To that end nearly 200 transfusionists and their colleagues gathered in Sydney in October 2005 to conduct a marathon symposium on methods and advances made towards achieving universal safety for blood and blood product transfusion. Advances in transfusion safety were put forward from all areas of the blood transfusion process, including establishment of services, donor selection, infections that may affect donors, safe testing, risk and safety assessment, the roles of government and regulation, the use of blood and its products as well as the need for effective communication. Book excerpt: This symposium highlighted the principle that blood transfusion is an entire process and not a single product, requiring haemovigilance and quality assurance from 'vein' to 'vein'. The prevention of contaminated blood and blood products was a major focus. Today's blood services provide remarkably safe therapeutic products that are virtually devoid of transmissible enveloped viruses. However, the risk of bacterial infection, especially in platelet concentrates, is probably underrated and may warrant routine sterility testing or universal pathogen inactivation without incurring the loss of the therapeutic functions of blood products. The unknown risk of prion transmission requires precautionary measures, including leukocyte depletion that may otherwise be beneficial in some patients. The methods for universal pathogen inactivation continue to evolve with some of them due to be licensed for use in the treatment of blood products. Stem cell transplantation portends a new chapter in haemotherapy. However, the question that no one has seriously addressed is the threshold of benefits commensurate with escalating costs that could lead to the unavailability of blood transfusion as an affordable therapy. This publication provides state-of-the-art reviews of hot topics to transfusionists, haematologists, virologists, and scientists in the plasma industry and regulatory authorities, and illustrates how the International Association for Biologicals continues to serve contemporary issues in science and society.

Taswell One year before this symposium, Cees Smit Sibinga and I began to discuss an approach to quality assurance in the blood bank which we felt would be both important and practical and could serve as the basis for the choice of subjects to be presented in the symposium. As an introduction to this book, I would like to outline our approach, the subjects chosen and the rationale behind our choice. What is the fundamental purpose of a blood bank and trans fusion service? Simply stated, the purpose of a blood bank and transfusion service and of a quality assurance program in blood banking is, for the one to provide and, the other to assure safe and effective transfusion therapy. This objective is in contrast to that of other clinical laboratories. The objective in a clinical chemistry laboratory is to produce accurate test results which will be meaningful to the clinician taking care of his patient. In most clinical laboratories, therefore, the goals of a quality assurance program are largely quantitative, that is, to assure accurate numerical test results. In contrast, in the blood bank, the goals of quality assurance are primarily qualitative, that is, to assure safe and effective transfusion. As a result, two somewhat different approaches to quality assurance are necessary.

In order to donate blood, prospective donors should be in good health and free from any infections that can be transmitted through transfusion. Most blood donors perceive themselves to be healthy, but some are unsuitable to donate blood due to the potential risk of compromising or worsening their own health or the risk of transmission of infections to patients. Blood transfusion services (BTS) have a duty of care towards blood donors as well as to the recipients of transfusion. This duty of care extends to prospective donors who are deferred from donation--whether on a temporary or permanent basis--as well as those who donate blood and are subsequently found to have unusual or abnormal test results. BTS have a responsibility to confirm test results and provide information, counseling and support to enable these individuals to understand and respond to unexpected information about their health or risk status. Counseling is part of the spectrum of care that a BTS should be able to provide to blood donors--including referral to medical practitioners or specialist clinical services. Pre-donation counseling was recognized as one element of the strategy to reduce and, if possible, prevent the donation of blood by individuals who might be at risk for HIV and other TTI including hepatitis B and C viruses as well as to inform the donor of the donation process and testing of blood for HIV. Post-donation counseling was acknowledged to be a necessary element of donor management as an adjunct to informing donors of unusual or abnormal test results. Blood donor counseling by trained specialist staff is now considered to be a key component of the blood system in most countries with a well-developed blood transfusion service. It may be required at a number of stages in the blood donation process or following blood screening and should be available at any point at which the BTS has an interface with donors. In many countries, however, blood donor counseling is not yet available in a structured way. Blood Donor Counselling: Implementation Guidelines has therefore been developed to provide guidance to blood transfusion services that have not yet established donor counseling programs.

The currently available analytical methods (both physicochemical and biological) can characterize the primary, secondary and to some extent, the higher order structure of proteins. The sensitivity and selectivity of these methods allow for the identification and characterization not only of the desired protein component, but also many product-related substances and impurities as well as process-related impurities present in the drug preparations. Using an appropriate selection of analytical tools, it may be possible to demonstrate physicochemical and functional comparability between protein products manufactured before and after a manufacturing change or during product development. However, the extent of demonstrable comparability depends on the tests used, on the nature of the product and on the basic understanding of structure-function relationships. Assessment of immunogenicity is also a critical component of product characterization and is highly dependent on the types of assays used.
