Si apre una nuova stagione per i piani strategici a livello aziendale in sanità, dopo che - per un decennio – il livello di interesse era stato quello dei sistemi regionali con i piani di rientro dal disavanzo sanitario. Questo testo prende in considerazione la relazione tra strategia e performance. Quest’ultima, sebbene vada sempre intesa in chiave multidimensionale, è trattata soltanto in termini di equilibrio tra costi e ricavi. Questa scelta non significa privilegiare unicamente gli equilibri di bilancio rispetto ad altre dimensioni di performance, ma il miglioramento dei risultati complessivi non può ottenersi senza riguardo anche all’equilibrio economico. La prima parte del testo concerne le necessità della prassi e i riferimenti teorici dell’attività strategica, nonché le principali determinanti per conseguire efficienze nel settore ospedaliero. La seconda parte tratta i temi di analisi e pianificazione, tanto gestionale (cd. operations) quanto economica, anche per determinare l’impatto delle azioni manageriali. La terza parte riguarda gli ospedali privati, interessati da pressioni regolatorie e competitive che spingono a modificare i modelli operativi e a riconfigurare i parametri delle determinanti del valore. Si è, pertanto, ritenuto opportuno trattare le loro caratteristiche e problematiche valutative, nonché i necessari adattamenti per stimarne il valore. Il testo è stato pensato, principalmente, per studenti, consulenti e practitioner.

Il libro ripercorre il percorso di definizione del modello di CDM, la sua implementazione in un panel di aziende ospedaliere e i principali risultati ottenuti; nello specifico: il percorso e i motivi che hanno portato alla definizione del progetto Academy dedicato al performance management delle aziende sanitarie (capitolo 1); la declinazione del Cruscotto Direzionale Multidimensionale, le sue finalità e il potenziale informativo dello strumento (capitolo 2); la definizione di un modello di Cruscotto per il Governo Clinico a presidio delle principali aree di attività sanitarie e di interesse delle Direzioni Sanitarie e Mediche di Presidio (capitolo 3); la formulazione di una proposta di Cruscotto per la Rendicontazione Sociale per i principali stakeholder dell’azienda ospedaliera (capitolo 4); le principali criticità legate all’implementazione di sistemi di misurazione delle performance e i possibili sviluppi futuri del processo di diffusione e implementazione del CDM e più in generale del progetto Academy (capitolo 5). Il libro è il frutto di un percorso durato oltre quattro anni con il coinvolgimento dei professionisti e del top management delle aziende ospedaliere partecipanti al progetto Academy of Health Care Management and Economics di SDA Bocconi, CeRGAS e Novartis Italia. L’Academy of Health Care Management and Economics è una partnership strategica tra SDA Bocconi School of Managment, CeRGAS dell’Università Bocconi e Novartis Italia che si propone sul panorama nazionale come laboratorio d’innovazione manageriale per le Aziende Sanitarie Pubbliche. Tra il 2010 e il 2015 un gruppo di Aziende Sanitarie Pubbliche rappresentative del panorama sanitario nazionale sono state coinvolte in attività di ricerca e formazione che hanno permesso al top management, ai professionisti sanitari e amministrativi di confrontarsi sui temi della strategia, del controllo e della valutazione delle performance.

Book excerpt: The authors of this book set out a system of safety strategies and interventions for managing patient safety on a day-to-day basis and improving safety over the long term. These strategies are applicable at all levels of the healthcare system from the frontline to the regulation and governance of the system. There have been many advances in patient safety, but we now need a new and broader vision that encompasses care throughout the patient’s journey. The authors argue that we need to see safety through the patient’s eyes, to consider how safety is managed in different contexts and to develop a wider strategic and practical vision in which patient safety is recast as the management of risk over time. Most safety improvement strategies aim to improve reliability and move closer toward optimal care. However, healthcare will always be under pressure and we also require ways of managing safety when conditions are difficult. We need to make more use of strategies concerned with detecting, controlling, managing and responding to risk. Strategies for managing safety in highly standardised and controlled environments are necessarily different from those in which clinicians constantly have to adapt and respond to changing circumstances. This work is supported by the Health Foundation. The Health Foundation is an independent charity committed to bringing about better health and health care for people in the UK. The charity’s aim is a healthier population in the UK, supported by high quality health care that can be equitably accessed. The Foundation carries out policy analysis and makes grants to front-line teams to try ideas in practice and supports research into what works to make people’s lives healthier and improve the health care system, with a particular emphasis on how to make successful change happen. A key part of the work is to make links between the knowledge of those working to deliver health and health care with research evidence and analysis. The aspiration is to create a virtuous circle, using what works on the ground to inform effective policymaking and vice versa. Good health and health care are vital for a flourishing society. Through sharing what is known, collaboration and building people’s skills and knowledge, the Foundation aims to make a difference and contribute to a healthier population.

A social protection system that includes a minimum basic level of protection for workers and citizens can complement new forms of employment. Improved private sector policies to encourage startup activity and competition can help countries compete in the digital age. Governments also need to ensure that firms pay their fair share of taxes, in part to fund this new social contract. The 2019 World Development Report presents an analysis of these issues based upon the available evidence.
