Book excerpt: L’objet principal de notre étude consiste à évaluer les conséquences des violences au sein du couple sur la santé mentale et adaptative des femmes, de façon à combler le manque de données françaises en ce domaine. Pour cela, nous avons interrogé 200 femmes, desquelles 73 sont victimes de violences psychologiques et/ou physiques de leur compagnon, 41 de violences sexuelles et 86 constituent le groupe témoin. Les variables de santé mentale et adaptative mesurées sont : la dépression, le trouble de stress post-traumatique, les cognitions post-traumatiques, l'estime de soi, les stratégies de coping, le soutien social, l'inadaptation à la vie quotidienne et la qualité de vie. Les analyses statistiques montrent que plus les violences sont sévères (sexuelles > psychologiques et/ou physiques > absence), plus les femmes présentent une santé mentale et adaptative dégradée, et ce, pour toutes les variables citées ci-dessus. Nous observons aussi que les femmes victimes de violences d'un précédent partenaire ont des scores d'estime de soi et de disponibilité du soutien social plus faibles, ainsi que des scores de dépression, autocritiques, blâme, inadaptation plus élevés que les femmes du groupe témoin. Enfin, nous constatons que parmi les victimes de violences au sein du couple, celles qui font la démarche de consulter dans un service de médecine légale ont des scores de dépression, déni, inadaptation plus élevés et des scores d'humour, expression des sentiments, réinterprétation positive et qualité de vie plus bas. Ces résultats sont essentiels pour concevoir les prises en charge thérapeutiques des victimes et les campagnes de prévention des violences au sein du couple.

La moitié d’entre elles souffrirait d'etat de stress post-traumatique dans ce contexte. La thérapie cognitivo-comportementale étant reconnue comme la psycho-thérapie la plus efficace dans la prise en charge de l'état de stress post-traumatique (E.S.P.T), nous avons voulu déterminer si pour cette population la thérapie cognitivo-comportementale (T.C.C) pouvait s'appliquer de façon spécifique et selon quelles procédures. Après avoir défini les différents aspects des violences conjugales et les caractéristiques cliniques de l'E.S.P.T, nous avons décrit les bases de la T.C.C de l'E.S.P.T, en nous interessant plus particulièrement à la population de femmes victimes de violences conjugales. Pour approfondir cette dernière qestion, nous avons éffectué une revue de la littérature en interoogeant plusieurs bases de données (Psycinfo, Pubmed, Pascal) à l'aide des mots-clefs suivants : "post-traumatic stress disorder", "P.T.S.D", "battered women", "domestic violence", "interpersonnal violence", "cognitive behavioral therapy". Il apparait qu'il existe peu d'études relatives aux modalités de prise en charge de l'état de stress post-traumatique par T.C.C chez ces femmes victimes de violences conjugales. Ces études montrent toutefois que la T.C.C a sa place dans la prise en charge de ces victimes, à condition de s'adapter aux spécificités de cette population. A savoir, inclure dans le programme de thérapie: la mise en sécurité, la psycho-éducation surles violences conjugales et ses effets, une approche cognitive sur les sentiments dce honte et de culpabilité. La T.C.C devrait cependant être inclue dans une approche plus globale de ces patientes (sociale et judiciaire) afin de mieux tenir compte des différents aspects de cette problématique.

Book excerpt: INTRODUCTION : Le médecin généraliste figure parmi les intervenants les plus sollicités par les victimes de violences conjugales. Le fait de vivre le cycle de la violence rend souvent les femmes violentées ambivalentes rendant leur prise en charge difficile. La violence conjugale n'épargne pas les personnes âgées. Dans le contexte de vieillissement de la population et de la récente évolution du droit des femmes il apparaît important de comprendre les besoins en termes de soins et d'accompagnement des victimes et les éventuelles spécificités chez les femmes de plus de 65 ans. MATÉRIEL ET MÉTHODE : Il s'agit d'une étude qualitative à partir d'entretiens individuels semi-dirigés enregistrés par dictaphone puis anonymisés et retranscrits intégralement. Les données ont été analysées en triangulation partielle : par théorisation ancrée pour la première partie et par analyse phénoménologique pour la deuxième. RÉSULTATS : 18 femmes ont été incluses dont 6 de plus de 65 ans. Les femmes faisaient appel aux intervenants des secteurs sociaux, juridique, psychologique et médical. Leurs besoins étaient regroupés en trois catégories : 1. Écoute. 2. Le besoin de soutien et information en vue de la séparation 3. Le besoin d'action : aide. Les attentes des femmes de plus de 65 ans étaient similaires mais celles toujours en couple ne souhaitaient généralement pas quitter le conjoint violent et développaient des stratégies d'adaptation. Les mêmes freins au départ étaient cités mais les femmes de plus de 65 ans se sentaient condamnées à rester à cause de leur âge. DISCUSSION : L'originalité de notre étude est de s'être intéressée à tous les acteurs de la prise en charge des femmes victimes. Nous sommes arrivées à saturation des données pour la première étude. Le recrutement a été difficile pour les plus de 65 ans. Les réponses ont pu être orientées par le fait que les participantes étaient toutes volontaires. On retrouve des différences dans le vécu et les attentes des femmes des plus de 65 ans qui motiveraient la réalisation d'études plus importantes, notamment quantitatives.

