Book excerpt: A review of literature revealed that the control of cigarette smoking could do more to improve health than any other single action in the field of preventive medicine. In Ontario, since 1989, both Public Health Units and Boards ofEducations have been mandated to provide educational studies related to tobacco use prevention. Even given this fact, there has been an increase in smoking behaviQurs at an earlier age and in females in particular. Smoking prevention progralns must use the most effective means to assist students to obtain the knowledge and skills required to remain or becom'e nonsmokers. In the Niagara Region, PAL smoking prevention programs are offered in some, but not all, schools. As a form of program evaluation, this research sought to determine if students who had PAL could answer correctly a greater number of smoking-related questions than students who did not have this program. Findings reported that students who had PAL in Grade 6 were able to correctly answer more knowledge-based questions (at a statistically significant level), could provide ways to refuse cigarettes at a greater rate, and were able to provide more reasons for remaining nonsmokers. Students who had smoking prevention programming re,ported smoking behavio'urs at a lower rate than those who did not receive this type of program.

The most efficient tools for helping individuals never to start or successfully to stop using tobacco should be developed; effective tobacco control endeavors are required to counteractthe actions ofthe powerful and influential tobacco manufacturers. With the help and under the aegis ofWHO, DICC, IUATLD, ISFC, IOCD, and IUHPE, an international alliance for health and against tobacco shouldunite all those who are engaged in this fight.

Nevertheless, in many of the countries of the Americas increasing urbanization and changing lifestyles have resulted in greater use of tobacco. At the same time, health and disease patterns in the developing countries of the hemisphere are undergoing and "epidemiologic transition," with chronic noncommunicable diseases displacing infectious diseases as the primary causes of morbidity and mortality. In the next few decades, these countries can be expected to face, and will need to prepare for, an epidemic of smoking-related diseases, particularly lung cancer, and a proportionaly imposing burden on national health services. To confront that epidemic and alleviate that burden, greater understanding is necessary of the historical, economic, political, and publich health aspects of the supply, demand, and consumption of tobacco. This publication aims to further that understanding by providing information on the general socioeconomic context, the tobacco industry, tobacco use, smoking and its effects on health, and smoking prevention and control activities in the countries of Latin America, the Caribbean, and Canada. Most of the information contained in this pulication cannot e found in any other single source. It will thus serve as a baseline of data for developing countries in the Region as they engage in the complex issues involved in preventing and controlling tobacco use. Health workers, researchers, and policy makers can use the information presented here to build a coalition against what may prove to be the most important pulic health problem of the 1990sm.
