Book excerpt: Introduction: Women living with HIV (WLHIV) account for more than half of all people living with HIV (PLHIV) globally (53%). In Indonesia, women aged 15 years and older represent 38% of the total number of PLHIV. HIV infection adds a significant burden on women, often leading to severe detrimental impact, not only on themselves, but also on their families. Despite the manifold increase of HIV infections in Indonesia in general, and among women in particular, little evidence exists about HIV-risk factors among WLHIV and impact on themselves and their families. The aims of this study were four-fold, to: (i) understand and compare HIV-risk factors among WLHIV and the impact of HIV on themselves and their families in Yogyakarta and Belu, Indonesia, (ii) understand and compare HIV-risk factors and the impact of HIV on WLHIV and MLHIV in Yogyakarta and Belu, (iii) understand and compare the perspectives and experiences of women/men living with HIV and healthcare providers about the determinants of access to HIV care services in Yogyakarta and Belu, and (iv) explore and compare the current and future perspectives of policy makers in Yogyakarta and Belu about policies and programs to address HIV and its impact on WLHIV and their families. -- Methods: This thesis employed a qualitative design and one-on-one in-depth interviews with four groups of participants: (i) WLHIV (n=52), (ii) men living with HIV (n=40), (iii) healthcare providers (n=20), and (iv) policy makers (n=9) in both study settings. Participants were recruited using a combination of purposive and snowball sampling techniques. Data were analysed using a framework approach with five steps of qualitative data analysis. This analysis was guided by the logical model for behavioural and environmental determinants diagnosis, the conceptual framework for socio-economic impact of HIV on households and HIV stigma framework, and the access to healthcare framework. The consolidated criteria for reporting qualitative research (COREQ) with a 32-item checklist was also employed to represent best-practice in qualitative research as well as to support a transparent and comprehensive reporting of this qualitative study. Ethics approvals were obtained from Flinders University, South Australia (No. 8286) and Duta Wacana Christian University, Indonesia (No. 1005/C.16/FK/2019). -- Results: The identified risk factors which facilitated HIV transmission among women and men living with HIV included: (i) poor knowledge of HIV and condoms, (ii) behavioural factors, (iii) socio-environmental factors, (iv) financial factors, (v) intimate partner violence, and (vi) religious and cultural factors. The study also identified several impacts of HIV on women and men living with HIV and their families: (i) stigma and discrimination, (ii) the psychological impact, (iii) family disputes and separation, and (iv) the physical impact. It also identified determinants of access to HIV care services among women and men living with HIV, including availability, approachability, affordability, appropriateness, acceptability of the services, and the ability of women and men living with HIV to reach the services, perceive need for the services, pay, engage in and seek the services. The study also reported HIV policies and programs that were in place in Yogyakarta and Belu, based on the perspectives of policy makers. -- Conclusions: This thesis reports several risk factors for HIV transmission among women and men living with HIV and outlines a range of factors, including cultural norms, values, practices and religious beliefs that influenced sexual relations, practices and behaviours of women and men living with HIV as novel findings or the independent contributions of this study to knowledge of risk factors for HIV transmission. It also reports on several negative impacts HIV has on women and men living with HIV and their families, and outlines social perceptions, norms, values, cultural practices, religious beliefs and moral judgements about HIV, PLHIV, illicit drugs, sex, and spousal relationships that drive stigma and discrimination as novel findings of this study, which contribute to the understanding of HIV impact on PLHIV and their families. It also reports determinants of HIV care access among PLHIV, and presents traditional treatments of HIV using traditional medicines through traditional healers, and the social influence of families, friends and neighbours in supporting traditional treatments as novel findings which are also key contributions of this study to the knowledge about barriers to HIV care access among PLHIV. The thesis suggests the need for HIV policies in Yogyakarta and Belu, which continue to address both HIV-risk factors and impacts on PLHIV and their families, support and expand the provision and coverage of antiretroviral therapy and HIV interventions or programs and activities. Future large-scale studies to explore socio-cultural and religious aspects that contribute to HIV transmission, HIV impact on women in Indonesia, and hinder their access to HIV care services are recommended.

Book excerpt: It has now been 25 years since the apocryphal report in the CDC Morbidity and Mortality Weekly Report dated June 5, 1981 entitled, “Pneumocystis Pneumonia - Los Angeles”, which announced what was to become HIV/AIDS. HIV has now affected virtually all countries that have looked for it and has had a devastating impact on the public health and medical care infrastructure around the world. HIV/AIDS has also disproportionately affected nations with the least capacity to confront it, especially the developing world nations in Sub-Saharan Africa, South and Southeast Asia, and the emerging republics of Eastern and Central Asia. The pandemic, unlike any other disease of our time, has had profound impacts on the practice of public health itself: bringing affected communities into decision making; demanding North-South partnerships and collaborations; and changing the basic conduct of clinical and prevention trials research. While much has been written in scholarly publications for medical, epidemiologic and disease control specialists, there is no comprehensive review of the public health impact and response to HIV/AIDS in the developing world. This edited volume seeks to systematically describe the emergence and form of the epidemics (epidemiology), the social, community and political response, and the various measures to confront and control the epidemic, with varying levels of success. Of particular importance are strategies that appear to have been useful in ameliorating the epidemic, while contrasting the situation in a neighboring country or region where contrasting prevention or care initiatives have had a deleterious outcome. Common to all responses has been the international multi-sectoral response represented by the Global Fund for HIV/AIDS, Malaria and Tuberculosis, the President's Emergency Plan for AIDS Relief, and the Gates Foundation, among others, to promote HIV pharmacologic therapy in resource-poor settings. The chapter authors will explore the political challenges in meeting HIV/AIDS prevention and care in concert with the public health realities in specific country and regional context.