Under the rubric of violence against women the two overlapping issues have received increasing attention. Much of the response to these forms of violence has focussed either on advocacy, providing adequate health and legal services and ensuring safety of victims once violence is disclosed, or on screening for violence in the context of health services. When the word "prevention" is used, it is usually in reference to secondary and tertiary prevention - helping women get out of violent situations and preventing further violence. Little attention has been given to primary prevention - addressing the root causes with the goal of reducing the number of new cases. Recent hallmark publications have called for increased investment in primary prevention. As part of the Global Campaign for Violence Prevention, WHO is giving increased attention to the primary prevention of intimate partner violence and sexual violence. Preventing intimate partner and sexual violence against women: taking action and generating evidence is an important new tool for violence prevention researchers, practitioners and advocates provides a planning framework for developing policies and programmes for the prevention of intimate partner and sexual violence. This guide outlines the nature, magnitude, risks and consequences of intimate partner and sexual violence. It outlines strategies to prevent these forms of violence against women and describes how these can be tailored to the needs, capacities and resources of particular settings. It also emphasizes the importance of integrating scientific evaluation into all prevention activities in order to expand current knowledge of what works. The manual describes interventions of known effectiveness, those supported by emerging evidence, and those that could potentially be effective, but have yet to be sufficiently evaluated for their impact. For instance, school-based programmes to prevent violence occurring in "dating relationships" have been shown to be effective in randomized trials in the USA and Canada. Evidence is emerging for the effectiveness of empowerment approaches which use microfinance combined with gender-equality training or are based on communications and relationship skills training such as the Stepping Stones training package, which has been widely used in low- and middle-income countries. Strategies that aim to prevent intimate partner and sexual violence through reducing the harmful use of alcohol also show promise. A six-step framework for planning, implementing and evaluating such prevention programmes and policies is presented. This document is primarily aimed at policy-makers, programme planners and donors from public health and related sectors who seek to advance the prevention of intimate partner and sexual violence against women. In addition to the principal audience, other interested parties will include those working in other government sectors such as education, child welfare, social care, criminal justice and departments of women or gender equality; advocates from civil society organizations; local authorities; environmental and urban planners; and researchers."

The measuring empowerment framework, rooted in both conceptual discourse and measurement practice, illustrates how to gather data on empowerment and structure its analysis. The framework can be used to measure empowerment at both the intervention level and the country level, as a part of poverty or governance monitoring. The authors first provide a definition of empowerment and then explain how the concept can be reduced to measurable components. Empowerment is defined as a person's capacity to make effective choices--that is, the capacity to transform choices into desired actions and outcomes. The extent or degree to which a person is empowered is influenced by personal agency (the capacity to make purposive choice) and opportunity structure (the institutional context in which choice is made). Asset endowments are used as indicators of agency. These assets may be psychological, informational, organizational, material, social, financial, or human. Opportunity structure is measured by the presence and operation of formal and informal institutions, including the laws, regulatory frameworks, and norms governing behavior. Degrees of empowerment are measured by the existence of choice, the use of choice, and the achievement of choice. Following the conceptual discussion and the presentation of the analytic framework, the authors show how the measuring empowerment framework can be applied using examples from four development interventions. Each example discusses how the framework guided analysis and development of empowerment indicators. They also present a draft module for measuring empowerment at the country level. The module can be used alone or be integrated into country-level poverty or governance monitoring systems that seek to add an empowerment dimension to their analysis.
