Book excerpt: "Over the last seven years the recruitment of overseas trained doctors (OTDs) has formed a significant part of Australia's policy to address the medical workforce issue of geographic maldistribution to ensure that communities in rural and remote Australia have access to adequate general practice (GP) services. This policy has not been without problems, particularly in the areas of assessment of skills and qualifications, appropriate orientation and integration into Australian communities, and retention of these doctors within rural and remote communities. To date there has been little evidence-based research on the role of OTDs in the medical workforce in Australia. This study explores the service provision and quality of care provided by OTDs using the 5 Year OTD Scheme as the case study. In doing so, it assesses the adequacy of this strategy and discusses the implications for future workforce policies and programs."--Abstract.

It argues that IMGs are ‘othered’ and ultimately positioned as an underclass, a positioning validated and reinforced by the intersecting inequalities of class, race and nation. It also suggests that the positioning of IMGs is organised through the dimensions of structural power, hegemonic power and interpersonal power, which allow an exploration of power relations between the structures of the health system, the Australian medical profession and the agency of IMGs. The Australian narrative presented to the world espouses a community of social justice and human rights. Instead, an historical lens traces the formation and persistence of difference represented in ethnocentrism, racism and xenophobia from 1788 to the present. The research presented is multidisciplinary in scope. An anti-oppressive theoretical framework enables the voices of lived experience to penetrate throughout and a social justice platform engages the participants and the reader into the interwoven conversations. The data set comprises a focus group, 10 individual interviews with IMGs and a selection of inquiry submissions revealing rich and sometimes shocking evidence to paint a stark picture. Other medical voices join the conversation via media responses to revelations of experiences not only by IMGs but also by Australian-trained doctors. It exposes a toxic culture endemic with bullying and sexual harassment.This book is of interest to practitioners, researchers and administrators in the fields of medical education, human resource management, legal studies, health sciences, social sciences, health services, government departments, universities and hospitals, as well as those tasked with duty of care and the provision of a safe workplace. The voices gifted to this study raise awareness of current issues within medicine in Australia at a very personal level and begin to formulate a policy and practical response to address these disturbing revelations.

Over the past decade, the number of doctors and nurses has increased in many OECD countries, and foreign-born and foreign-trained doctors and nurses have contributed to a significant extent. New in-depth analysis of the internationalisation of medical education shows that in some countries (e.g. Israel, Norway, Sweden and the United States) a large and growing number of foreign-trained doctors are people born in these countries who obtained their first medical degree abroad before coming back. Book excerpt: "Australian estimates of the doctor-to-population ratios for the cities, regional and remote areas seem to exceed the 0.71:1,000 benchmark ratio of the Commonwealth Department of Health and Ageing. However, statistics are misleading because they do not account for the time and distance involved to see a doctor in the less densely populated rural and remote areas. This is a qualitative study of the problem of retention and recruitment of rural doctors. In 2006-08, 13 in-depth structured interviews were conducted to target the three main interest groups - Federal and State governments, professional associations and training institutions (medical schools and hospitals). We analyse the results within the framework of the public interest theory and the special interest theory. We found from the interviews that in the future, the provision of better financial and non-financial incentives tailored specifically to GPs working in the rural areas would be crucial to attracting and retaining more doctors in these areas. The conclusion is that the rich information that was analysed in this paper could only have been obtained from face-to-face in-depth interviews, and not from publicly accessible sources. Qualitative research is a useful complement to the traditional quantitative studies of economic issues and should be conducted more often in the future"--Abstract.

Book excerpt: The human resources crisis in the health sector has been gathering attention on the global stage. To date, however, most of this attention has focused on shortages of health human resources (HRH) at the national level. At least as important are problems at the sub-national level. Massive geographic and skill mix imbalances are reflected in the perilous undersupply of HRH in most rural areas. Virtually all Sub-Saharan African countries suffer from significant geographic imbalances. Very little substantive information or documentation exists on the problem. Even less is known about the lessons from policies aimed at addressing urban-rural human resource imbalances, let alone experiences of Sub-Saharan Africa countries, with such policies. There also appears to be a disconnect between the objectives and efforts of policymakers on the one hand and the functioning of national health labor markets and labor market behavior on the other hand. This disconnect hinders policy effectiveness and the efficient utilization of resources intended to narrow urban-rural inequities. In Sub-Saharan Africa government policies, often limited to the management of public sector vacancies, appear to be elaborated, prescribed, and implemented independently of labor market considerations. Partly as a result, they are unable to effectively address urban-rural imbalances, which are an outcome of labor market dynamics. This report discusses and analyzes labor market dynamics and outcomes (including unemployment, worker shortages, and urban-rural imbalances of categories of health workers) from a labor economics perspective. It then use insights from this perspective as a basis for elaborating policy options that incorporate the underlying labor market forces. The goal of the study is to address undesirable outcomes (including urban-rural HRH imbalances) more effectively. The book is thus suitable for researchers, policy analysts and policy makers with an interest in understanding and improving the allocation of human resources for health in the developing world.

Many government programs and incentive schemes have been established to encourage doctors to practise in rural areas. However, there is little robust evidence of the effectiveness of such incentive schemes. The aim of this study is to examine the preferences of general practitioners (GPs) for rural location using a discrete choice experiment. This is used to estimate the probabilities of moving to a rural area, and the size of financial incentives GPs would require to move there.
