Population aging is one of the most important trends of the 20th and 21st centuries, and it is occurring worldwide, especially in more developed countries such as the United States. Population aging is more rapid in rural than urban areas of the U.S. In 2010, 15 percent of the nonmetropolitan compared to 12 percent of the metropolitan population were 65 years of age and older. By definition rural communities have smaller sized populations, and more limited healthcare, transportation and other aging-relevant services than do urban areas. It is thus especially important to study and understand aging in rural environments. Rural Aging in 21st Century America contributes evidence-based, policy-relevant information on rural aging in the U.S. A primary objective of the book is to improve understanding of what makes the experience of rural aging different from aging in urban areas and to increase understanding of the aged change the nature of rural places. The book addresses unique features of rural aging across economic, racial/ethnic, migration and other structures and patterns, all with a focus on debunking myths about rural aging and to emphasize opportunities and challenges that rural places and older people experience.

The plan of the book is as follows: In Chapter 1, we describe how rural Japan has changed socially and demographically. We explain how rural depopulation has led to political consolidation, and how the welfare system in Japan is placing more responsibility and autonomy on the municipalities. Some rural towns in Japan, such as Kawanehonchō, are actively responding to the demographic challenges initiated by municipal governments that have the advantage of developing unique programs reflecting the voices of local residents. Chapter 2 describes Kawanehonchō, explains how it became a rural depopulated town, and discusses why the town provides an important example to understand and discuss rural aging comparatively. In Chapter 3, we review theoretical frameworks (collective efficacy theory and social capital) to understand the inseparability of successful aging from the quality of neighborhoods and communities. We explain our research methods in Chapter 4. In Chapter 5, we examine Kawanehonchō with secondary data. Chapter 6 summarizes the findings based on observations of activities provided for the elderly in this town, and Chapter 7 sheds light on rural aging through the eyes and words of its leaders. Chapter 8 presents our findings from our survey of elders participating in two community programs developed locally and specifically for them. Book excerpt: The social life of older rural Americans is made up of relationships formed through kinship, their neighborhoods, and the organizations to which they belong. These social institutions are shaped by the ways people use them, and therefore change through time. In this precedent-setting study, John van Willigen uses the concept of social network to investigate life-course changes in the relationships of older people within the context of community history. Gettin' Some Age on Me grew out of a study of more than 130 older people in a rural Kentucky county. They were interviewed concerning their relationships with others, and data were collected on the give and take of support that is part of their social life. An understanding of community life and history, developed through interviews and period documentation, provided a context for understanding the changes these people have experienced over time. Finally, related studies by other researchers provided a framework for interpreting rural and urban differences. Van Willigen skillfully interweaves these various accounts to reveal fundamentally important patterns. It is clear that these other people should be viewed not as dependent and isolated but as important sources for social support; that even though their social relationships decline in number late in life, early in the post retirement period there is an apparent increase in social involvement; and that older people are much less isolated in the rural community studied than in many urban areas. This book makes a substantial contribution to the very limited literature on aging in rural America. It is important reading for social gerontologists and for all social scientists with an interest in American communities.

"Although research activity in social gerontology has increased significantly in America during the past three decades", observes Dr. Parks, "very little of this research has focused upon rural elderly persons. Such a deficiency is not altogether surprising, because innovation and new fields of inquiry typically emerge in urban research centers, where urban populations tend to be studied. Then, only gradually, do these advances in scientific knowledge spread to the more remote areas. Since social gerontology is a relatively new discipline, research in this area has only recently been conducted in non-urban locations. The failure of researchers to concern themselves with the study of rural aged populations is directly related to the very nature of their work. For example, most social gerontologists are urban dwellers or persons who at least work in urban institutions. Therefore, their research efforts have been mainly directed toward studies of human aging among those in metropolitan settings. Therefore, it has become quite self-evident in government and social scientific circles that the rural elderly populations of America have not been studied adequately, and there has been asimilar lack of empirical research conducted on black aged populations as a whole. In summary, over the past few decades, the bulk of gerontological research has centered primarily on elderly white populations with the result being that very little is known about elderly blacks and/or elderly rural black Americans. The purpose of this nationally-sponsored data-base study is specifically to correct this void in scientific research with an eye toward providing information needed for effective and responsible social policy".

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.
