An aspect of the syndrome of fibromyalgia is fluctuation as in onset of pain, variation in the level of symptoms, time off from pain and recovery from pain and other symptoms. The analysis of these fluctuations might create a basis for solid suggestions regarding the nature of the syn-drome itself. E.g. the pain level is well known to vary with mental and physical load including exposure to cold. Simultaneously, fibromyalgia has been found to mean an altered balance in the autonomic nervous system. In the first section of the book a developmental stage or life before fibromyalgia is covered. Intra- and interpersonal patterns based on narrations of the afflicted are pictured. Identified patterns are psychometrically examined and environmental as well as psychobiological patterns are accounted for. In the mid-section of the book life with fibromyalgia is scrutinized including biomarkers. Patterns regarding variation in the level of pain, gaps in fibromyalgia pain and environmental factors influencing these gaps are related. The effect on life, symptoms and defense measures is elucidated from the angle of mental load. The last sections portrait psychological and environmental influences concerning recovery, but especially the striking phenomenon of recovery being scarce. Special attention is paid to cognitive-emotional functioning, the need to target dissociation and environmental influences on maintenance.

We all experience them, use them as signals to guide our behavior, and usually assume that they accurately represent underlying physiological activity. At the same time, we implicitly know that bodily sensations are often vague, ambiguous, and subject to a variety of interpretations. It is not surprising, then, that there is often a disparity between what we think is going on in our bodies and what is objectively occurring. In short, phenomena such as physical symptoms are the stuff of psychology. My own research into physical symptoms started by accident several years ago. In a hastily devised experiment dealing with the effects of noise on behavior, I had to write a post-experimental questionnaire that would be long enough to allow the experimenter time to calibrate some equipment for a later portion of the study. I included some physical symptoms on the questionnaire as fillers. The experiment was a total failure, with the exception of the symptom reports. People's perceptions of symptoms were easily influenced by our manipulations, even though their actual physiological state had not changed. And so began the present inquiry. Despite the pervasiveness, importance, and sheer amount of time and money devoted to discussing and curing common physical symptoms and sensations, very little empirical work has been devoted to examining the psychological and perceptual factors related to sensory experience. Occa sional papers have tested a specific theory, such as cognitive dissonance, wherein physical symptoms served as an interesting dependent measure.

The clinical case definition was developed by an expert medical consensus panel of treating physicians, teaching faculty and world leaders in the research of ME/CFS. An expert subcommittee of Health Canada established the Terms of Reference for the consensus panel. The definition more adequately reflects the complexity of symptoms of a given patient’s pathogenesis and should establish ME/CFS as a distinct medical entity and help distinguish it from overlapping medical conditions in the absence of a definitive laboratory test. “The clinical definition will enable clinicians to make an early diagnosis which may assist in lessening the impact of ME/CFS in some patients,” said Dr. Bruce M. Carruthers, lead author of the definition. “It will reduce the expensive problem of patients being sent to many specialists before being diagnosed and will allow patients to receive appropriate treatments in a timely fashion.” The panel's clinical case definition determines that more of the prominent symptoms are compulsory and symptoms that share a common region of pathogenesis are grouped together for clarity. In addition to severe prolonged fatigue, the definition includes the hallmark symptoms of post-exertional malaise and/or fatigue, sleep dysfunction, pain, two or more of the given neurological/cognitive manifestations, and at least one of the given symptoms from two of the categories of autonomic, neuroendocrine, and immune manifestations. Diagnostic exclusions and common co-morbid entities are also given. The special issue of the Journal of Chronic Fatigue Syndrome also includes a discussion of prominent symptoms, clinical practice diagnostic and treatment guidelines based on the best available research evidence, and an overview of available research on ME/CFS. The expert panel of 11 physicians—who have diagnosed and/or treated more than 20,000 ME/CFS patients between them—has developed a clinical case definition that provides a flexible conceptual framework based on the characteristic patterns of symptom clusters, which reflect specific areas of pathogenesis.
