During an MS attack, inflammation occurs in areas of the white matter of the central nervous system (nerve fibres that are the site of MS lesions) in random patches called plaques. This process is followed by destruction of myelin, which insulates nerve cell fibres in the brain and spinal cord. Myelin facilitates the smooth, high-speed transmission of electrochemical messages between the brain, the spinal cord, and the rest of the body. The initial symptom of MS is often blurred or double vision, red-green colour distortion, or even blindness in one eye. Most MS patients experience muscle weakness in their extremities and difficulty with co-ordination and balance. Most people with MS also exhibit paresthesias, transitory abnormal sensory feeling such as numbness or pins and needles. Some may experience pain or loss of feeling. About half of people with MS experience cognitive impairments such as difficulties with concentration, attention, memory, and judgement. This volume presents leading research from around the globe.

In the past five years, this bewildering array of more than 100 effector molecules and associated cell surface receptors has been simplified by study of cytokine and cytokinereceptor structure; elucidation of convergent intracellular signalling pathways; and molecular genetics, and targeted gene disruption to 'knock-out' production of individual cytokines in mice. It is also now clear that the pathophysiology of infectious, autoimmune and malignant disease can bepartially explained by the induction of cytokines and the subsequent cellular response. Viral homologues exist for many cytokines and receptors and genetic variations in cytokine production may influence response to pathogenic stimuli. Cytokine and cytokine antagonists have shown therapeuticpotential in a number of chronic and acute diseases. Substances released from activated immune cells (cytokines) stimulate peripheral nerves, thereby signaling the brain and spinal cord that infection/inflammation has occurred. Additionally, peripheral infection/inflammation leads to de novo synthesis and release of cytokines within the brain and spinal cord. Thus, cytokines effect neural activation both peripherally and centrally. Through this communication pathway, cytokines such as interleukin-1, interleukin-6 and tumor necrosis factor markedly alter brain function, physiology and behavior. One important but underrecognized aspect of this communication is the dramatic impact that immune activation has on pain modulation. The purpose of this book is to examine, for the first time, immune-to-brain communication from the viewpoint of its effect on pain processing. It is aimed both at the basic scientist and health care providers, in order to clarify the major role that substances released by immune cells play in pain modulation. Fatigue is perhaps the single most prevalent and disabling symptom of the multiple sclerosis, and limits patients' activity more than any other MS symptom. The identification of fatigue as a distinct clinical entity requires both art and science, and most of all, a willingness and ability to listen carefully to patients and their families. The physician's ability to obtain a comprehensive history requires a full understanding of the circumstances in which fatigue occurs (physical, cognitive, and psychosocial) and demands consideration of a large number of disorders, including anxiety, depression, excessive daytime sleepiness, pain, and spasticity, all of which may mimic or contribute to fatigue. While fatigue is almost exclusively a subjective experience, depending on the patient's ability to understand and report this symptom, there are effective methods for identifying the existence of fatigue, determining its severity, and distinguishing it from related or contributing disorders such as depression, pain, and sleep disorders. Readers will learn that fatigue need not be tolerated by the MS patient. Various therapies, support systems, and treatment of underlying affective disorders can all alleviate fatigue or reduce its impact, restoring the patient's energy levels and ability to participate in life. Virtually every MS patient with fatigue can benefit from intervention, and failing to treat the symptom of fatigue with the respect that it deserves is a serious detriment to patient care.
