This book consists of 10 chapters and begins by presenting an elementary account of the factors contributing to the interaction between a steroid hormone and another molecule. The discussion then turns to the three types of methods used to study the interaction between steroids and tissue receptors: cell fractionation, autoradiography, and the separation of specifically and non-specifically bound steroids. The next chapter deals with the metabolism of steroid hormones in relation to their binding and mechanism of action, focusing on testosterone and progesterone. The reader is also introduced to the intracellular binding of androgens, glucocorticoids, mineralocorticoids, and estrogens and to how binding is related to biological activity. It is far less important which surgical approach is selected for the larger group of patients who present with occult distant metastases. The whole subject of adjuvant therapy is still in its infancy. We have progressed from single-agent adjuvant chemotherapy to combined modality regimens consisting of combination chemotherapy plus im munotherapy, plus radiotherapy, plus endocrine therapy. It will un doubtedly take many years to sort out the proper use of these agents.

Numerous advances have occurred during this period. This book does not attempt to summarize past achievements. Rather, it deals with the contemporary issues in hGH research. A discussion of the present state of the art, of necessity, includes a review of the past. Some of the topics herein discussed include the following: 1. Growth hormone releasing factor (GRF). In 1973, the growth hormone inhibitory factor (somatostatin) had recently been discovered. The search for a releasing factor in humans led to its discovery not in the pituitary but in a pancreatic tumor that secreted growth hormone. The advances are discussed in this book. The current hope is that GRF will eventually become an effective therapeutic agent for idiopathic hypopituitarism in childhood and adolescence. 2. Biosynthesis of hGR by recombinant DNA technology. Current advances are discussed. Although hGH is not yet an approved drug, it will eventually become one. This will broaden our horizons in terms of hGH effectiveness in disorders other than hypopituitary dwarfism. The current experience with this type of hGH in both the Vnited States and Europe is reviewed by several authors.

The contributors to the volume are all noted scholars who are personally investigating these problems. The first chapter addresses the question, do hormones cause breast cancer? Segaloff provides us with a rational up-to-date overview of the existing data. He concludes that hormones by themselves are not tumor initiators but rather alter the hast environment so that other carcinogens are effective. lt is pointed out that the selection of the modeltest system is critical; one can almost assure any desired result by choosing an ap propriately biased test system. The question of the role of viruses in the etiology of human breast cancer remains unanswered despite elegant studies in mause systems.

It is only by comparing experimental results that it is possible to move forward, albeit slowly, towards a generally agreed consensus based upon objective scientific data. It is for this reason that we are particularly grateful to Ayerst Laboratories whose support and help have enabled us to turn our original proposal for an International Symposium into a reality. We are also grateful to the publishers for the efficiency with which they have organized the publication of the Proceedings. It is our hope and that of all the distinguished participants that all readers of this volume will be able to find something in it which will stimulate further thought and discussion - even though they may not necessarily agree with all the conclusions expressed - for the success of a Symposium turns not only on the subject under examination but also on the quality of debate and discussion it encourages.
