Book excerpt: Concussions and attention-deficit/hyperactivity disorder (ADHD) are both major public health concerns that affect many children and adolescents every year. These conditions can present with an overlap of symptoms, which makes treatment and assessment more difficult. A diagnosis of ADHD can affect baseline test scores as well as post-injury scores for concussed athletes. The Immediate Postconcussion Assessment and Cognitive Testing battery (ImPACT) is currently one of the most widely used computer based tests to assess and manage concussions in athletes. There is limited research on whether and/or how a diagnosis of ADHD affects ImPACT scores in an adolescent population. Therefore, the purpose of this study was to examine if having a premorbid diagnosis of ADHD affects baseline and post-injury ImPACT scores for adolescent athletes who have experienced sports-related concussions. Mean ImPACT baseline and post-injury composite scores from concussed adolescent athletes with and without premorbid ADHD diagnoses were compared. No significant differences were found between the groups on baseline ImPACT composite scores. A significant difference was found between post-injury impulse control composite scores between the two groups. Baseline-to post-injury change scores were compared between the two groups; no significant differences were found. Although the hypotheses of this study were largely unsupported, these results, and those obtained by other researchers, suggest that concussed athletes with ADHD may not recover the same way as other athletes, and may have different ImPACT score patterns than athletes without ADHD. Future research should continue to examine the effects of ADHD on ImPACT test performance.

Elon BrainCARE Research Institute not only serves as a service to sports medicine and student health staff at Elon, but also a model of co-mentored undergraduate research training for students headed into both academic and clinical health profession fields. Book excerpt: This study sought to examine how effectively concussed college athletes are able to predict the effects of their concussion on their neuropsychological testing performance. Performance prediction tasks are often used to assess an individual's level of self-awareness, an area of deficit that has commonly been noted following more severe forms of traumatic brain injury. Little research has been conducted on the effects of sports-related concussion on an athlete's self-awareness. Surveys were administered to athletes before and after neuropsychological testing to assess an athlete's awareness of any deficits since their concussion and whether the athlete expected their concussion to affect their testing performance. Results indicate that athlete report of symptoms is most likely not a good indicator of when an athlete can be safely returned to play following a concussion due to inconsistent report of symptoms and the possible presence of impaired self-awareness or denial.

Book excerpt: The following experiment was designed to determine if there is a relationship between self-reported symptoms of Attention-Deficit/Hyperactivity Disorder (ADHD) and performance on both a computerized measure of concussion, the Concussion Resolution Index, and the Standardized Assessment of Concussion. Symptoms of ADHD were measured using the Conner's Adult ADHD Rating Scale (CARRS). Two hundred and sixty-two student athletes were used in the analyses to determine if there was a relationship between higher ADHD symptom endorsement and worse performance on the baseline concussion measures. Correlational Pearson (bivariate) and comparative (t-test) analyses indicated that in general, those with higher self-endorsement of ADHD symptoms did not perform significantly differently than did those with lower rates of symptom endorsement. Further, athletes reporting a previous diagnosis of ADHD did not perform differently on the baseline measures than did those who were not previously diagnosed with ADHD, despite typically endorsing more symptoms on the CAARS. The results indicate that self-reported symptoms of ADHD are unrelated to performance on baseline measures of concussion, but that further research is needed to determine if a relationship between ADHD and performance on concussion measures exists for both baseline and post-trauma assessment.
