Officials need to plan for a rapid response, and to do so, they need information about where farms are located and how they cluster spatially. No public domain, nationwide database currently exists for actual farm locations. RTI researchers in coordination with University of Pennsylvania infectious disease modelers developed a new way to create a synthetic dataset that replicates the spatial distribution of poultry farms and the type and number of birds raised on them. The RTI method produced a more realistic dataset than the currently used random placement alternative. The results suggest the RTI method could be used to simulate other livestock farms of interest to infectious disease researchers.

Book excerpt: In 2005, RTI International researchers developed methods to generate synthesized population data on US households for the US Synthesized Population Database. These data are used in agent-based modeling, which simulates large-scale social networks to test how changes in the behaviors of individuals affect the overall network. Group quarters are residences where individuals live in close proximity and interact frequently. Although the Synthesized Population Database represents the population living in households, data for the nation’s group quarters residents are not easily quantified because of US Census Bureau reporting methods designed to protect individuals’ privacy Including group quarters population data can be an important factor in agent-based modeling because the number of residents and the frequency of their interactions are variables that directly affect modeling results. Particularly with infectious disease modeling, the increased frequency of agent interaction may increase the probability of infectious disease transmission between individuals and the probability of disease outbreaks This report reviews our methods to synthesize data on group quarters residents to match US Census Bureau data. Our goal in developing the Group Quarters Population Database was to enable its use with RTI’s US Synthesized Population Database in the Modeling of Infectious Diseases Agent Study.

Book excerpt: Communicable-disease transmission models are useful for the testing of prevention and intervention strategies. Agent-based models (ABMs) represent a new and important class of the many types of disease transmission models in use. Agent-based disease models benefit from their ability to assign disease transmission probabilities based on characteristics shared by individual agents. These shared characteristics allow ABMs to apply transmission probabilities when agents come together in geographic space. Modeling these types of social interactions requires data, and the results of the model largely depend on the quality of these input data. We initially generated a synthetic population for the United States, in support of the Models of Infectious Disease Agent Study. Subsequently, we created shared characteristics to use in ABMs. The specific goals for this task were to assign the appropriately aged populations to schools, workplaces, and public transit. Each goal presented its own challenges and problems; therefore, we used different techniques to create each type of shared characteristic. These shared characteristics have allowed disease models to more realistically predict the spread of disease, both spatially and temporally.

Book excerpt: By understanding the movement patterns of people, mathematical modelers can develop models that can better analyze and predict the spread of infectious diseases. People can come into close contact in their workplaces. This report describes methods to develop georeferenced commuting patterns that can be used to characterize the work-related movement of US populations and help agent-based modelers predict workplace contacts that result in disease transmission. We used a census data product called "Census Spatial Tabulation: Census Track of Work by Census Tract of Residence (STP64)" as the data source to develop commuting pattern data for agent-based synthesized populations databases and to develop map products to visualize commuting patterns in the United States. The three primary maps we developed show inbound, outbound, and net change levels of inbound versus outbound commuters by census tract for the year 2000. Net change counts of commuters are visualized as elevations. The results can be used to quantify and assign commuting patterns of synthesized populations among different census tracts.

Book excerpt: Maternal nutrition influences a child’s birthweight, which affects the child’s growth and subsequent survival. However, the broad consequences of maternal undernutrition and the outcomes of interventions to improve maternal nutrition take years to manifest. To examine the long-term health outcomes of low birthweight infants in response to a maternal nutritional supplementation intervention without this obstacle, we developed the Forecasting Population Progress (FPOP) microsimulation model. The intervention we assessed was based on the findings of a published clinical trial outcome that reduced the incidence of low birthweight, a known cause of stunting. We implemented the “before intervention” and “after intervention” simulations and generated the difference in outcomes, using a spatially explicit synthetic baseline population of Indonesia generated from a microdata sample of the Indonesian 2010 census. We focused specifically on two provinces—Yogyakarta and Bali—which represent different levels of fertility and mortality, but both exhibit significant underweight birth. The baseline scenario represented the current nutritional status of pregnant women in the two Indonesian provinces and projected that implementing a multiple nutrition supplementation intervention would, after 30 years, avert 8 per 1,000 low birthweight births, 3.8 per 1,000 stunted children younger than 5 years of age, and 0.25 infant deaths per 1,000 births. As our model results demonstrate, improvement in maternal nutrition would reduce infant mortality, but an even greater impact could be the reduction in growth stunting.

In addition, the incidence, prevalence, and death rates from NCDs are expected to increase in the future. No single discipline can address the issues of prevention, early detection, access to care, and appropriate treatment that are needed to improve outcomes for NCDs; this growing need will require transdisciplinary collaborations. Improving outcomes in NCDs is clearly a difficult endeavor that will require significant efforts. However, previous research and program implementation indicate that improving outcomes for NCDs is an achievable goal. Given the tremendous impact of NCDs on morbidity, mortality, and costs worldwide, there is little doubt that efforts to improve NDC outcomes are much needed. Recognizing the importance of new strategies and interventions addressing NCDS globally, RTI funded the projects described here to assess how the institute’s range of expertise could be focused on the reduction of NCDs. The projects described in this book focus on the risk factors or treatment (or both) of one or more of the four NCDs that are responsible for the greatest impact on morbidity and mortality: cardiovascular disease, cancer, diabetes, and chronic respiratory diseases. The pilot projects were carried out in low- and middle-income countries, which are disproportionately affected by NCDs. The lessons learned from these initial pilot projects have great potential to be increased in their scale and reduce the devastating impact of noncommunicable diseases.

Coronavirus disease 2019 (COVID-19) has rapidly disseminated worldwide, with clinical manifestations ranging from mild respiratory symptoms to severe pneumonia and a fatality rate estimated around 2%. Person to person transmission is occurring both in the community and healthcare settings. The World Health Organization (WHO) has recently declared the COVID-19 epidemic a public health emergency of international concern. The ongoing outbreak presents many clinical and public health management challenges due to limited understanding of viral pathogenesis, risk factors for infection, natural history of disease including clinical presentation and outcomes, prognostic factors for severe illness, period of infectivity, modes and extent of virus inter-human transmission, as well as effective preventive measures and public health response and containment interventions. There are no antiviral treatment nor vaccine available but fast track research and development efforts including clinical therapeutic trials are ongoing across the world. Managing this serious epidemic requires the appropriate deployment of limited human resources across all cadres of health care and public health staff, including clinical, laboratory, managerial and epidemiological data analysis and risk assessment experts. It presents challenges around public communication and messaging around risk, with the potential for misinformation and disinformation. Therefore, integrated operational research and intervention, learning from experiences across different fields and settings should contribute towards better understanding and managing COVID-19. This Research Topic aims to highlight interdisciplinary research approaches deployed during the COVID-19 epidemic, addressing knowledge gaps and generating evidence for its improved management and control. It will incorporate critical, theoretically informed and empirically grounded original research contributions using diverse approaches, experimental, observational and intervention studies, conceptual framing, expert opinions and reviews from across the world. The Research Topic proposes a multi-dimensional approach to improving the management of COVID-19 with scientific contributions from all areas of virology, immunology, clinical microbiology, epidemiology, therapeutics, communications as well as infection prevention and public health risk assessment and management studies.
