Push mode core segments were removed from risers 8 and 9B between June 5, 1996, and July 26, 1996. Segments were received and extruded at the 222-S Analytical Laboratory. Analyses were performed in accordance with Tank 241-BY-107 Push Mode Core Sampling and analysis Plan (TSAP) and the Safety Screening Data Quality Objective (DQO). None of the subsamples submitted for Total Alpha Activity (AT) analysis or Differential Scanning Calorimetry (DSC) exceeded the notification limits as stated in the DQO. Statistical evaluation of results by calculating the 95% upper confidence limit is not performed by the 222-S Laboratory and is not considered in this report. Primary safety screening results are included in the data summary table. The raw data from DSC and TGA analyses are included in this report.

Two segments from each core yielded a total of 11 samples which were analyzed. The data quality objectives (DQOs) applicable to this sampling event were the Safety Screening DQO (Dukelow et al. 1995) and the Organic Safety DQO (Turner et al. 1995). The samples were received, extruded and analyzed at PNNL 325 Analytical Chemistry Laboratory (ACL). The analyses were performed in accordance with the Sample Analysis Plan (Gretsinger 1996) and indicated that the tank is safe with respect to the criteria in the Safety Screening and Organic DQO. Detailed analytical results were described in the analytical laboratory 45-day Report (Attachment 1, WHC-SD-WM-DP-171, REV. 0) and final report (Attachment 2, PNL-BX-104 REV. 1) prepared by PNNL, 325 Laboratory. Corrections and/or exceptions to the PNNL final report are provided.

Four specific topics are addressed. They are summarized below. Section 3.0 contains mean concentration estimates of analytes found in T-107. The estimates of {open_quotes}error{close_quotes} associated with the concentration estimates are given as 95% confidence intervals (CI) on the mean. The results given are based on three types of samples: core composite samples, core segment samples, and drainable liquid samples. Section 4.0 contains estimates of the spatial variability (variability between cores and between segments) and the analytical variability (variability between the primary and the duplicate analysis). Statistical tests were performed to test the hypothesis that the between cores and the between segments spatial variability is zero. The results of the tests are as follows. Based on the core composite data, the between cores variance is significantly different from zero for 35 out of 74 analytes; i.e., for 53% of the analytes there is no statistically significant difference between the concentration means for two cores. Based on core segment data, the between segments variance is significantly different from zero for 22 out of 24 analytes and the between cores variance is significantly different from zero for 4 out of 24 analytes; i.e., for 8% of the analytes there is no statistically significant difference between segment means and for 83% of the analytes there is no difference between the means from the three cores. Section 5.0 contains the results of the application of multiple comparison methods to the core composite data, the core segment data, and the drainable liquid data. Section 6.0 contains the results of a statistical test conducted to determine the 222-S Analytical Laboratory's ability to homogenize solid core segments.

Book excerpt: NASA commissioned the Columbia Accident Investigation Board (CAIB) to conduct a thorough review of both the technical and the organizational causes of the loss of the Space Shuttle Columbia and her crew on February 1, 2003. The accident investigation that followed determined that a large piece of insulating foam from Columbia's external tank (ET) had come off during ascent and struck the leading edge of the left wing, causing critical damage. The damage was undetected during the mission. The Columbia accident was not survivable. After the Columbia Accident Investigation Board (CAIB) investigation regarding the cause of the accident was completed, further consideration produced the question of whether there were lessons to be learned about how to improve crew survival in the future. This investigation was performed with the belief that a comprehensive, respectful investigation could provide knowledge that can protect future crews in the worldwide community of human space flight. Additionally, in the course of the investigation, several areas of research were identified that could improve our understanding of both nominal space flight and future spacecraft accidents. This report is the first comprehensive, publicly available accident investigation report addressing crew survival for a human spacecraft mishap, and it provides key information for future crew survival investigations. The results of this investigation are intended to add meaning to the sacrifice of the crew's lives by making space flight safer for all future generations.
