The scope of the challenge is well documented in the report by the United Nations Environment Program (UNEP) and GRID Arendal entitled “Mine Tailings Storage: Safety Is No Accident,” which was released in October 2017. The report recommends that “Regulators, industry and communities should adopt a shared, zero-failure objective to tailings storage facilities...” and identifies several areas where further improvements are required. In this context, the application of cutting-edge risk-assessment methodologies and risk-management practices can contribute to a significant reduction and eventual elimination of dam failures through Risk Informed Decision Making. As such, the book focuses on identifying and describing the risk-assessment approaches and risk-management practices that need to be implemented in order to develop a way forward to achieve socially acceptable levels of tailings dam risk.

The advent of rock mechanics, engineering geology and a better understanding of materials have made it possible to construct higher and larger dams and to tackle more difficult sites. The assumptions and risks used in the theory of dam design include such unpredictable events as earthquakes, floods, and geological faults or soft seams, which may be either underestimated or completely missed during initial exploration. Incidents relating to dams are manageable at an early stage, whereas accidents, which are largely unforeseen, result in unexpected behaviour of dams and in catastrophic failures. Investigations conducted to determine the cause of a failure may not reveal the true sequence of events, while expert analyses are often controversial. From the dams that do not fail, of course, we learn nothing. Systematically monitoring the dam’s behaviour from the potential risk stage to the accident event, would allow a hazard-management programme to be implemented, minimising loss of life and property, and provide useful data.

Also, the growing imp- tance of the topics mine wastes, mine site pollution and mine site rehabilitation in universities, research organizations and industry requires a textbook suitable for undergraduate students. Until recently, undergraduate earth science courses tended to follow rather classical lines, focused on the teaching of palaeontology, cryst- lography, mineralogy, petrology, stratigraphy, sedimentology, structural geology, and ore deposit geology. However, today and in the future, earth science teachers and students also need to be familiar with other subject areas. In particular, earth science curriculums need to address land and water degradation as well as rehabili- tion issues. These topics are becoming more important to society, and an increasing number of earth science students are pursuing career paths in this sector. Mine site rehabilitation and mine waste science are examples of newly emerging disciplines. The consequences of these failures have been so devastating they have pushed conversations about the risks inherent in these structures beyond the mining community into the general population. We are left to question how we address the risks associated with tailings disposal, and in so doing, transform the image of the mining industry and perhaps the industry itself. With this as a backdrop, the Society for Mining, Metallurgy & Exploration (SME) challenged tailings and mining professionals to re-imagine the future of tailings. The Mine Tailings: Perspectives for a Changing World symposium, held at the SME 2020 annual conference, started that conversation. Over three days, tailings professionals from around the world gathered to discuss tailings storage practices and the changes both the industry and the world want and need. The discussions squarely focused on how we, as an industry, can collectively make changes that will eliminate catastrophic tailings dam failures and lead to better outcomes for the industry and society. Through sharing and conversation, the symposium participants recognized risks associated with our approach to tailings management and existing structures and discussed the gaps that need to be addressed, including how the behavior of tailings and mining professionals must change. The human element of risk must be recognized so it can be talked about openly, given the attention it deserves, and adequately addressed. We need to own this problem and the impact of our actions. We have the power to change this; when we own our actions, we can act differently for a different outcome.
