Book excerpt: Over the past two decades increasing interest has emerged in the contribu tions that the social sciences might make to the epidemiological study of patterns of health and disease. Several reasons can be cited for this increasing interest. Primary among these has been the rise of the chronic, non-infectious diseases as important causes of morbidity and mortality within Western populations during the 20th century. Generally speaking, the chronic, non infectious diseases are strongly influenced by lifestyle variables, which are themselves strongly influenced by social and cultural forces. The under standing of the effects of the behavioral factors in, say, hypertension, thus requires an understanding of the social and cultural factors which encourage obesity, a sedentary lifestyle, non-compliance with anti-hypertensive medica tions (or other prescribed regimens), and stress. Equally, there is a growing awareness that considerations of human behavior and its social and cultural determinants are important for understanding the distribution and control of infectious diseases. Related to this expansion of epidemiologic interest into the behavioral realm 'has been the development of etiological models which focus on the psychological, biological and socio-cultural characteristics of hosts, rather than exclusive concern with exposure to a particular agent or even behavioral risk. Also during this period advances in statistical and computing techniques have made accessible the ready testing of multivariate causal models, and so have encouraged the measurement of the effects of social and cultural factors on disease occurrence.

Book excerpt: From the President of the Research Society on Alcoholism In recent years the alcohol research field has matured and is attracting a substantial number of eager and technically sophisticated researchers. There is a feeling of excitement and premonitions of breakthroughs as more and more of alcohol's actions are being detailed. I, however, have at times been sobered by the perception that the lure of parsimonious explanations and the appeal of studying easily demonstrable effects obscure the critical issues re garding alcohol abuse and alcoholism. Central questions regarding (1) the reinforcing properties of ethanol and other factors responsible for inappro priate consumption of alcohol, (2) the heterogenous characteristics (both bi ologic and sociologic) of the alcoholic population, and (3) the differential predisposition of individuals to alcohol-induced medical problems needed to be vigorously pursued. Researchers who used animals as models for the human condition needed to become more intimately aware of the variety of factors that are of importance in the development of alcoholism in an indi vidual. On the other hand, researchers studying humans needed to attempt to more clearly define and categorize diagnostic criteria and characteristics of various alcoholic populations. Such categorization and continued character ization of alcoholism allows not only for a framework of concepts within which proper animal models for studies of biologic mechanisms can be de veloped but also allows for consideration of the most apt treatment approaches.

This monograph takes as its point of departure the virtual absence of such patients anywhere except the U.S.A. and even then it is a relatively small number of psychologists and psychiatrists who report the overwhelming majority of cases. The book provides the first comprehensive review of the burgeoning literature from the beginning of the century to the present and covers more than 300 articles and books. It should prove of interest to psychologists, psychiatrists, psychotherapists and social workers and is an invaluable reference for students on courses in clinical and abnormal psychology as well as to practising clinicians and social workers. Following an introduction to a selection of the more notable cases, a number of critical issues are examined in ensuing chapters. These are devoted to problems of definition and differential diagnosis; aetiology; psychophysiological, psychometric and experimental studies; attempts at theoretical explanation and the relationship between MPS, hypnosis and dissociation. The author, a practising clinical psychologist and lecturer in psychopathology, gradually develops the hypothesis that MPS is best explained under the rubric of social role theory. It is argued that MPS is a culture-bound variant of hysterical psychosis occurring in individuals with high 'hypnotisability'. The tentative conclusion is that even if one accepts the reality of MPS it is unhelpful to regard it as a discrete clinical entity, and it is being grossly overdiagnosed.
