Book excerpt: Purpose: The purpose of this study was to uncover adolescents' beliefs regarding the development and prevention of dental caries using the Theory of Planned Behavior (TPB)5,6. Methods: Adolescents aged 13 to 17 years, with active dental caries diagnosed within the past year, were recruited at the Nationwide Children's Hospital dental clinic prior to a routine dental hygiene appointment. Researchers conducted semi-structured interviews rooted in TPB to determine the adolescents' understanding of the causes and outcomes of dental caries. Once completed, interviews were transcribed and qualitatively analyzed by the research team. Particular attention was given to TPB constructs including: adolescents' attitudes, subjective norms, and perceived behavioral control associated with dental caries. Researchers identified themes and used these themes to create a study questionnaire to be used in future studies. Results: Fifteen patients met the inclusion criteria and were interviewed for this study. Common themes were identified in each construct of TPB (attitudes, subjective norms, and perceived behavioral control). Attitudes: Most patients believed that both diet and poor oral hygiene practices were causes of dental caries. However, only half said modifying their diet would prevent dental caries while the majority believed maintaining good oral hygiene would prevent dental caries. Five patients did not know they had dental caries. When the remaining ten patients were asked how they felt when they were told they had dental caries, most expressed disappointment. Subjective Norms: Several patients learned what causes dental caries from their families because a family member had dental caries. Many patients worried about what their friends or family think about them having dental caries. Many patients feel that their friends or family surround them with sweet drinks and sugary snacks. Perceived Behavioral Control: Most patients believed they could personally stop getting dental caries, but they reported having something in their lives that would make it difficult. Patients felt surrounded by and obligated to consume sweet drinks and sugary snacks. They also felt they were too busy, too forgetful, or unmotivated to maintain good oral hygiene. Conclusions: Our study reveals we don't understand the beliefs or the influences on our patients as well as we should. The TPB study questionnaire developed using the themes we identified will help us conduct future research to better identify adolescents' knowledge, attitudes, and beliefs regarding the development and progression of caries. Having such information could provide valuable insights for creating behavioral interventions designed to prevent and treat adolescent dental caries.

In addition, the IOM stresses three key areas needed for successfully maintaining oral health as a priority issue: strong leadership, sustained interest, and the involvement of multiple stakeholders from both the public and private sectors. Advancing Oral Health in America provides practical recommendations that the Department of Health and Human Services can use to improve oral health care in America. The report will serve as a vital resource for federal health agencies, health care professionals, policy makers, researchers, and public and private health organizations.
