Now the work is finished, and our book enters the literature as, we hope, a valuable contribution to understanding the terribly complex and subtle problem of the neuro biology of motivated behaviors. We would also like the book to stand as a personal mark of a cooperative relationship between father and son. This special relationship between the authors gave us an extra dimension of pleasure in writing the book, and it would delight us if it gave anyone else an extra dimension of enjoyment from reading it. One thing we hope happens is that anyone or simply considering entering similar considering a similar partnership, of this book as encouragement. Such re fields, will take the existence lationships are highly satisfying if both parties take care to protect the partnership. When we actually sat down to write the book, we were humbled by the immense literature and the smallness of both our conceived space for putting it down and of our brains for processing all the information.

In order to emphasize the main themes, essays on a given topic have been grouped together.Learning, Motivation, and Their Physiological Mechanisms begins when the author first discovered the thrill of designing and executing experiments to get clear-cut answers concerning the behavior of children and of rats. The first study was one of the earliest ones on the behavioral effects of the recently synthesized male hormone, testosterone. The second was one of the earliest studies demonstrating the value of using a variety of behavioral techniques to investigate the motivational effects of a physiological intervention. The next studies investigated the satisfying and rewarding effects of food or water in the stomach versus in the mouth and the thirst-inducing and reducing effects of hyper- and hypotonic solutions, respectively, injected into the brain. The last study describes a technique devised for extending the analysis of the mechanism of hunger to the effects of humoral factors in the blood.The study is completed with an examination of trial-and-error learning that was motivated by direct electrical stimulation of the brain and rewarded by the termination of such stimulation. Other studies show that the stimulation via such electrodes not only elicits eating, but also has the principal motivational characteristics of normal hunger. The conclusion deals with a series of experiments that overthrows strong traditional beliefs by proving that glandular and visceral responses mediated by the autonomic nervous system are subject to instrumental learning, which can be

In a sense this is the opposite of the question from which work on motivation began, and for which Claude Bernard invented the concept of the fixity of the internal milieu: How does an animal maintain constancy in the face of a fluctuating environment? Dealing with motivation has become extremely complex as new experiments, phenomena, and theories have extended the concept. This book embodies some of the ways in which work on motivation is currently proceeding. One of the major changes has been the recognition that motivation cannot be explained without an understanding of the biological rhythms and activational systems that underlie behavior. Another is that ecological and evolutionary perspectives add enormously to answering the central problem of why an animal does what it does when it does. The book suffers from several omissions. There is no chapter on the devel opment of motivated behavior. There is none on reward systems in the brain, owing to the untimely death of James Olds, whose contribution would have enriched this book appreciably, and to whom we dedicate it. Book excerpt: Deep brain stimulation (DBS) is a neurosurgical intervention whose efficacy, safety, and utility have been shown in the treatment of movement disorders. For the treatment of chronic pain refractory to medical therapies, many prospective case series have been reported, but few have published findings from patients treated during the past decade using current standards of neuroimaging and stimulator technology. We summarize the history, science, selection, assessment, surgery, and personal clinical experience of DBS of the ventral posterior thalamus, periventricular/periaqueductal gray matter, and, latterly, the rostral anterior cingulate cortex (Cg24) in 100 patients treated now at two centers (John Radcliffe Hospital, Oxford, UK, and Hospital de São João, Porto, Portugal) over 12 years. Several experienced centers continue DBS for chronic pain with success in selected patients, in particular those with pain after amputation, brachial plexus injury, stroke, and cephalalgias including anesthesia dolorosa. Other successes include pain after multiple sclerosis and spine injury. Somatotopic coverage during awake surgery is important in our technique, with cingulate DBS considered for whole-body pain or after unsuccessful DBS of other targets. Findings discussed from neuroimaging modalities, invasive neurophysiological insights from local field potential recording, and autonomic assessments may translate into improved patient selection and enhanced efficacy, encouraging larger clinical trials.
