Book excerpt: The National Institute on Disability and Rehabilitation Research (NIDRR) is the principal federal agency supporting applied research, training, and development to improve the lives of individuals with disabilities. NIDRR's mission is to generate new knowledge and promote its effective use in improving the ability of persons with disabilities to perform activities of their choice in the community, as well as to expand society's capacity to provide full opportunities and accommodations for its citizens with disabilities. NIDRR prides itself on being proactive in establishing program performance measures and developing accountability data systems to track the progress of its grantees. An electronic annual reporting system is used to collect data from grantees on many aspects of grant operation and outputs. Various formative and summative evaluation approaches have been used to assess the quality of the performance and results of the agency's research portfolio and its grantees. Prompted by the need to provide more data on its program results, in 2009 NIDRR requested that the National Research Council (NRC) conduct an external evaluation of some of the agency's key processes and assess the quality of outputs produced by NIDRR grantees (National Institute on Disability and Rehabilitation Research, 2009a). Review of Disability and Rehabilitation Research presents the results of that evaluation.

Book excerpt: The Cooksey Review ("A review of UK health research funding", ISBN 9780118404884), published in December 2006, was commissioned by the Chancellor of the Exchequer to examine the best design and institutional arrangements for a new single funding system for UK health research. The Committee's report supports the decision by Sir David Cooksey to broaden his review beyond its original terms of reference to make wider recommendations for the UK health research framework, for example relating to healthcare treatment priorities, as well as outlining a number of tasks for the Office of Science and Innovation to address. The Review found that although good progress has been made in some areas, further work is needed to ensure that publicly-funded health research is carried out in the most effective and efficient way and to facilitate translation of research findings into health and economic benefits. Amongst its recommendations, the Review proposed that a new body should be established, the Office for Strategic Coordination of Health Research (OSCHR), as the central co-ordinating body for all health research carried out by the NHS and the Medical Research Council. The Committee raises some concerns that although the pharmaceutical sector is of great importance, allied health research such as medical engineering and technology, preventive and public health research should not be overlooked. It also calls on the OSCHR to operate as a light touch organisation that does not complicate the existing successful administrative mechanisms of the Medical Research Council. However, risk assessment is at a crossroads. The largest government funder is the National Institutes of Health (NIH), followed by the Department of Defense (DoD). Almost half of DoD's medical research funding is administered by the Congressionally Directed Medical Research Programs (CDMRP). The mission of CDMRP is to foster innovative approaches to medical research in response to the needs of its stakeholdersâ€"the U.S. military, their families, the American public, and Congress. CDMRP funds medical research to be performed by other government and nongovernmental organizations, but it does not conduct research itself. The major focus of CDMRP funded research is the improved prevention, diagnosis, and treatment of diseases, injuries, or conditions that affect service members and their families, and the general public. The hallmarks of CDMRP include reviewing applications for research funding using a two-tiered review process, and involving consumers throughout the process. Evaluation of the Congressionally Directed Medical Research Programs Review Process evaluates the CDMRP two-tiered peer review process, its coordination of research priorities with NIH and the Department of Veterans Affairs, and provides recommendations on how the process for reviewing and selecting studies can be improved.

Book excerpt: The Mobile Source Emissions Factor (MOBILE) model is a computer model developed by the U.S. Environmental Protection Agency (EPA) for estimating emissions from on-road motor vehicles. MOBILE is used in air-quality planning and regulation for estimating emissions of carbon monoxide (CO), volatile organic compounds (VOCs), and nitrogen oxides (NOx) and for predicting the effects of emissions-reduction programs. Because of its important role in air-quality management, the accuracy of MOBILE is critical. Possible consequences of inaccurately characterizing motor-vehicle emissions include the implementation of insufficient controls that endanger the environment and public health or the implementation of ineffective policies that impose excessive control costs. Billions of dollars per year in transportation funding are linked to air-quality attainment plans, which rely on estimates of mobile-source emissions. Transportation infrastructure decisions are also affected by emissions estimates from MOBILE. In response to a request from Congress, the National Research Council established the Committee to Review EPA's Mobile Source Emissions Factor (MOBILE) Model in October 1998. The committee was charged to evaluate MOBILE and to develop recommendations for improving the model.

Book excerpt: Manufactured vitreous fibers (MVF), also known as synthetic vitreous fibers, are considered to be less hazardous than asbestos to human health. They are used in many thermal- and acoustical-insulation applications as an asbestos substitute or as a filtration medium. The Navy uses MVF in shipboard and onshore applications. To protect Navy personnel from harmful exposures to MVF, the U.S. Navy Environmental Health Center (NEHC) developed occupational exposure standards. The documentation assists industrial hygienists, occupational medicine physicians, and other Navy health professionals in assessing and controlling the health hazards linked with exposure to MVF. In 1997, the National Research Council (NRC) was asked to conduct an independent review of the Navy's toxicological assessment of MVF and to evaluate the scientific validity of its exposure standard of 2 fibers per cubic centimeter of air (f/cm3). The NRC assigned the task to the Committee on Toxicology, which established the Subcommittee on Manufactured Vitreous Fibers, a multidisciplinary group of experts, to determine whether all relevant toxicological and epidemiological data were correctly considered in developing the exposure standard; and to examine the uncertainty, variability, and quality of data and the appropriateness of assumptions used in the derivation of the exposure standard. The subcommittee was also asked to identify deficiencies in the MVF database and, where appropriate, to make recommendations for future research and data development. Review of the U.S. Navy's exposure Standard for Manufactured Vitreous Fibers represents the subcommittee's final report. The committee had expanded its review when in January 1999, the Navy revised its Occupational Safety and Health Program Manual (CNO 1999), changing the occupational exposure limit for MVF to the American Conference of Governmental Industrial Hygienists (ACGIH) threshold limit value (TLV) of 1 f/cm3. The report features recommendations by the subcommittee as well as information gaps found throughout investigation. Overall, the subcommittee found that the Navy made a good start in assessing the health effects of MVF, but needed further research.

Book excerpt: In 1997, the U.S. Environmental Protection Agency (EPA) established regulatory standards to address health risks posed by inhaling tiny particles from smoke, vehicle exhaust, and other sources. At the same time, Congress and the EPA began a multimillion dollar research effort to better understand the sources of these airborne particles, the levels of exposure to people, and the ways that these particles cause disease. To provide independent guidance to the EPA, Congress asked the National Research Council to study the relevant issues. The result was a series of four reports on the particulate-matter research program. The first two books offered a conceptual framework for a national research program, identified the 10 most critical research needs, and described the recommended timing and estimated costs of such research. The third volume began the task of assessing initial progress made in implementing the research program. This, the fourth and final volume, gauged research progress made over a 5-year period on each of the 10 research topics. The National Research Council concludes that particulate matter research has led to a better understanding of the health effects caused by tiny airborne particles. However, the EPA, in concert with other agencies, should continue research to reduce further uncertainties and inform long-term decisions.

Since 1986, the U.S. Environmental Protection Agency has been tasked with identifying extremely hazardous substances and, in cooperation with the Federal Emergency Management Agency and the Department of Transportation, assist local emergency response planners. The National Advisory Committee on Acute Exposure Guideline Levels for Hazardous Substances was established in 1995 to develop acute exposure guideline levels (AEGLs) for high priority toxic chemicals that could be released into the air from accidents at chemical plants, storage sites, or during transportation. The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.
