Book excerpt: Abstract: The impacts of social network on the adoption of modern contraception are investigated using panel survey from Ghana. The data used in this paper were collected in six communities in southern Ghana. Women of reproductive age were interviewed in eight rounds over a five-year period (from late 1998 until early 2004). An analytic model using the contraceptive calendar data (n=35,366 woman-month contributed by 870 women) generated estimates to reveal the power of social network measures to predict the use of modern contraception in Southern Ghana. After controlling for the unobserved woman-specific factors through fixed-effects models, the social network measures still have their power in predicting contraceptive use. Related social diffusion measures are also taken into account including mass media exposure and geographic mobility. Based on the analysis of these results, I conclude that social learning and social influence are primarily two ways through which social networks affect modern contraceptive use.

This dissertation, in three chapters, will examine each of these pathways with the objective to further knowledge on modern fertility trends such as unintended pregnancy and low fertility. Chapter 2 uses intensive longitudinal data to examine the importance of pregnancy intendedness for the receipt of social support. Chapter 3 uses social network data from adolescent friendship networks to examine the role of social learning and contagion in contraceptive use and sexual risk. As adolescence is a time where individuals are at a high risk of experiencing unintended pregnancies or contraction of sexually transmitted infections, understanding peer effects are crucial to interpret the high, and increasing rates, of these two social problems. Chapter 4 uses uniquely detailed life history data to investigate the relationship between life course exposure to mortality and fertility preferences and behavior. These three chapters highlight the many intricate ways social networks are related to fertility.

Book excerpt: Considerable controversy exists among demographers, economists, and sociologists over the causes of fertility change in developing and developed countries. The neoclassical economic approach to fertility is embraced by its supporters because it facilitates the application of sophisticated consumer and household production theory to one of the most private and intimate questions: a couple's reproductive behavior. Despite the theoretical appeal of the economic approach, it has been eschewed by many critics because of its lack of social and institutional context, its neglect of cultural factors, and its requirement of 'rationality'. The integration of social interaction with economic fertility models in this book emerges as a powerful tool to overcome many of these criticisms. First, the analysis provides a formal integration of economic, sociological, and other approaches to fertility, and shows that there is a useful and promising agenda at the intersection of these schools. The second and more important goal is to sharpen the analytic lens with which theorists from different schools investigate fertility. For economists the work shows the advantages of moving beyond individual decision-making and embedding fertility decisions in a 'local environment' with interpersonal information flows, 'atmospheric' or social externalities, norms, and customs. For sociologists the work shows that theorizing about interactions within social networks can be more sophisticated. The implications of social networks depend substantially on the specific contexts and stages of the demographic transition, and these differences can be used to empirically distinguish between social learning and social influence. Thirdly, the findings have important implications for population policy. The analyses in this book indicate when family planning is likely to diffuse and lead to rapid adoption of birth control, and they derive conditions where Pareto-improving policy measures are likely to exist.

Children born from unplanned conception are at greater risk of low birth weight, of being abused, and of not receiving sufficient resources for healthy development. A wider range of contraceptive options is needed to address the changing needs of the populations of the world across the reproductive life cycle, but this unmet need has not been a major priority of the research community and pharmaceutical industry. New Frontiers in Contraceptive Research: A Blueprint for Action, a new report from the Institute of Medicine of the National Academies, identifies priority areas for research to develop new contraceptives. The report highlights new technologies and approaches to biomedical research, including genomics and proteomics, which hold particular promise for developing new products. It also identifies impediments to drug development that must be addressed. Research sponsors, both public and private, will find topics of interest among the recommendations, which are diverse but interconnected and important for improving the range of contraceptive products, their efficacy, and their acceptability.

The marked declines in fertility seen in some developing nations have been accompanied by slowing population growth, which in turn provided a window of opportunity for rapid economic growth. For many sub-Saharan African nations, this window has not yet opened because fertility rates have not declined as rapidly there as elsewhere. Fertility rates in many sub-Saharan African countries are high: the total rate for the region is estimated to be 5.1 births per woman, and rates that had begun to decline in many countries in the region have stalled. High rates of fertility in these countries are likely to contribute to continued rapid population growth: the United Nations projects that the region's population will increase by 1.2 billion by 2050, the highest growth among the regions for which there are projections. In June 2015, the Committee on Population organized a workshop to explore fertility trends and the factors that have influenced them. The workshop committee was asked to explore history and trends related to fertility, proximate determinants and other influences, the status and impact of family planning programs, and prospects for further reducing fertility rates. This study will help donors, researchers, and policy makers better understand the factors that may explain the slow pace of fertility decline in this region, and develop methods to improve family planning in sub-Saharan Africa.
