Book excerpt: Systolic time intervals (STI), known for a long time, were revived in the early 1960s by Arnold Weissler. The first important papers came from him; they eval uated STI measurements in larger populations, established norms, and compared STI measurements with other invasive methods. Since then, several hundred ex cellent papers have appeared using and evaluating STI measurements for myo cardial function studies. These have appeared in prominent cardiologic journals throughout the world as well as in the anesthesiologic and pediatric medical literature. When consent of subjects to medical experimentation became a prominent and necessary issue, the noninvasiveness of investigative methods in medicine, wher ever possible, resolved the issue. Noninvasive measurements of STI proved to be an excellent monitoring parameter of cardiac function during anesthesia, and anesthesiologists wished to learn more from the long experience of cardiologists with STI. An organizing committee of List, Gravenstein, and Spodick was accordingly formed. Invitations were issued for an international conference on STI. With more than 40 clinical researchers using STI, it was, of course, difficult to hear them all within two days. A generous sponsor, the AVL Company, Graz-Schaffhausen, made possible the International Conference at Graz. Publication of the proceedings of the Confer ence became feasible with help from Boehringer-Mannheim.

Book excerpt: It has been our experience that instruction in physical examination of the heart in medical schools has been deteriorating since the advent of such modern diagnostic tools as two-dimensional echocardiography and nuclear imaging. At best, the teaching has been sketchy and too superficial for the student to appreciate the pathophysiological correlates. Both invasive and the noninvasive modern technologies have contributed substantially to our knowledge and understanding of cardiac physical signs and their pathophysiological correlates. However, both students and teachers alike appear to be mesmerized by technological advances to the neglect of the age-old art, as well as the substantial body of science, of cardiac physical examination. It is also sad to see reputed journals give low priority to articles related to the clinical examination. Our experience is substantiated by a nationwide survey of internal medicine and cardiology training programs, which concluded that the teaching and practice of cardiac auscultation received low emphasis, and perhaps other bedside diagnostic skills as well (1). The state of the problem is well reflected in the concerns expressed in previous publications (2–4), including the 2001 editorial in the American Journal of Medicine (Vol. 110, pp. 233–235), entitled “Cardiac auscultation and teaching rounds: how can cardiac auscultation be resuscitated?”, as well as in the rebuttal, “Selections from current literature. Horton hears a Who but no murmurs—does it matter?” (5).

Following a description of the physiology of respiration, the pathophysiology underlying central apneas in different disorders and particularly in heart failure is discussed. The similarities and differences of obstructive and central apneas are then considered. The book looks beyond the concept of sleep apnea to daytime CSR and periodic breathing during effort and contrasts the opposing views of CSR as a compensatory phenomenon or as detrimental to the failing heart. The diagnostic tools currently in use for the detection of CSR are thoroughly reviewed, with guidance on interpretation of findings. The book concludes by describing the various forms of treatment that are available for CSR and by explaining how to select patients for treatment.