Available in PDF, EPUB and Kindle. Book excerpt: The number of women living with HIV (WLWH) in Indonesia increased 344 times between 2008 and 2013, from 3,565 to 12,279. Housewives comprised the largest number of new AIDS cases reported in 2013. Many of these women get stigmatized in their communities once their diagnosis is known, and we know little about these experiences of WLWH in Indonesia. This dissertation examines the stigma experiences of WLWH in Jakarta, Indonesia in three ways. Manuscript one reviews the stigma experiences of WLWH in Southeast Asia in order to synthesize the current knowledge, identify existing gaps and make recommendations for future research. The review showed how society's role-based expectations affect the lives of WLWH. Women across Southeast Asia experienced stigma in their daily lives that affected their financial situation, their adherence to medication, and their access to health care. Future interventions using cultural approaches that address the relationships with their family, health care providers, and community members are urgently needed. Manuscript two examines how stigma theories are applicable to the realities of HIV-positive housewives in Indonesia and to expand upon the current theories. Results show that Goffman's stigma theory and other stigma theories cannot be fully applied to the situation of HIV-positive housewives in Indonesia. Most stigma theories focus on individuals who practiced high-risk behaviors and get rejected for that behavior by society; however, these women did not engage in any of these behaviors. Rather than explaining stigma from the individual to the family, to the community, to the society level, many decisions in Indonesia are made by the family for the individual, and the birth family often stigmatizes the women the most for not following the family's recommendations. Also, future stigma theories need to expand to address the areas of stigma prevention, intervention, and community engagement. Manuscript three explores the women's accounts of how they were treated when their HIV status became known and to describe the women's recommendations for programs to support WLWH in Jakarta, Indonesia. Three themes emerged: being treated negatively, receiving acceptance and support, and impact on their children. Women experienced negative treatments from their families, health care providers (HCPs), and neighbors because these groups were misinformed about HIV, fearful of being infected with HIV, and felt that the women should have to live with the consequences of marrying an injection drug user. Seeing women as a victim and realizing nothing could change the reality of their infection caused women to find acceptance and support in their husband's family. The women reported that their children often suffered from the same negative treatment in the community. HIV-focused education programs were identified as the most-needed step to eliminate the stigma of living with HIV. These programs were viewed as urgently needed for societal education. To positively impact the fears and misconceptions of family, HCPs, and community members, more information about HIV is critically needed in Indonesia. This can be delivered in a form of role-plays, posters, and workshops designed specifically for communities and healthcare providers.

Book excerpt: The purpose of this study was to conduct a phenomenological inquiry into the impact HIV/AIDS-related stigma has on People Living with HIV/AIDS (PLHA) and their family members in Ghana and the overall relationship family members have with PLHA. The study explored the concept of stigma in the Ghanaian context, ways in which it is expressed, factors influencing HIV- related stigma and its consequences on both PLHA and their family members. Strategies that PLHA and their family members consider for effective HIV- related stigma prevention were also explored. The study further explored some of the gender- biased nature of HIV- related stigma in Ghana. Data was gathered qualitatively through interviews with five PLHA and their discordant family members. Interviews were transcribed and translated into English, coded and analyzed. After inductively establishing themes and categories, final confirmatory analysis was deductively established, by using the Bronfenbrenner's ecological model and Symbolic interaction theory to affirm the authenticity and appropriateness of the inductive content analysis. The study found that HIV- related stigma begins with serostatus disclosure. Stigma is manifested in myriad contexts including the family, community, healthcare institutions and gender. The major factors influencing stigma are insufficient knowledge of HIV transmission, fear and misconception of HIV created by the media, cultural and religious factors as well as poverty. Family members experienced similar stigma as PLHA, such as loss of jobs, loss of social network, loss of identity and self stigma. However extreme impacts such as suicidal thoughts were only experienced by PLHA. The impact of HIV- related stigma is worst for women because of beliefs and values relating to gender- role expectations. While women accept and support their husbands when they have HIV/AIDS, women are often neglected and abandoned by their husbands. To address this stigma, participants suggested house to house education, financial support from the government, revision of educational content especially discontinuation of negative images of HIV/AIDS used by the media. Implications for this study in the areas of research, practice and policy are provided.
