Book excerpt: Annotation Tuberculosis is still a significant health and economic problem in Central Asia, despite recent progress of the overall economic situation in the region. Over 50,000 new cases were detected in 2003, and over 7,000 people died due to tuberculosis in Kazakhstan, Kyrgyz Republic, Tajikistan, and Uzbekistan the four countries analyzed in this study. Stopping Tuberculosis in Central Asia examines the control efforts and financing of tuberculosis programs in the region. It also addresses the impending epidemiological crisis that has been posed by the sudden increase in cases of HIV/AIDS infection combined with a prevalent epidemic of tuberculosis. The authors find that it is highly unlikely that the Central Asian Republics will succeed in achieving the global targets for tuberculosis control in the short-term unless decisive action is taken.

Book excerpt: Tuberculosis (TB) and HIV/AIDS have a significant impact on the economy and society of Kazakhstan and a growing challenge to the country's health system. Together these two diseases could severely constrain the future economic growth and the human capital development of the country. To face the challenge of the dual threat of HIV/AIDS and TB focused, highly cost-effective programs will be required to ensure that limited resources produce the highest impact. To this end, this study addresses the overall performance of the tuberculosis, human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) programs in Kazakhstan. This paper provides a detailed analysis of the efficiency, quality, and effectiveness of each of the programs and identifies the structure of financing and cost-effectiveness. It focuses on the availability of resources to combat the diseases and the distribution of these resources within the country. (See related World Bank Technical Paper, "Stopping TB in Central Asia: Priorities for Action.") Also, the report identifies opportunities to improve performance, specifically analyzing the relationship between inputs and outcomes and highlighting, in the case of the TB Program, opportunities to produce significant savings or improved value-for-money in the program. The executive summary presents an overview of the study, summarizing the key findings and recommendations. The second section under each program then outlines the key issues and actions associated with the performance of the national programs. The key actions summarize recommendations on future areas of interest that would complement the findings of this report and deepen policymakers' understanding of how to improve the system.

Book excerpt: Between 2011 and 2019, WHO has developed and issued evidence-based policy recommendations on the treatment and care of patients with DR-TB. These policy recommendations have been presented in several WHO documents and their associated annexes, including the WHO Consolidated Guidelines on Drug Resistant Tuberculosis Treatment, issued by WHO in March 2019. The policy recommendations in each of these guidelines have been developed by WHO-convened Guideline Development Groups, using the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) approach to summarize the evidence, and formulate policy recommendations and accompanying remarks. The present WHO Consolidated Guidelines on Tuberculosis, Module 4: Treatment - Drug-Resistant Tuberculosis Treatment includes a comprehensive set of WHO recommendations for the treatment and care of DR-TB. The document includes two new recommendations, one on the composition of shorter regimens and one on the use of the BPaL regimen (i.e. bedaquiline, pretomanid and linezolid). In addition, the consolidated guidelines include existing recommendations on treatment regimens for isoniazid-resistant TB and MDR/RR-TB, including longer regimens, culture monitoring of patients on treatment, the timing of antiretroviral therapy (ART) in MDR/RR-TB patients infected with the human immunodeficiency virus (HIV), the use of surgery for patients receiving MDR-TB treatment, and optimal models of patient support and care. The guidelines are to be used primarily in national TB programmes, or their equivalents in Ministries of Health, and for other policy-makers and technical organizations working on TB and infectious diseases in public and private sectors and in the community.

Module 2: screening – systematic screening for tuberculosis disease is an updated and consolidated summary of WHO recommendations on systematic screening for tuberculosis (TB) disease, containing 17 recommendations for populations in which TB screening should be conducted and tools to be used for TB screening. TB screening is strongly recommendations for household and close contacts of individuals with TB, people living with HIV, miners exposed to silica dust, and prisoners. In addition, screening is conditionally recommended for people with risk factors for TB attending health care, and for communities with risk factors for TB and limited access to care (e.g. homeless, urban poor, refugees, migrants). General population screening is recommended in high-burden settings (0.5% prevalence or higher). Symptoms, chest radiography (CXR), and molecular WHO-recommended rapid diagnostic tests for TB are recommended as screening tools for all adults eligible for screening. Computer-aided detection programmes are recommended as alternatives to human interpretation of CXR in settings where trained personnel are scarce. For people living with HIV, C-reactive protein is also a good screening tool. This guideline document is accompanied by an operational handbook, the WHO operational handbook on tuberculosis. Module 2: screening – systematic screening for tuberculosis disease, that presents principles of screening, steps in planning and implementing a screening programme, and algorithm options for screening different populations.

The guidelines primarily target staff and medical practitioners working in TB treatment and control, and partners and organizations providing technical and financial support for care of drug-resistant TB in settings where resources are limited.

Book excerpt: WHO estimates that up to half a million new cases of multidrug-resistant tuberculosis (MDR-TB) occur worldwide, each year. Current treatment regimens for MDR-TB present many challenges: treatment lasts 20 months or more, requiring daily administration of drugs that are more toxic, less effective, and far more expensive than those used to treat drug-susceptible TB. Globally, less than half of all patients who start MDR-TB therapy are treated successfully. For the first time in over 40 years, a new TB drug with a novel mechanism of action - bedaquiline- is available, and was granted accelerated approval by the United States Food and Drug Administration in December 2012. There is considerable interest in the potential of this drug to treat MDR-TB. However, information about this new drug remains limited. It has only been through two Phase IIb trials for safety and efficacy. The World Health Organization (WHO) is therefore issuing "interim policy guidance". This interim guidance provides advice on the inclusion of bedaquiline in the combination therapy of MDR-TB in accordance with the existing WHO Guidelines for the Programmatic Management of Drug-resistant TB (2011 Update).
