Nous avons analysé les recours aux SU par le médecins libéraux et cherché à savoir s'ils constituent un facteur prédictif d'hospitalisation et de prise en charge spécialisée. METHODE: Etude prospective descriptive mono-centrique non randomisée réalisée sur 3 semaines dans le SU du CHU de Pointe-à-Pitre. Ont été inclus tous les patients munis d'un courrier d'admission. Les médecins concernés ont été joints par télephone. Les variable analysées concernaient les caractéristiques de la consultation en ville, les motifs de recours au SU et l'avis du médecin sur la prise en charge. Cette population a ensuite été comparée à une population témoin. Les principaux critères de comparaison étaient la demande d'examen complémentaire, d'avis spécialisé, le taux d'hospitalisation et la Classification Clinique des Malades des Urgences (CCMU). RESULTATS: 154 patients inclus. Sex-ratio 1,17. Les pathologies somatiques concernent plus de 80% des recours. Les principaux motifs de recours aux urgences sont la demande d'examen complémentaire (57%), d'avis spécialisé (45%) et/ou d'hospitalisation (36%). Leur proportion aux urgences est plus importante chez les patients adressés (respectivement 82% vs 49%,36% vs 17% et 38% vs 9%), qui sont principalement classés CCMU 2 (74%) et CCMU 1 (13%). CONCLUSION: Le recours aux urgences par le médecin généraliste semble souvent motivé par le manque d'accessibilité aux ressources disponibles en extrahospitalier. Le degré d'urgence n'apparaît pas comme le facteur principal. Le fait d'être adressé semble être un facteur prédictif de prise en charge plus spécifique.

Clinicians have to rely on their own experience, which is often limited, when faced with several delicate diagnostic and treatment issues. This book aims to approach for the first time in a comprehensive way this emerging area of clinical psychiatry. The World Health Organisation has found that mental disorders such as depression and psychosis rank in the top 10 of leading causes of disability in the world, creating a significant social, emotional and economic burden for young people, their families and society. Early detection of these potentially disabling disorders and treatment at the time of initial onset can greatly improve long-term outcomes and effect dramatic cost-savings. The related Birchwood title has sold nearly 2300 copies and is popular at psychiatry conferences. From an R&D perspective, industry have a stake in this area. Obviously, if biomarkers or other prodromal evidence can be determined in various disorders, then drugs will start to be dispensed earlier. The topic has increased in popularity as a subject for satellite symposia e.g Jeffrey Lieberman chaired the Bristol Myers Squibb symposia at the American Psychiatric Association. This dealt with early intervention in schizophrenia and the need for novel approaches in the prodromal phase. In addition, more symposia (and research) look at disorders "across the lifespan." This phrase indicates an approach to research, diagnosis and treatment from a more quality-of-life vantage and early diagnosis is part of this movement.
