Book excerpt: This synthesis report is based on three national studies on the evolution of the Ebola epidemic and its impact on Guinea, Liberia and Sierra Leone. Using computable general equilibrium (CGE) models, this report provides a more comprehensive assessment of the socio-economic impact of the epidemic and offers a more solid base to plan for recovery and medium term development efforts. The epidemic is disrupting the development progress achieved since the restoration of peace and democracy in the three most-affected countries. As of 10 December, almost 18,000 people had been infected and more than 6,400 had already died. Health services in Guinea, Liberia and Sierra Leone were not well equipped to fight the disease and the crisis is now completely outstripping their ability to stem its spread. Some specific features in the three countries have made Ebola particularly difficult to control. Lack of medical personnel and beds in Ebola Treatment Units, the complexity of identifying active cases and contacts, and the slowness of the response have all contributed to the seriousness of this health crisis. Finally, in the midst of the crisis, we must not lose sight of these countries' desperate need to re-set development, but on a more sustainable path. Evidence from this study shows that an increase in quality spending in health and development projects is a critical path to recovery. Governments and donors are understandably eager to devote as many resources as possible to containing the epidemic. But attention must still be given to how these economies can best recover and again achieve improvements in human welfare, once the disease has been contained.

Book excerpt: The most recent Ebola epidemic that began in late 2013 alerted the entire world to the gaps in infectious disease emergency preparedness and response. The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats. This publication summarizes the presentations and discussions from the workshop.

Since the previous round of data collection, in December 2014, Liberia continues to show progress toward getting to zero cases, and in the 21 days leading up to January 28, there were only 20 confirmed cases nationally, down from 92 in the 21 days ending on December 31, 2014, and a high of 546 in late August and early September. It is believed that all those currently infected are seeking care in Ebola Treatment Units (ETUs), some of which have begun to close in recent weeks as a result of declining need. The World Bank Group, with the Liberia Institute of Statistics and Geo-Information Services and the Gallup Organization, has continued to monitor the socio-economic impacts of EVD on households through a series of mobile-phone surveys conducted in October, November, and December 2014, and January 2015. As the health crisis continues to abate in Liberia, it will be crucial to tackle the important medium- and long-term economic and welfare impacts of Ebola on households. As Liberia continues on the path to eradicating Ebola, it will be important to identify and support those who are most vulnerable to the sluggish economy and whose long-term welfare may be negatively impacted by coping decisions made in the name of immediate stability. As households send their children back to school in the next month, the World Bank Group and partners will continue to monitor the crisis and work to support the Liberian government and its people as they move into economic recovery.

In addition to the number of patients infected with the SARS virus, the disease had profound economic and political repercussions in many of the affected regions. Recent reports of isolated new SARS cases and a fear that the disease could reemerge and spread have put public health officials on high alert for any indications of possible new outbreaks. This report examines the response to SARS by public health systems in individual countries, the biology of the SARS coronavirus and related coronaviruses in animals, the economic and political fallout of the SARS epidemic, quarantine law and other public health measures that apply to combating infectious diseases, and the role of international organizations and scientific cooperation in halting the spread of SARS. The report provides an illuminating survey of findings from the epidemic, along with an assessment of what might be needed in order to contain any future outbreaks of SARS or other emerging infections.

Despite a late arrival, the COVID-19 virus has spread rapidly across Sub-Saharan Africa in recent weeks. Economic growth in Sub-Saharan Africa is projected to decline from 2.4 percent in 2019 to -2.1 to -5.1 percent in 2020, the first recession in the region in 25 years. The coronavirus is hitting the region’s three largest economies —Nigeria, South Africa, and Angola— in a context of persistently weak growth and investment. In particular, countries that depend on oil and mining exports would be hit the hardest. The negative impact of the COVID-19 crisis on household welfare would be equally dramatic. African policymakers need to develop a two-pronged strategy of “saving lives and protecting livelihoods.†? This strategy includes relief measures and recovery measures aimed at strengthening health systems, providing income support to workers and liquidity support to viable businesses. However, financing of these policies will be challenging amid deteriorating fiscal positions and heightened public debt vulnerabilities. Therefore, African countries will require financial assistance from their development partners -including COVID-19 related multilateral assistance and a debt service stand still with creditors.
