Book excerpt: The Composite Analysis (CA) performed for the Savannah River Site (SRS) in 2009 (SRS CA 2009) included a simplified uncertainty analysis. The uncertainty analysis in the CA (Smith et al. 2009b) was limited to considering at most five sources in a separate uncertainty calculation performed for each POA. To perform the uncertainty calculations in a reasonable amount of time, the analysis was limited to using 400 realizations, 2,000 years of simulated transport time, and the time steps used for the uncertainty analysis were increased from what was used in the CA base case analysis. As part of the CA maintenance plan, the Savannah River National Laboratory (SRNL) committed to improving the CA uncertainty/sensitivity analysis. The previous uncertainty analysis was constrained by the standard GoldSim licensing which limits the user to running at most four Monte Carlo uncertainty calculations (also called realizations) simultaneously. Some of the limitations on the number of realizations that could be practically run and the simulation time steps were removed by building a cluster of three HP Proliant windows servers with a total of 36 64-bit processors and by licensing the GoldSim DP-Plus distributed processing software. This allowed running as many as 35 realizations simultaneously (one processor is reserved as a master process that controls running the realizations). These enhancements to SRNL computing capabilities made uncertainty analysis: using 1000 realizations, using the time steps employed in the base case CA calculations, with more sources, and simulating radionuclide transport for 10,000 years feasible. In addition, an importance screening analysis was performed to identify the class of stochastic variables that have the most significant impact on model uncertainty. This analysis ran the uncertainty model separately testing the response to variations in the following five sets of model parameters: (a) K{sub d} values (72 parameters for the 36 CA elements in sand and clay), (b) Dose Parameters (34 parameters), (c) Material Properties (20 parameters), (d) Surface Water Flows (6 parameters), and (e) Vadose and Aquifer Flow (4 parameters). Results provided an assessment of which group of parameters is most significant in the dose uncertainty. It was found that K{sub d} and the vadose/aquifer flow parameters, both of which impact transport timing, had the greatest impact on dose uncertainty. Dose parameters had an intermediate level of impact while material properties and surface water flows had little impact on dose uncertainty. Results of the importance analysis are discussed further in Section 7 of this report. The objectives of this work were to address comments received during the CA review on the uncertainty analysis and to demonstrate an improved methodology for CA uncertainty calculations as part of CA maintenance. This report partially addresses the LFRG Review Team issue of producing an enhanced CA sensitivity and uncertainty analysis. This is described in Table 1-1 which provides specific responses to pertinent CA maintenance items extracted from Section 11 of the SRS CA (2009). As noted above, the original uncertainty analysis looked at each POA separately and only included the effects from at most five sources giving the highest peak doses at each POA. Only 17 of the 152 CA sources were used in the original uncertainty analysis and the simulation time was reduced from 10,000 to 2,000 years. A major constraint on the original uncertainty analysis was the limitation of only being able to use at most four distributed processes. This work expanded the analysis to 10,000 years using 39 of the CA sources, included cumulative dose effects at downstream POAs, with more realizations (1,000) and finer time steps. This was accomplished by using the GoldSim DP-Plus module and the 36 processors available on a new windows cluster. The last part of the work looked at the contribution to overall uncertainty from the main categories of uncertainty variables: K{sub d}s, dose parameters, flow parameters, and material properties. This was not intended to be a detailed sensitivity analysis but only to see in very general terms what broad category of parameters contributes most significantly to overall uncertainty in the CA dose. This analysis was not intended to be a final CA uncertainty calculation and there was no intent to revise the stochastic distributions as part of this work.

With development of surgical and endovascular techniques, treatment outcomes have been improved gradually. However, a lot of problems still require to be resolved. Aortic diseases in the region of visceral arteries are difficult to treat, especially for aortic dissection, which is usually associated with the narrow true lumen. Conventional open repair is associated with high trauma and high morbidity and mortality. Endovascular intervention has been applied in the last decade. Fenestration technique, chimney technique, and branched stent-grafts have been used for aortic aneurysms. For aortic dissection, management in the region of the visceral arteries is more complicated, and more techniques have been described. Besides these three methods, exclusion of the re-entry by vascular occluder, road block strategy, and candy-plug technique are also reported. However, these endovascular techniques vary in characteristics, procedures, and outcomes. There is no consensus on the optimal treatment for these complicated entities.

Book excerpt: Stereotactic Radiosurgery (SRS) is a common treatment method for multiple metastasis in the brain. SRS is prescribed to a high dose with low fractionation. Two properties of the dose distribution desired in SRS treatments are conformity and quick dose fall off. A common means of evaluating these properties is the Paddick Conformity Index, the Paddick Dose Gradient Index, and the RTOG Conformity Index. With the advances in multi-leaf collimators (MLCs) design, new means of treatment have been developed; this include Single Isocenter Multi-Target SRS (SIMT SRS). SIN'IT SRS differs from SRS in that it utilizes a single isocenter and treats all of the lesions in one plan. This single isocenter approach allows for a lower treatment time and less monitor units (MUs) delivered to the patient. Evaluating and comparing SINIT SRS plans can become difficult due to the fact that the isodose lines from one lesion, particularly the 50% isodose line, often intersects with another lesion. This complicates the analysis of both the dose fall off and conformity of each lesion. The purpose of this project is to develop a quantitative means to evaluate the conformity and dose fall off of an Single Isocenter Multi-Target SRS plan. For this project, a standard patient was created with seven randomized spherical targets varying in diameters (8mm-20mm). Good, intermediate, and bad plans were generated for each size target for this patient using Raystation v8A SPI. Each plan uses 0.1x0.1x0.1 cc dose grid, 9 non-coplanar arcs, the same arc angles, and a single isocenter. Each lesion was prescribed to 20 Gy for a single fraction To evaluate the dose distribution for each plan, the volume of 12 Gy (V12), volume of 10 Gy (VIO), and volume of 20 Gy (V20) in the brain were analyzed. These dose volumes were selected because an accumulation of these doses can lead to radionecrosis (RN) which has been used as a common indicator to evaluate the generated plans. [I] Two methods have been used to analyze the generated plans; the first analysis was done by combining the individual planning target volumes (P TVs) to one large PTV. Then the Paddick Conformity Index (PCI) and Paddick Dose Gradient Index (PGI) were applied to the combined dose statistics. The second method involved determining a defined volume to be applied to each target lesion. This volume was the exact area where the conformity and dose fall off were evaluated. The defined volume was determined by measuring from the centroid of a target to the 50% isodose line in multiple directions. The expansion volume was calculated to be within a 95% confidence interval of the mean for each size. Once the expansion volume was created, the previously mentioned indices were applied to it. It was observed that both the composite method and the expansion method are able to distinguish the difference in quality between plans. There seemed to be a trend occurring between target size and expansion volume; but, because only four expansion volumes were used in this study a conclusion cannot be drawn on this. It was noted that the PCI and PGI for the smaller lesions were significantly impacted by plan quality versus the larger lesions. Contrary to other studies, we found no observable relationship between target coverage and the distance from the treatment isocenter. Overall, the two methods proposed can be applied in a clinical setting various types of cases.
