommon; the center typically retains control of broad geographic targeting. Ravallion and Wodon argue that partial decentralization of program placement decisions creates control and instrumental variables useful for identifying program benefits. The central allocation to a local level of government is presumably based on observable indicators. The central allocation will also influence the allocation to an individual but is unlikely to determine outcomes at the individual level conditional on individual program participation. So with suitable controls for the welfare-relevant geographic characteristics determining program placement decisions, the center's allocation across areas can be used as an instrumental variable for individual participation. The authors use Bangladesh's Food for Education program to illustrate their approach. A single post-intervention cross-sectional household survey was used to identify the impact of the program on school attendance, using geographic placement at the village level as an instrument for individual program placement. To deal with bias from the endogeneity of village selection, the authors used a detailed community survey coordinated with the household survey to control for likely sources of heterogeneity in geographicinfluences on school attendance, consistent with prior information on how the government targeted the program geographically. They found that the programs had significant and sizable impacts on school attendance. At mean points, the program's incentive increased attendance by 24 percent of the maximum feasible days of schooling. A regression estimator ignoring the purposive program placement was found to result in a substantial underestimation of the program's impact. Indeed, the simplest possible control group method-assuming that nonparticipants provide a valid counterfactual-performed much better than a regression method treating placement as exogenous. This paper-a product of the Development Research Group-is part of a larger effort in the group to evaluate the impact of social programs.
